Humans have both an amazing capacity to remember and a tremendously limited capacity depending upon which phase of the memory process. Our long-term store of information is virtually unlimited. On the other hand, short-term memory is extremely limited. This is called our working memory and it can contain only about 5 to 9 pieces of information at a time. This fact of a limited working memory store shapes a great deal about how we think, summarize, and categorize our world.

 

Memory ability is so important to everyday human functioning that it is important to study ways to maintain or improve it. Short-term, working, memory can be improved. Mindfulness has been shown to improve working memory capacity. Yoga practice has also been shown to have improve memory and reduce the decline in memory ability that occurs with aging. In addition Tai Chi practice has also been shown to improve memory. These effects are well established in adults but have not been explored in adolescents. It is thus important to study the detailed effects of mindful movement practice on working memory ability in adolescents.

 

In today’s Research News article “Meditative Movement Affects Working Memory Related to Neural Activity in Adolescents: A Randomized Controlled Trial.” (See summary below or view the full text of the study at: https://www.frontiersin.org/articles/10.3389/fpsyg.2020.00931/full?utm_source=F-AAE&utm_medium=EMLF&utm_campaign=MRK_1332835_69_Psycho_20200519_arts_A), Kang and colleagues recruited healthy adolescents, aged 17-18 years, who were naïve to meditative or mindful movement practices. They were randomly assigned to either a relaxation control condition or to receive training in meditative movement. Relaxation and meditative movements were practiced for 9 minutes twice a day for 3 weeks.

 

Before and after training they were measured for working memory while simultaneously having their electroencephalogram (EEG) recorded. They were presented with a sequence of audio and visual targets and had to recall the target presented a number of positions back in the series, the further back they could successfully go, the better the working memory.

 

They found that in comparison to baseline and the relaxation group, the group that performed meditative movements had a significantly greater improvement in working memory, being able to recall targets further back in the sequence. They also found for the meditative movement group but not the relaxation group that the better the working memory score the lower the power of the high beta frequency (30-40 hz. Waves per second) in the EEG over the dorsolateral prefrontal cortex.

 

These results suggest that meditative movement practice improves working (short-term) memory and that this improvement was related to changes in the EEG. The high beta frequency in the EEG has been shown to be related to filtering irrelevant information in working memory and thereby to improve working memory. So, the EEG data are compatible with the memory data suggesting better brain processing of working memory underlay the better working memory.

 

The study did not have a follow-up beyond the time immediately after the 3-week training period. It would be important in future research to investigate the duration of the effect by having follow-up measurements at delayed intervals. Also, the control condition in the present study was sedentary while the meditative movement training was active. It would be important for future research to include an active control perhaps performing a non-meditative exercise.

 

Regardless, the study demonstrates that improved working memory results from meditative movement practice in adolescents as has previously been found for adults. The improvement in working memory is important particularly for adolescents who are in a very active learning period of their lives. Working memory is the foundation for all memory and as such improvement would be important for their academic learning. It remains to be seen if meditative movement improves scholastic performance.

 

So, improve working memory in adolescents with mindful movement.

Numerous studies have revealed that meditative movement changes brain activity and improves the cognitive function of adults. However, there is still insufficient data on whether meditative movement contributes to the cognitive function of adolescents whose brain is still under development. Therefore, this study aimed to uncover the effects of meditative movement on the cognitive performance and its relation with brain activity in adolescents. Forty healthy adolescent participants (mean age of 17∼18) were randomly allocated into two groups: meditative movement and control group. The meditative movement group was instructed to perform the meditative movement, twice a day for 9 min each, for a duration of 3 weeks. During the same time of the day, the control group was instructed to rest under the same condition. To measure changes in cognitive abilities, a dual n-back task was performed before and after the intervention and analyzed by repeated two-way analysis of variance (ANOVA). During the task, electroencephalogram signals were collected to find the relation of brain activity with working memory performance and was analyzed by regression analysis. A repeated two-way ANOVA with Bonferroni correction showed that working memory performance was significantly increased by meditative movement compared with the retest effect. Based on regression analysis, the amplitude of high-beta rhythm in the F3 channel showed a significant correlation with dual n-back score in the experimental group after the intervention, while there was no correlation in the control group. Our results suggest that meditative movement improves the performance of working memory, which is related to brain activity in adolescents.

Chronic Obstructive Pulmonary Diseases (COPD) are progressive lung diseases that obstruct airflow. The two main types of COPD are chronic bronchitis and emphysema. COPD is very serious being the third leading cause of death in the United States, over 140,000 deaths per year and the number of people dying from COPD is growing. More than 11 million people have been diagnosed with COPD, but an estimated 24 million may have the disease without even knowing it. COPD causes serious long-term disability and early death.

 

There is no cure for COPD. Treatments include lifestyle changes, medicine, bronchodilators, steroids, pulmonary rehabilitation, oxygen therapy, and surgery. They all attempt to relieve symptoms, slow the progress of the disease, improve exercise tolerance, prevent and treat complications, and improve overall health. Gentle mind-body exercise such as Yoga, Tai Chi and Qigong practices could improve COPD symptoms. Yoga has been shown to improve exercise tolerance and overall health and includes breathing exercises. Indeed, it has been shown that yoga practice improves the mental and physical health of patients with COPD. Mindful movement practices such Tai Chi and Qigong are ancient Chinese practices involving mindfulness and gentle movements. They are easy to learn, safe, and gentle. So, it may be appropriate for patients with COPD who lack the ability to engage in strenuous exercises to engage in these gentle mind-body practices.

