Reduce Injurious Falls in the Elderly with Tai Chi

With regular practice, tai chi improves balance by strengthening muscles and co-ordination; at the same time, it strengthens the mind, thereby improving calmness and confidence in not falling. Thus, both physically and mentally, tai chi is an extremely effective exercise for fall prevention.” – Paul Lam

 

The process of aging affects every aspect of the physical and cognitive domains. Every system in the body deteriorates including motor function with a decline in strength, flexibility, and balance. Impaired balance is a particular problem as it can lead to falls. In the U.S. one third of people over 65 fall each year and 2.5 million are treated in emergency rooms for injuries produced by falls. About 1% of falls result in deaths making it the leading cause of death due to injury among the elderly.

 

Falls, with or without injury, also carry a heavy quality of life impact. A growing number of older adults, fear falling and, as a result, limit their activities and social engagements. This can result in further physical decline, depression, social isolation, and feelings of helplessness. It is obviously important to discover methods to improve balance and decrease the number of falls in the elderly.

 

Tai Chi training is designed to enhance and regulate the functional activities of the body through regulated breathing, mindful concentration, and gentle movements. It includes balance training and has been shown to improve balance and coordination. Indeed, Tai Chi training has been shown to reduce the frequency of falls in the elderly. It is not known, however, if Tai Chi training is better or worse than other exercises for reducing falls in the elderly.

 

In today’s Research News article “Effectiveness of Tai Ji Quan vs Multimodal and Stretching Exercise Interventions for Reducing Injurious Falls in Older Adults at High Risk of Falling: Follow-up Analysis of a Randomized Clinical Trial.” (See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6484587/), Harmer and colleagues recruited elderly individuals (70 years of age and older) who had impaired mobility and who had experienced falls in the last year. They were randomly assigned to either to twice weekly, 1-hour, sessions, for 24 weeks of Tai Chi practice, multimodal exercise, or stretching exercises. Multimodal exercises consisted of “a mix of aerobic conditioning and strength, balance, and flexibility activities.” They were measured before, after, and 3 and 6 months after the interventions for falls, and the “number of fall-related emergency department visits and hospitalizations.”

 

They found that at 6 and 12 months, the Tai Chi  groups had significantly fewer injurious falls both moderately serious and serious than the stretching group and significantly fewer than the multimodal exercise group at 6 months. The multimodal exercise group also had significantly fewer serious falls than the stretching group at 12 months. At the 12 month point the Tai Chi group had significantly fewer emergency department visits and hospitalizations than the multimodal exercise group and the stretching group. The multimodal exercise group had significantly fewer emergency department visits than the stretching group

 

The results demonstrate that although multimodal exercise is beneficial for the elderly in reducing injurious falls, practicing Tai Chi  was significantly more beneficial; reducing injurious falls, emergency department visits and hospitalizations. Tai Chi  is gentle and safe, is appropriate for all ages including the elderly and for individuals with illnesses that limit their activities or range of motion, is inexpensive to administer, can be performed in groups or alone, at home or in a facility, and can be quickly learned. In addition, it can be practiced in social groups. This can make it fun, improving the likelihood of long-term engagement in the practice. So, Tai Chi  practices would appear to be an excellent gentle practice to reduce injurious falls in aging individuals.

Abstract

Importance

Exercise has been shown to reduce injurious falls in older adults. Evidence, however, is lacking regarding the types of intervention that are most effective in preventing injurious falls among older adults at high risk of falling.

Objective

To determine the longer-term effectiveness of therapeutic tai ji quan intervention vs multimodal exercise and stretching exercise in decreasing injurious falls among older adults at high risk of falling.

Design, Setting, and Participants

This trial involves a prespecified analysis with the data analyzed by intent-to-treat. Follow-up analysis of a single-blind randomized trial conducted in community settings of 7 urban and suburban cities in Oregon from February 20, 2015, to September 15, 2018, compared a therapeutic tai ji quan intervention with multimodal exercise and stretching exercise. Eligible participants were community-dwelling adults aged at least 70 years who were considered by a clinician to be at high risk of falling because they had fallen during the preceding year or who had impaired mobility with scores higher than 13.5 seconds on the Timed Up & Go test. Participants were randomized to 1 of the 3 interventions and were assessed monthly after randomization for 12 months, encompassing a 6-month active intervention phase and a 6-month after intervention follow-up phase.