 

In today’s Research News article “Effects of Tai Chi training on the physical and mental health status in patients with chronic obstructive pulmonary disease: a systematic review and meta-analysis.” (See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC7139037/), Guo and colleagues review, summarize, and perform a meta-analysis of the published research studies on the effectiveness of Tai Chi practices for the treatment of Chronic Obstructive Pulmonary Diseases (COPD).

 

They found 16 published research studies that report that Tai Chi practice results in improvements in lung function including forced volume capacity, forced expiratory volume in 1 s, and degree of lung function recovery. It also increased exercise ability as measured by the distance walked in 6 minutes, improved mood by decreasing anxiety and depression, and improved the patient’s quality of life.

 

The results are striking and important. Tai Chi practice improves the lung function, exercise capacity, mood, and quality of life in patients with Chronic Obstructive Pulmonary Diseases (COPD). It is safe and effective mindfulness practice. It is appropriate for all ages including the elderly and for individuals with illnesses that limit their activities or range of motion. It is inexpensive to administer, can be performed in groups or alone, at home or in a facility, and can be quickly learned. In addition, it can be practiced in social groups. This can make it fun, improving the likelihood of long-term engagement in the practice. Hence, Tai Chi practice would appear to be an excellent practice to be added to routine treatment of COPD.

 

So, improve the physical and mental health of patients with COPD with Tai Chi.

Abstract

Background

Tai Chi is a systematic whole body movement developed in ancient China. It plays an increasingly important role in the field of pulmonary rehabilitation for patients with chronic obstructive pulmonary disease (COPD). Our review aimed to explore the impact of Tai Chi on the physical and mental health of patients with COPD.

Methods

We searched several English and Chinese databases and used the combination of subject words and free words to search for available literature from the establishment of the library until August 28, 2018. Two researchers screened studies and collected the data independently. The study inclusion criteria included: (I) patients diagnosed with COPD; (II) Tai Chi or Tai Chi Qigong as an intervention in addition to routine treatment; (III) routine treatment with or without exercises as control group. The primary outcomes were lung function, exercise capacity and health status; (IV) randomized controlled trials.

Results

Sixteen articles were included from 2009 to 2018 (n=1,096). The average time duration of Tai Chi program was 53.4 minutes each session, 4.13 sessions a week for a total of 4.13 months. Comparing with control group, Tai Chi group improved some lung function (forced volume capacity: mean difference =0.12, 95% CI: 0.03–0.21), (forced expiratory volume in 1s: mean difference =0.15, 95% CI: 0.08–0.21), enhanced 6-minute walking distance score (mean difference =30.78, 95% CI: 15.15–46.42), decreased COPD Assessment Test score (mean difference =−5.00, 95% CI: −7.51 to −2.50), decreased St. George’s Respiratory Questionnaire score (mean difference =−8.66, 95% CI: −14.60 to −2.72), enhanced Chronic Respiratory Disease Questionnaire score (mean difference =2.16, 95% CI: 1.49–2.83), decreased Hospital Anxiety and Depression Scale score(anxiety: mean difference =−1.04, 95% CI: −1.58 to −0.51; depression: mean difference =−1.25, 95% CI: −1.77 to −0.73). Comparing with exercise group, Tai Chi group statistically enhanced 6-minute walking distance score (mean difference =7.77, 95% CI: 2.63–12.91).

Conclusions

Tai Chi may represent an appropriate alternative or complement to standard rehabilitation programs. However, whether Tai Chi is better than pulmonary rehabilitation exercise has not been determined.

In today’s Research News article “Mind-Body Exercise for Anxiety and Depression in COPD Patients: A Systematic Review and Meta-Analysis.” (See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6981896/), Li and colleagues review, summarize, and perform a meta-analysis of the published research studies of the effectiveness of mind-body practices on the symptoms of Chronic Obstructive Pulmonary Diseases (COPD).

 

They found 13 peer-reviewed randomized controlled trials; 7 employing Qigong, 3 Tai Chi, and 3 yoga. They report that the published research found that mind-body practices produced significant reductions in anxiety and depression in patients with Chronic Obstructive Pulmonary Diseases (COPD).

 

Mindfulness practices, in general have been found to reduce anxiety and depression. The present review extends this effectiveness to mind-body mindfulness practices with patients with COPD. The mechanisms by which these practices produce these effects are not known. But all these practices involve focusing on the present moment. Anxiety is produced by fear of the future while depression is produced by rumination about the past. While focusing on the present, anxiety and depression are eliminated. Obviously, training does not eliminate thinking about the past and future. But, it may reduce the amount of time spent outside the present moment and thereby reduce the overall levels of anxiety and depression.