Interventions

The 3 group-based interventions were therapeutic tai ji quan (Tai Ji Quan: Moving for Better Balance [TJQMBB]), multimodal exercise, and stretching exercise, each implemented twice weekly in 60-minute sessions for 24 weeks.

Main Outcomes and Measures

Primary outcomes were the incidence of moderate and serious injurious falls at 12 months, measured as incidence rate ratios (IRRs).

Results

Of the 1147 persons screened, 670 (mean [SD] age, 77.7 [5.6] years; 436 women [65.1%]) were randomly assigned to 1 of 3 intervention groups: 224 persons in TJQMBB, 223 in multimodal exercise, and 223 in stretching exercise. At 12 months, the unadjusted IRR for moderate injurious falls was lower in the TJQMBB (IRR, 0.51; 95% CI, 0.35-0.74; P < .001) and multimodal exercise (IRR, 0.62; 95% CI, 0.42-0.89; P = .01) groups compared with the stretching exercise group. There was no difference between TJQMBB and multimodal exercise groups (IRR, 0.85; 95% CI, 0.58-1.25; P = .42). Both TJQMBB and multimodal exercise significantly reduced serious injurious falls (TJQMBB: IRR, 0.25 [95% CI, 0.13-0.48; P < .001]; multimodal: IRR, 0.56 [95% CI, 0.33-0.94; P = .03]) compared with stretching exercise. Use of TJQMBB was more effective than multimodal exercise (IRR, 0.47; 95% CI, 0.24-0.92; P = .03) in reducing serious injurious falls.

Conclusions and Relevance

For preventing injurious falls, including those that resulted in medical treatment, TJQMBB intervention was found to be superior to multimodal and stretching exercises for older adults at high risk of falling. The findings appear to strengthen the clinical use of TJQMBB as a single exercise intervention to prevent injurious falls in this population.

It is a common human phenomenon that being in a social situation can be stressful and anxiety producing. Most people can deal with the anxiety and can become quite comfortable. But many do not cope well and the anxiety is overwhelming, causing the individual to withdraw. Social Anxiety Disorder (SAD) is characterized by a persistent, intense, and chronic fear of being watched and judged by others and feeling embarrassed or humiliated by their actions. This fear may be so severe that it interferes with work, school, and other activities and may negatively affect the person’s ability to form relationships.

 

Anxiety disorders have generally been treated with drugs. But there are considerable side effects and these drugs are often abused. There are a number of psychological therapies for anxiety. But, about 45% of the patients treated do not respond to the therapy. So, there is a need to develop alternative treatments. Recently, it has been found that mindfulness training can be effective for anxiety disorders including Social Anxiety Disorder (SAD)Mindfulness-Based Stress Reduction (MBSR) and also Cognitive Behavioral Therapy (CBT) have been shown to be effective in treating Social Anxiety Disorder (SAD).

 

A therapeutic technique that contains mindfulness training and Cognitive Behavioral Therapy (CBT) is Acceptance and Commitment Therapy (ACT). It is a mindfulness-based psychotherapy technique that is employs many of the techniques of Cognitive Behavioral Therapy (CBT). ACT focuses on the individual’s thoughts, feelings, and behavior and how they interact to impact their psychological and physical well-being. It then works to change thinking to alter the interaction and produce greater life satisfaction. ACT employs mindfulness practices to increase awareness and develop an attitude of acceptance and compassion in the presence of painful thoughts and feelings. ACT teaches individuals to “just notice”, accept and embrace private experiences and focus on behavioral responses that produce more desirable outcomes.

 

In today’s Research News article “The effectiveness of acceptance and commitment group therapy on social anxiety in female dormitory residents in Isfahan university of medical sciences.” (See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6432841/), Toghiani and colleagues recruited female college students and randomly assigned them to receive 5-week, one 2-hour session per week, group Acceptance and Commitment Therapy (ACT) or to a no-treatment control group. Before the intervention and 2 months later the students were measured for social anxiety, performance anxiety and psychological flexibility.