Abstract

Objectives: Mind–body exercise has been generally recognized as a beneficial strategy to improve mental health in those with Chronic Obstructive Pulmonary Disease (COPD). However, to date, no attempt has been made to collate this literature. The aim of the present study was to systematically analyze the effects of mind–body exercise for COPD patients with anxiety and depression and provide scientific evidence-based exercise prescription. Methods: both Chinese and English databases (PubMed, the Cochrane Library, EMBASE, Web of Science, Google Scholar, Chinese National Knowledge Infrastructure, Wanfang, Baidu Scholar) were used as sources of data to search randomized controlled trials (RCT) relating to mind–body exercise in COPD patients with anxiety and depression that were published between January 1982 to June 2019. 13 eligible RCT studies were finally used for meta-analysis. Results: Mind–body exercise (tai chi, health qigong, yoga) had significant benefits on COPD patients with anxiety (SMD = −0.76, 95% CI −0.91 to −0.60, p = 0.04, I2 = 47.4%) and depression (SMD = −0.86, 95% CI −1.14 to −0.58, p = 0.000, I2 = 71.4%). Sub-group analysis indicated that, for anxiety, 30–60 min exercise session for 24 weeks of health qigong or yoga had a significant effect on patients with COPD who are more than 70 years and have more than a 10-year disease course. For depression, 2–3 times a week, 30–60 min each time of health qigong had a significant effect on patients with COPD patients who are more than 70 years old and have less than a 10-year disease course. Conclusions: Mind–body exercise could reduce levels of anxiety and depression in those with COPD. More robust RCT are required on this topic.

Cardiovascular disease is the number one killer, claiming more lives than all forms of cancer combined. Lifestyle changes have proved to be quite effective in reducing the risk of cardiovascular disease. These include quitting smoking, weight reduction, improved diet, physical activity, and reducing stresses. Contemplative practices, such as meditation, tai chi, and yoga, have also been shown to be helpful for heart health. These practices have also been shown to be helpful for producing the kinds of other lifestyle changes needed such as smoking cessationweight reduction and stress reduction.

 

Tai Chi and Qigong are ancient mindfulness practices involving slow prescribed movements. They are gentle and completely safe, can be used with the elderly and sickly, are inexpensive to administer, can be performed in groups or alone, at home or in a facility or even public park, and can be quickly learned. In addition, they can also be practiced in social groups without professional supervision. This can make it fun, improving the likelihood of long-term engagement in the practice. Since Tai Chi and Qigong are both mindfulness practices and exercises, they may be an acceptable and effective method to reduce the risk of cardiovascular disease.

 

In today’s Research News article “Qigong for the primary prevention of cardiovascular disease.” (See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6956616/), Hartley and colleagues review, summarize, and perform a meta-analysis of the published research literature on the effectiveness of Qigong practice to reduce the risk of cardiovascular disease. They identified 12 published randomized controlled trials with a total of 1369 participants.

 

They report that in a couple of studies that followed up 20 to 30 years after Qigong training that there was a significant reduction in all-cause mortality. They also report that Qigong training produced significant reductions in systolic blood pressure and blood fat levels including total cholesterol, triglycerides, and low-density lipoproteins. They found that the published trials, though, had significant risks of bias.

 

The primary conclusion was that larger better controlled trials are needed. But the results are promising and suggest that Qigong training reduced risk factors for cardiovascular disease. This gentle exercise may be an effective treatment to change lifestyle, increasing exercise, decreasing the likelihood of cardiovascular disease, and maybe increasing longevity.

 

So, improve cardiovascular function with Qigong.

Abstract

Background

Two major determinants of cardiovascular disease (CVD) are a sedentary lifestyle and stress. Qigong involves physical exercise, mind regulation and breathing control to restore the flow of Qi (a pivotal life energy). As it is thought to help reduce stress and involves exercise, qigong may be an effective strategy for the primary prevention of CVD.

Objectives

To determine the effectiveness of qigong for the primary prevention of CVD.

Search methods

We searched the following electronic databases: the Cochrane Central Register of Controlled Trials (CENTRAL) (November 2014, Issue 10 of 12); MEDLINE (Ovid) (1946 to 2014 October week 4); EMBASE Classic + EMBASE (Ovid) (1947 to 2014 November 4); Web of Science Core Collection (1970 to 31 October 2014); Database of Abstracts of Reviews of Effects (DARE), Health Technology Assessment Database and Health Economics Evaluations Database (November 2014, Issue 4 of 4). We searched several Asian databases (inception to July 2013) and the Allied and Complementary Medicine Database (AMED) (inception to December 2013), as well as trial registers and reference lists of reviews and articles; we also approached experts in the field and applied no language restrictions in our search.

Selection criteria

Randomised controlled trials lasting at least three months involving healthy adults or those at high risk of CVD. Trials examined any type of qigong, and comparison groups provided no intervention or minimal intervention. Outcomes of interest included clinical CVD events and major CVD risk factors. We did not include trials that involved multi‐factorial lifestyle interventions or weight loss.

Data collection and analysis

Two review authors independently selected trials for inclusion. Two review authors extracted data from included studies and assessed the risk of bias.

Main results

We identified 11 completed trials (1369 participants) and one ongoing trial. Trials were heterogeneous in participants recruited, qigong duration and length of follow‐up periods. We were unable to ascertain the risk of bias in nine trials published in Chinese, as insufficient methodological details were reported and we were unable to contact the study authors to clarify this.

We performed no meta‐analyses, as trials were small and were at significant risk of bias. Clinical events were detailed in subsequent reports of two trials when statistically significant effects of qigong were seen for all‐cause mortality, stroke mortality and stroke incidence at 20 to 30 years after completion of the trials. However, these trials were designed to examine outcomes in the short term, and it is not clear whether qigong was practised during extended periods of follow‐up; therefore effects cannot be attributed to the intervention. None of the included studies reported other non‐fatal CVD events.