 

They found that in comparison to baseline and the no-treatment control group, the students who received ACT had significantly reduced social anxiety, social avoidance, performance anxiety, and performance avoidance. These results must be interpreted cautiously as there was not an active control condition leaving open the possibility of placebo effects and experimenter bias effects. Nevertheless, the study suggests that ACT is an effective therapy to reduce anxiety in female college students with lasting effects.

Abstract

AIM AND BACKGROUND:

Social anxiety can interfere with performance and academic success in students. One of the third-generation treatments for social anxiety is acceptance and commitment therapy. Therefore, the current study aims to determine the effectiveness of acceptance and commitment group therapy on social anxiety of female dormitory residents of Isfahan University of Medical Sciences.

METHODS:

This was a semiempirical study with pre- and posttest conducted on 71 female students living in the dormitory of Isfahan University of Medical Sciences. The study was carried out in five training sessions using the Liebowitz Social Anxiety Scale and second version of acceptance and commitment scale whose validity and reliability were confirmed. Data were analyzed using Student’s t-test.

RESULTS:

The findings showed that acceptance and commitment group therapy has affected the social anxiety in female dormitory residents of Isfahan University of Medical Sciences (P < 0.0001).

CONCLUTIONS:

The findings of this study can be used by student deputies of universities, consultation centers, as well as counselors and psychologists to improve the conditions for students.

Yoga practice is becoming increasingly popular in the west, for good reason. It has documented benefits for the individual’s psychological and physical health and well-being. It has also been shown to have cognitive benefits, improving memory. Yoga, however, consists of a number of components including, poses, breathing exercises, meditation, concentration, and philosophy/ethics.  So, it is difficult to determine which facet or combination of facets of yoga are responsible for which benefit. Hence, it is important to begin to test each component in isolation to determine its effects.

 

Alternate nostril yoga breathing is a regulated breathing alternating between the left and right nostril that is commonly practiced in yoga. Breathing through each nostril is thought to affect its respective hemisphere in the brain producing differential effects. Recently, it has been shown to reduce blood pressure and increase vigilance and reduce perceived stress.

 

In today’s Research News article “Changes in Shape and Size Discrimination and State Anxiety After Alternate-Nostril Yoga Breathing and Breath Awareness in One Session Each.” (See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6496972/), Telles and colleagues recruited healthy adult males with at least 3 months of yogic breathing practice. They each completed 3 sessions, alternate nostril breathing, breath awareness, and quiet sitting in counterbalanced orders. Each session consisted of three 5-minute practice periods separated by one minute. Before and after each session the participants competed measures of state and trait anxiety and completed a shape and size discrimination task involving inserting “50 coins and squares of different sizes and thickness into 5 slits of a wooden box, where each slit has been especially designed to allow a square or coin of a specific size and thickness to pass through it.”

 

They found that after the alternate nostril breathing session but not breath awareness or quiet sitting sessions there were significantly fewer error on the shape and size discrimination task. On the other hand, after the breath awareness or quiet sitting sessions but not the alternate nostril breathing session there were significantly lower levels of anxiety.

 

It would appear that alternate nostril breathing heightens awareness, increasing vigilance allowing for enhanced sensory discrimination ability while breath awareness and quiet sitting are calming reducing negative emotions. It’s interesting that different approaches to the breath have such large differences in their effects. This suggests that further research into the effects of the individual components of yoga practice may be a fruitful approach to understanding and potentially enhancing the benefits of yoga practice.

Abstract

Background

Yoga breathing techniques like high-frequency yoga breathing (HFYB) and breath awareness (BAW) have been associated with improved performance in the shape and size discrimination task. A PubMed search of the literature revealed that alternate-nostril breathing has been shown to improve performance in attention tasks, but the effect on tactile perception has not been studied. Hence, the present study was designed to assess the immediate effects of alternate-nostril yoga breathing (ANYB) compared to breath awareness on shape and size discrimination and state anxiety.

Material/Methods

Fifty healthy male volunteers ages 20–50 years (group mean ±S.D., 28.4±8.2 years) were recruited. Each participant was assessed in 3 sessions conducted on 3 separate days at the same time of day. The 3 sessions were (i) alternate-nostril yoga breathing (ANYB), (ii) breath awareness (BAW), and (iii) quiet sitting (QS), and the sequence of the sessions was randomly allocated. The shape and size discrimination task and state anxiety were assessed before and after all 3 sessions. Repeated measures analysis of variance (RM-ANOVA) followed by post hoc tests for multiple comparisons, which were Bonferroni-adjusted, were performed to compare data before and after all 3 sessions using SPSS version 18.0.

Results

The errors scores in the shape and size discrimination task showed a significant reduction after the ANYB session (p<0.001). A significant reduction was found in the level of state anxiety after breath awareness (p<0.05) and quiet sitting sessions (p<0.001).

Conclusions

The present results suggest that ANYB: (i) improves performance in a task which requires perceptual sensitivity and focused attention, but (ii) does not reduce state anxiety following this task.

There has accumulated a large amount of research demonstrating that mindfulness has significant benefits for psychological, physical, and spiritual wellbeing. One way that mindfulness practices may produce these benefits is by altering the brain. The nervous system is a dynamic entity, constantly changing and adapting to the environment. It will change size, activity, and connectivity in response to experience. These changes in the brain are called neuroplasticity. Over the last decade neuroscience has been studying the effects of contemplative practices on the brain and has identified neuroplastic changes in widespread areas. In other words, mindfulness practice appears to mold and change the brain, producing psychological, physical, and spiritual benefits.

 

In today’s Research News article “Meditation and yoga practice are associated with smaller right amygdala volume: the Rotterdam study.” (See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6302143/), Gotink and colleagues utilized the data on participants in the longitudinal Rotterdam Study who were 45 years of age and older  at the time of recruitment and at the time of measurement had a mean age of 64 years. They were interviewed to determine if the practiced meditation and yoga and whether these practices improved their coping with stress. They were also measured for body size, blood pressure, blood fat, diabetes, smoking, alcohol use, stress, anxiety, and depression. In addition, their brains were scanned with Magnetic Resonance Imaging (MRI).

 

They found that practitioners of meditation and yoga reported higher stress levels than non-practitioners, but reported that the practice helped them cope with the stress. In addition, meditation practitioners had higher depression levels than non-practitioners. It is possible that people who are under high levels of stress or are depressed tend to engage in meditation and yoga practices to help cope with it.

 

They also report that the practitioners had smaller volumes of the brain structures right side amygdala and left hippocampus. In addition, over a five-year period the practitioners had a significant decrease in amygdala volume. The amygdala is associated with negative emotions and its smaller volume may suggest fewer or weaker negative emotions in practitioners.

 

This was a cross-sectional study and causation cannot be determined. It is possible that people with certain types of brains are more likely to practice. It will require a randomized controlled trial to determine what effects yoga and meditation practice may have on the psychological state and nervous system volumes.

Abstract

To determine the association between meditation and yoga practice, experienced stress, and amygdala and hippocampal volume in a large population-based study. This study was embedded within the population-based Rotterdam Study and included 3742 participants for cross-sectional association. Participants filled out a questionnaire assessing meditation practice, yoga practice, and experienced stress, and underwent a magnetic resonance scan of the brain. 2397 participants underwent multiple brain scans, and were assessed for structural change over time. Amygdala and hippocampal volumes were regions of interest, as these are structures that may be affected by meditation. Multivariable linear regression analysis and mixed linear models were performed adjusted for age, sex, educational level, intracranial volume, cardiovascular risk, anxiety, depression and stress. 15.7% of individuals participated in at least one form of practice. Those who performed meditation and yoga practices reported significantly more stress (mean difference 0.2 on a 1–5 scale, p < .001) and more depressive symptoms (mean difference 1.03 on CESD, p = .015). Partaking in meditation and yoga practices was associated with a significantly lower right amygdala volume (β = − 31.8 mm3, p = .005), and lower left hippocampus volume (β = − 75.3 mm3, p = .025). Repeated measurements using linear mixed models showed a significant effect over time on the right amygdala of practicing meditation and yoga (β = − 24.4 mm3, SE 11.3, p = .031). Partaking in meditation and yoga practice is associated with more experienced stress while it also helps cope with stress, and is associated with smaller right amygdala volume.