Six trials provided data that could be used to examine the effects of qigong on blood pressure. Reductions in systolic blood pressure (SBP) and diastolic blood pressure (DBP) were seen in three and two trials, respectively. Three trials examined the effects of qigong on blood lipids when favourable effects were seen in one trial for total cholesterol, low‐density lipoprotein (LDL) cholesterol and triglycerides, and two trials showed favourable effects on high‐density lipoprotein (HDL) cholesterol. The only trial considered at low risk of selection and detection bias did not demonstrate statistically significant effects on CVD risk factors with qigong, but this study was small and was underpowered. None of the included studies reported incidence of type 2 diabetes (T2D), adverse events, quality of life or costs.

Authors’ conclusions

Currently, very limited evidence is available on the effectiveness of qigong for the primary prevention of CVD. Most of the trials included in this review are likely to be at high risk of bias, so we have very low confidence in the validity of the results. Publication of the ongoing trial will add to the limited evidence base, but further trials of high methodological quality with sufficient sample size and follow‐up are needed to be incorporated in an update of this review before the effectiveness of qigong for CVD prevention can be established.

The process of aging affects every aspect of the physical and cognitive domains. Every system in the body deteriorates including motor function with a decline in strength, flexibility, and balance. Impaired balance is a particular problem as it can lead to falls. In the U.S. one third of people over 65 fall each year and 2.5 million are treated in emergency rooms for injuries produced by falls. About 1% of falls result in deaths making it the leading cause of death due to injury among the elderly. It is obviously important to discover methods to improve balance and decrease the number of falls in older adults.

 

Tai Chi training is designed to enhance and regulate the functional activities of the body through regulated breathing, mindful concentration, and gentle movements. It includes balance training and has been shown to improve balance and coordination. Indeed, Tai Chi training has been shown to reduce cognitive decline and to reduce the frequency of falls in the elderly.

 

In today’s Research News article “Correlation Between Cognition and Balance Among Middle-Aged and Older Adults Observed Through a Tai Chi Intervention Program.” (See summary below or view the full text of the study at: https://www.frontiersin.org/articles/10.3389/fpsyg.2020.00668/full?utm_source=F-AAE&utm_medium=EMLF&utm_campaign=MRK_1302118_69_Psycho_20200416_arts_A), Xiao and colleagues recruited middle age and older health adults (average age of 59 years). They received Tai Chi training for 1 hour, 3 days per week, for 12 weeks. They were measured before and after training for global cognitive function, static and dynamic balance, body size, lower limb strength, and aerobic endurance.

 

They found in comparison to baseline after Tai Chi training there were significant increases in global cognitive function, and static and dynamic balance. Also, they found that before training the higher the levels of both dynamic and static balance the higher the levels of cognitive function. In addition, they found that the greater the increase in both static and dynamic balance after Tai Chi training, the greater the increase in cognitive function. These improvements were found to be related to increases in lower limb strength.

 

These results are interesting as both balance and cognition decline with age. Tai Chi training has been shown in prior research to reduce the decline in both with aging. To my knowledge this is the first time that these improvements appear to be linked where improvements in balance co-occur with improvements in balance. These results suggest that Tai Chi training in older adults may delay cognitive decline and also delay decline in balance. This may tend to reduce the likelihood of falls and precipitous loss of cognitive ability.

 

These findings support the use of Tai Chito improve balance and cognition in older adults. Some advantages of Tai Chi include the facts that it is not strenuous, involves slow gentle movements, and is safe, having no appreciable side effects, it is appropriate for all ages including the elderly and for individuals with illnesses that limit their activities or range of motion. It can also be practiced without professional supervision and in groups making it inexpensive to deliver and fun to engage in. This makes Tai Chi practice an excellent means to improve balance and reduce falls and delay cognitive decline in older individuals.

Abstract

Background: Age-associated decline in cognition and balance may cause severe ability loss for daily living activities among middle-aged and older adults. The relationship between cognition and balance in this aging population remains to be explored.

Objective: The present study Is exploratory in nature and aimed to examine the relationship between balance (both static and dynamic components) and global cognitive function among middle-aged and older adults through Tai Chi (TC) practice as a research avenue.

Methods: A short-term (12 weeks) intervention of TC was conducted among middle-aged and older adults in the community setting. Global cognitive function (using the Chinese version of the Montreal Cognitive Assessment score (MoCA) and balance (i.e., one leg standing test score; Timed Up and Go Test score, TUGT) of all participants were assessed before and after the intervention. Age, body mass index (BMI), sex, and physical fitness variables (Chair Stand Test, CST; the 6-Meter Walk Test, 6MWT) were also collected as confounding factors.

Results: Significant moderator effects of baseline CST on the association between the dichotomized baseline MoCA score and the baseline left leg balance score (p = 0.0247), the baseline right leg balance score (p = 0.0140) and the baseline TUGT score (p = 0.0346) were found. Change score of left score balance (p = 0.0192) and change score of TUGT (p = 0.0162) were found to be significantly associated with change score of cognitive function.

Conclusion: Cognitive function and balance are interrelated in middle-aged and older adults. The association between global cognitive function and balance Is moderated by strength of lower limbs. The change scores of cognitive function and balance introduced by TC training were found to be positively correlated. Future research Is warranted to further confirm the cause-effect relationship of cognitive function and balance and its influencing factors among middle-aged and older adults utilizing intervention studies with larger sample sizes.

Cardiovascular disease is the number one killer, claiming more lives than all forms of cancer combined. There are myriads of treatments that have been developed to treat cardiovascular disease including a variety of surgical procedures and medications. Importantly, lifestyle changes have proved to be quite effective. These include quitting smoking, weight reduction, improved diet, physical activity, and reducing stresses. Contemplative practices, such as meditation, tai chi, and yoga, have also been shown to be helpful for heart health. These practices have also been shown to be helpful for producing the kinds of other lifestyle changes needed such as smoking cessationweight reduction and stress reduction.

 

Cardiac rehabilitation programs for patients recovering from implantation of a stent for coronary heart disease, emphasize lifestyle changes. Unfortunately, for a variety of reasons, 60% of coronary heart disease patients decline participation, making these patients at high risk for a heart attack.  Tai Chi and Qigong are ancient mindfulness practices involving slow prescribed movements. They are gentle and completely safe, can be used with the elderly and sickly, are inexpensive to administer, can be performed in groups or alone, at home or in a facility or even public park, and can be quickly learned. In addition, they can also be practiced in social groups without professional supervision. This can make it fun, improving the likelihood of long-term engagement in the practice. Since Tai Chi is both a mindfulness practice and a gentle exercise, it may be an acceptable, safe, and effective treatment for coronary heart disease patients.

 

In today’s Research News article “The 24-Form Tai Chi Improves Anxiety and Depression and Upregulates miR-17-92 in Coronary Heart Disease Patients After Percutaneous Coronary Intervention.” (See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC7078346/), Liu and colleagues recruited coronary heart disease patients who had a myocardial infarction and a stent implanted less than 4 days prior. The patients received usual care and were randomly assigned to receive either no further treatment or Tai Chi practice twice a day for 60 minutes for 10 months. They were measured before and after the 10-month practice period for anxiety, depression, perceived stress, and quality of life. In addition, blood was drawn and measured for miR-17-92.

 

They found that in comparison to baseline and the control group, the group that received Tai Chi training had significantly lower levels of anxiety, depression, and perceived stress and significantly higher levels of quality of life and miR-17-92. In addition, they found that the higher the levels of miR-17-92 the lower the levels of anxiety, depression, and perceived stress.

 

It should be noted that the control condition was not active. Future research should have an active control that performed some other activity of comparable duration, perhaps another type of exercise or health education. Nevertheless, the results show that Tai Chi practice can significantly improve the psychological well-being of coronary heart disease patients after surgery to insert a stent.

 

Prior research has demonstrated the RNA segments are associated with anxiety and depression. The current research also detected this relationship. But the study also demonstrated that the improvements in anxiety, depression, and perceived stress were associated with increased levels of miR-17-92. This may indicate a mechanism of action by which Tai Chi practice improves psychological well-being. It remains for future research to further explore this interesting possibility.

 

So, reduce anxiety and depression in coronary heart disease patients with Tai Chi.

Abstract

Background

Anxiety and depression are common symptoms in patients with coronary heart disease (CHD) after percutaneous coronary intervention (PCI). The 24-form Tai Chi may exert a protective function for CHD patients after PCI by improving anxiety and depression.

Methods

Patients who received PCI after 1–4 days were randomly assigned to the 24-form Tai Chi group (TG) and the control group (CG). The differences in anxiety and depression, using the Medical Outcomes Study 36−item Short−Form Health Survey (SF-36), before and after an average of 10 months of Tai Chi intervention were compared in both groups to analyze the effects of Tai Chi on the emotion and the life quality of CHD patients. Meanwhile, the relative levels of miR-17-92 were measured by using real-time qPCR. The association between the relative levels of miR-17-92 and the anxiety and the depression of CHD patients after PCI was analyzed. Adjusted Cox models were used to explore the effect of Tai Chi exercise in CHD patients.

Results

After 10 months of intervention, the changes in the anxiety subscale (P = 0.002), in the depression subscale (P = 0.008), and in the stress (P = 0.015) scores were higher in the TG group when compared to those of the CG group. The proportion of anxious (P = 0.045) and depressed subjects (P = 0.042) in the TG group was lower than that in the CG group. On the other hand, the increase in the SF-36 scores and in the relative levels of miR-17-92 was significantly higher in the TG group when compared with that of the CG group (P < 0.05). The serum level of miR-17-92 had a negative correlation with the anxiety, the depression, and the stress scores (P < 0.01).

Conclusion

The 24-form Tai Chi improved the anxiety and the depression symptoms and upregulated the miR-17-92 levels in CHD patients after PCI.

Parkinson’s Disease (PD) is an incurable progressive degenerative disease of the central nervous system. The condition is caused by the death of nerve cells in the brain that produce the neurotransmitter dopamine. There are around seven million people worldwide and one million people in the U.S. living with PD and about 60,000 people are diagnosed with PD every year. PD is associated with aging as the vast majority of patients are diagnosed after age 50. In fact, it has been speculated that everyone would eventually develop PD if they lived long enough.

 

Its physical symptoms include resting tremor, slow movements, muscle rigidity, problems with posture and balance, loss of automatic movements, and slurring of speech. PD itself is not fatal but is often associated with related complications which can reduce life expectancy, such as falls, choking, and cardiovascular problems. Parkinson’s Disease (PD) also has psychological effects, especially cognitive decline, anxiety, and depression. All of these symptoms result in a marked reduction in the quality of life.

 

There are no cures for Parkinson’s Disease or even treatments to slow its progression. There are only treatments that can produce symptomatic relief. So, there is a need to discover new and different treatments. Mindfulness training has been found to improve the psychological symptoms and the quality of life with PD patients.  In addition, Tai Chi practice has been shown to improve the symptoms of Parkinson’s Disease. Hence, mind-body practices may be excellent treatments for the symptoms of Parkinson’s Disease.

 

In today’s Research News article “The Impact of Mind-body Exercises on Motor Function, Depressive Symptoms, and Quality of Life in Parkinson’s Disease: A Systematic Review and Meta-analysis.” (See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6981975/), Jin and colleagues review, summarize, and perform a meta-analysis of the effectiveness of mind-body practices for the relief of the symptoms of Parkinson’s Disease. They selected randomized controlled trials with Tai Chi, Qigong and Yoga practices for patients with Parkinson’s Disease over 40 years of age. They identified 22 studies with a total of 1199 participants, 18 of which employed Tai Chi and Qigong practices and 4 employed Yoga practice.

 

They report that the studies found that the mind-body practices produced significant improvements in overall Parkinson’s Disease motor function, walking ability, balance, depression, and quality of life. Hence, the published research studies demonstrate that mind-body practices significantly improve the physical and psychological symptoms of Parkinson’s Disease.

 

Tai Chi and Qigong practices have been demonstrated in prior research to improve balance, walking ability, depression, and quality of life in a variety of healthy and sick people. In addition yoga practice has been demonstrated to improve balance, walking ability, depression and quality of life in various populations. The present study extends these findings to patients with Parkinson’s Disease. These practices appear to be a safe and effective treatment to relieve the symptoms and suffering of patients with Parkinson’s Disease.

Abstract

Purpose: To systematically evaluate the effects of mind-body exercises (Tai Chi, Yoga, and Health Qigong) on motor function (UPDRS, Timed-Up-and-Go, Balance), depressive symptoms, and quality of life (QoL) of Parkinson’s patients (PD). Methods: Through computer system search and manual retrieval, PubMed, Web of Science, The Cochrane Library, CNKI, Wanfang Database, and CQVIP were used. Articles were retrieved up to the published date of June 30, 2019. Following the Cochrane Collaboration System Evaluation Manual (version 5.1.0), two researchers independently evaluated the quality and bias risk of each article, including 22 evaluated articles. The Pedro quality score of 6 points or more was found for 86% (19/22) of these studies, of which 21 were randomized controlled trials with a total of 1199 subjects; and the trial intervention time ranged from 4 to 24 weeks. Interventions in the control group included no-intervention controls, placebo, waiting-lists, routine care, and non-sports controls. Meta-analysis was performed on the literature using RevMan 5.3 statistical software, and heterogeneity analysis was performed using Stata 14.0 software. Results: (1) Mind-body exercises significantly improved motor function in PD patients, including UPDRS (SMD = −0.61, p < 0.001), TUG (SMD = −1.47, p < 0.001) and balance function (SMD = 0.79, p < 0.001). (2) Mind-body exercises also had significant effects on depression (SMD = −1.61, p = 0.002) and QoL (SMD = 0.66, p < 0.001). (3) Among the indicators, UPDRS (I2 = 81%) and depression (I2 = 91%) had higher heterogeneity; according to the results of the separate combined effect sizes of TUG (I2 = 29%), Balance (I2 = 16%) and QoL (I2 = 35%), it shows that the heterogeneity is small; (4) After meta-regression analysis of the age limit and other possible confounding factors, further subgroup analysis showed that the reason for the heterogeneity of UPDRS motor function may be related to the sex of PD patients and severity of the disease; the outcome of depression was heterogeneous. The reason for this may be the use of specific drugs in the experiment and the duration of intervention in the trial. Conclusion: (1) Mind-body exercises were found to have significant improvements in motor function, depressive symptoms, and quality of life in patients with Parkinson’s disease, and can be used as an effective method for clinical exercise intervention in PD patients. (2) Future clinical intervention programs for PD patients need to fully consider specific factors such as gender, severity of disease, specific drug use, and intervention cycle to effectively control heterogeneity factors, so that the clinical exercise intervention program for PD patients is objective, scientific, and effective.

Anxiety disorders are the most common mental illness in the United States, affecting 40 million adults, or 18% of the population. A characterizing feature of anxiety disorders is that the suffer overly identifies with and personalizes their thoughts. The sufferer has recurring thoughts, such as impending disaster, that they may realize are unreasonable, but are unable to shake. Indeed, Mindfulness practices have been shown to be quite effective in relieving anxiety. Clinically diagnosed depression affects over 6% of the population. Depression can be difficult to treat. Fortunately, Mindfulness training is also effective for treating depression.

 

Anxiety disorders and clinical depression have generally been treated with drugs. But there are considerable side effects and these drugs are often abused. There are a number of psychological therapies for anxiety. But, about 45% of the patients treated do not respond to the therapy. So, there is a need to develop alternative treatments. Recently, it has been found that mindfulness training can be effective for anxiety disorders and for depression either alone or in combination with other therapies. Mindful Movement practices such as Qigong and Tai Chi have been found to be effective for depression and anxiety. Research has been accumulating. So, it is important to step back and examine what has been learned regarding the application of Qigong and Tai Chi practices for mood.

 

In today’s Research News article “Qigong and Tai-Chi for Mood Regulation.” (See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6519567/), Yeung and colleagues review and summarize the published research studies of the effectiveness of Qigong and Tai Chi practices for improving mood.

 

They found that the published research reports that Qigong and Tai Chi practice produce significant decreases in anxiety and depression and increases in psychological well-being, self-esteem, and self-efficacy. In addition, Qigong and Tai Chi practice have been shown to be effective in reducing depression that accompanies diseases including fibromyalgia, arthritis, obesity, and cardiovascular disease.

 

The mechanisms by which Qigong and Tai Chi practice improves mood are unknown. But it has been speculated that it may work by increasing mindfulness, reduces perceived stress, improving interoception, producing neuroplastic changes in the brain, improving respiration control, and altering genes. It may be that these practices produce the benefits through a combination of mechanisms or that different mechanisms underlie different benefits. Regardless, the evidence is compelling that Qigong and Tai Chi practice have beneficial effects on the psychological well-being of healthy people and people with diseases.

 

Qigong and Tai Chi  practices are gentle and completely safe, can be used with the elderly and sickly, are inexpensive to administer, can be performed in groups or alone, at home or in a facility or even public park, and can be quickly learned. In addition, they can also be practiced in social groups without professional supervision. This can make it fun, improving the likelihood of long-term engagement in the practice. Abstract

Qigong and Tai-Chi are traditional self-healing, self-cultivation exercises originating in ancient China. These exercises are characterized by coordinated body posture and movements, deep rhythmic breathing, meditation, and mental focus based on traditional Chinese medicine theories. Although the exact mechanisms of Qigong’s and Tai-Chi’s effects on physical and mental well-being are unknown, these practices may be viewed as meditative movements and share many of the healing elements observed in mindfulness meditation. Clinical studies including randomized controlled trials and meta-analyses have shown that both Qigong and Tai-Chi have beneficial effects on psychological well-being and reduce symptoms of anxiety and depression. Qigong and Tai-Chi frequently involve anchoring attention to interoceptive sensations related to breath or other parts of the body, which has been shown to enhance nonreactivity to aversive thoughts and impulses. Preliminary studies suggest that the slow movements in Qigong and Tai-Chi with slowing of breath frequency could alter the autonomic system and restore homeostasis, attenuating stress related to hypothalamus-pituitary-adrenal axis reactivity and modulating the balance of the autonomic nervous system toward parasympathetic dominance. Qigong’s and Tai-Chi’s effects on emotion regulation could occur through changes in multiple prefrontal regions, the limbic system, and the striatum or in the expression of genes linked to inflammatory responses and stress-related pathways.

The process of aging affects every aspect of the physical and cognitive domains. Every system in the body deteriorates including motor function with a decline in strength, flexibility, and balance. Impaired balance is a particular problem as it can lead to falls. In the U.S. one third of people over 65 fall each year and 2.5 million are treated in emergency rooms for injuries produced by falls. About 1% of falls result in deaths making it the leading cause of death due to injury among the elderly. Falls, with or without injury, also carry a heavy quality of life impact.

 

Peripheral neuropathy can compound the effects of aging on motor ability. It involves damage to the peripheral nerves carrying information to the spinal cord and brain. This results in deficits in sensory information from the body and motor control. Peripheral neuropathy can impair muscle movement, prevent normal sensation in the arms and legs, and cause pain. In this way peripheral neuropathy may further impair older adults motor abilities and increase the likelihood of falls.

 

Tai Chi training is designed to enhance and regulate the functional activities of the body through posture, regulated breathing, mindful concentration, and gentle movements. It includes posture and balance training and has been shown to improve balance and coordination. Indeed, Tai Chi training has been shown to reduce the likelihood of falls in the elderly. One possible way that Tai Chi training may contribute to the decrease in falls is by improving postural control. This interesting speculation has not been previously investigated in older patients with peripheral neuropathy.

 

In today’s Research News article “Complexity-based measures inform Tai Chi’s impact on standing postural control in older adults with peripheral neuropathy.” (See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC3640896/), Manor and colleagues recruited older adults (average age 71 years) with peripheral neuropathy. They were provided 3 1-hour Tai Chi classes per week for 24 weeks. They were measured before and after training for foot sole sensation, leg strength, mobility, functional ability, and postural control, including measures of balance, speed, area under the curve, and complexity of postural control while standing.

 

They found that in comparison to baseline, after Tai Chi training there were significant improvements in foot sole sensation, leg strength, mobility, and physical function, including knee extension, walking, and speed. There was also a significant increase in postural control complexity. In addition, they found that the greater the increase in postural control complexity the greater the improvements in foot sole sensation, mobility, and functional ability.

 

This study did not have a control condition so conclusions must be tempered with the understanding that the results may have been due to time-based confounding variables. Nevertheless, the results suggest that Tai Chi practice improves the physical condition and abilities of older adults with peripheral neuropathy. Aging is associated with a loss of motoric complexity. The improvement in postural complexity observed in the present study may then represent the ability of Tai Chi practice to restore this complexity and thereby help the older adults’ physical abilities.

Tai Chi training enhances physical function and may reduce falls in older adults with and without balance disorders, yet its effect on postural control as quantified by the magnitude or speed of center-of-pressure (COP) excursions beneath the feet is less clear. We hypothesized that COP metrics derived from complex systems theory may better capture the multi-component stimulus that Tai Chi has on the postural control system, as compared with traditional COP measures.

Methods

We performed a secondary analysis of a pilot, non-controlled intervention study that examined the effects of Tai Chi on standing COP dynamics, plantar sensation, and physical function in 25 older adults with peripheral neuropathy. Tai Chi training was based on the Yang style and consisted of three, one-hour group sessions per week for 24 weeks. Standing postural control was assessed with a force platform at baseline, 6, 12, 18, and 24 weeks. The degree of COP complexity, as defined by the presence of fluctuations existing over multiple timescales, was calculated using multiscale entropy analysis. Traditional measures of COP speed and area were also calculated. Foot sole sensation, six-minute walk (6MW) and timed up-and-go (TUG) were also measured at each assessment.

Results

Traditional measures of postural control did not change from baseline. The COP complexity index (mean±SD) increased from baseline (4.1±0.5) to week 6 (4.5±0.4), and from week 6 to week 24 (4.7±0.4) (p=0.02). Increases in COP complexity—from baseline to week 24—correlated with improvements in foot sole sensation (p=0.01), the 6MW (p=0.001) and TUG (p=0.01).

Conclusions

Subjects of the Tai Chi program exhibited increased complexity of standing COP dynamics. These increases were associated with improved plantar sensation and physical function. Although more research is needed, results of this non-controlled pilot study suggest that complexity-based COP measures may inform the study of complex mind-body interventions, like Tai Chi, on postural control in those with peripheral neuropathy or other age-related balance disorders.

Diabetes is a major health issue. It is estimated that 30 million people in the United States have diabetes and the numbers are growing. Type 2 Diabetes results from a resistance of tissues, especially fat tissues, to the ability of insulin to promote the uptake of glucose from the blood. As a result, blood sugar levels rise producing hyperglycemia. Diabetes is the 7th leading cause of death in the United States. In addition, diabetes is heavily associated with other diseases such as cardiovascular disease, heart attacks, stroke, blindness, kidney disease, and circulatory problems leading to amputations. As a result, diabetes doubles the risk of death of any cause compared to individuals of the same age without diabetes.

 

Type 2 diabetes is largely preventable. One of the reasons for the increasing incidence of Type 2 Diabetes is its association with overweight and obesity which is becoming epidemic in the industrialized world. A leading cause of this is a sedentary life style. Current treatments for Type 2 Diabetes focus on diet, exercise, and weight control. Recently, mindfulness practices have been shown to be helpful in managing diabetes. Mindful movement practices such as Tai Chi and Qigong  and yoga are mindfulness practices that are also gentle exercises. There is accumulating research on the effectiveness of these mindful movement practices for the treatment of Type 2 Diabetes. So, it makes sense to examine what has been learned.

 

In today’s Research News article “Meditative Movements for Patients with Type 2 Diabetes: A Systematic Review and Meta-Analysis.” (See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC7016481/), Xia and colleagues review, summarize, and perform a meta-analysis of the published research literature on the effectiveness of mindful movement practices for the treatment of Type 2 Diabetes. They found 21 controlled studies; 6 employing Tai Chi practice, 3 Qigong practice, and 12 yoga.

 

They report that the published research found that mindful movement practices produced significant reductions in fasting blood glucose, glycated hemoglobin (HbA1c), and postprandial blood glucose levels. These results suggest that mindful movement practices improve glycemic control in type 2 diabetes. There were no significant differences found between different mindful movement practices.

 

They also report that the published research found that mindful movement practices produced significant reductions in total cholesterol and low-density lipoprotein cholesterol and significant increases in high-density lipoprotein cholesterol. These results suggest that mindful movement practices improve blood lipid levels in type 2 diabetes. There were no significant differences found between different mindful movement practices.

 

The improvements observed produced by Tai Chi, Qigong  and yoga practices are very important for the treatment and control of type 2 diabetes. Glycemic control is a key to successful treatment and lipid control is important for reducing cardiovascular problems that can occur. So, these exercises significantly improve the metabolic state of patients with type 2 diabetes. The fact that the different practices were equivalent in effectiveness suggests that the patient can select the practice type that they enjoy most and best suits their lifestyle.

 

So, improve Type 2 Diabetes with mindful movement.

Abstract

Objective

Physical activity plays a specific role in the fundamental aspect of diabetes care. It is necessary to develop exercise programs for these patients. The aim of this systematic review is to summarize current evidence regarding the effectiveness of meditative movement in patients with type 2 diabetes.

Methods

The following databases were searched: PubMed, CENTRAL, Web of Science, Ovid LWW, and EMBASE. Two independent investigators searched and screened the studies by finding duplications, excluding irrelevant titles and abstracts, and then selecting eligible studies by reviewing full texts. 21 studies fulfilled the inclusion criteria. Meta-analyses were performed on glycated hemoglobin (HbA1c), fasting blood glucose (FBG) and postprandial blood glucose (PPBG), total cholesterol (TC), triglycerides (TG), high-density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol (LDL-C), and body mass index (BMI).

Results

Meta-analyses showed that meditative movements significantly improved FBG, HbA1c, PPBG, TC, LDL-C, and HDL-C. No improvement was found in BMI.

Conclusions

The results demonstrated a favorable effect or tendency of meditative movements to improve blood glucose and blood lipid levels in patients with type 2 diabetes mellitus. The special effects of meditative movements in type 2 diabetes mellitus patients need further research.