We have pursued these freedoms with scandal, exploitation, and domination. Today, instead, I urge you to consider inner freedom.” – Arnie Kozak

 

Virtually every country in the world sets aside one day each year to celebrate its independence. In the U.S. that day is July 4th. On this day the country’s citizens celebrate their freedom and independence and the fight that achieved it. It’s a pleasant holiday filled with patriotism, flags, parades, picnics, and fireworks displays. Although the founding of the great American democracy is something to celebrate, a mindful look at it produces a recognition that there are significant limitations on independence and freedom. We are nowhere near as free and independent as we think we are.

 

Independence from what? It’s certainly not from the imposition of government on the individual. July 4th only celebrates the changeover from government by the British monarchy to government by a more local political system. It’s certainly not independence from the imposition of laws and restrictions on the individual’s freedom. Perhaps there was a change of a few laws and regulations, but actually only a small number. It’s certainly not even the production of self-determination. In fact, the U.S. democracy was crafted and established by a few elite individuals and not by each individual in the country. In addition, democracy is rule by the majority, with the will of a significant number of people ignored. What we appear to be celebrating is the replacement of one system of control with another, perhaps better, system of control, but nevertheless a system of control; hardly independence.

 

Mindful reflection quickly produces an understanding that we’re never really independent. It’s certainly not even complete independence from another country. To this day the U.S. and the U.K. are very much dependent upon one another for trade of goods, ideas, culture, and mutual security. They’re locked together by treaties, cultural similarities, and close economic ties. The current political system that we’re celebrating is itself a recognition of how dependent upon one another we are. The system functions to set down the rules by which our relationships with one another are conducted. It’s there to insure orderly cooperation supposedly for the benefit of all participants.

 

Mindful reflection reveals that we’re not only dependent upon each other but we’re also dependent upon our environment, animate and inanimate. We’re dependent upon the air we breathe that is in turn dependent upon all other living organisms. We’re dependent upon the water we drink that is in turn dependent on global weather systems and solar evaporative power. We’re dependent upon the food we drink that is in turn dependent upon air, water, soil, and sun, and the farmers who grow it. In fact, we are so dependent upon everything and everybody that it may be more appropriate to be celebrating Dependence Day.

 

Well maybe then on July 4th we’re celebrating freedom and liberty. But, is any individual truly free. As the French philosopher Jean-Jacques Rousseau said “Man is born free: and everywhere else he is in chains. One thinks himself the master of others, and still remains a greater slave than they.” Regardless of the political independence each individual’s behavior is highly regulated by law and regulation. Our freedoms are actually very limited. They are bounded not only by law but also the practicalities of earning a living, maintaining a residence, having a family, and limitations on resources. Our freedom is also highly constrained by the laws of physics, chemistry, and biology. After all, we can’t fly, become taller, change our eye color, stay underwater for protracted periods, stay awake continuously, or withstand cold or heat outside of a fairly small range, and we’re not faster than a speeding bullet. How much freedom do we actually have in any particular day?

 

Independence Day, though, does celebrate acquiring many soft freedoms. The freedoms to think and express our opinions and ideas, to worship as we please, to vote for whoever we like, to associate with whomever we choose, to live wherever we like, etc. Although there are bounds to many of these freedoms by the requirements of public safety, economics, cultural norms, and the practicalities of existence, these are very important and significant freedoms. Perhaps that is what we’re really celebrating, these soft freedoms that were provided by our Constitution as a result of the War for Independence.

 

Regardless, Independence Day should be celebrated mindfully. It is often spent with family and friends and the pleasure of these interactions can be amplified by doing it mindfully; by being truly present for them and deeply listening to them rather than thinking about our next response. By being mindful we can see them with compassion and understanding. Being in, and focusing on, the present moment we can enjoy these interactions, we can enjoy the picnics and parades, we can enjoy the fireworks, rather than thinking about where we would rather be or where we’re going next. We can find happiness precisely where we are.

 

But are we truly free. A bit of mindful reflection reveals that we find existence very unsatisfactory. In fact, unsatisfactoriness is everywhere. We’re not satisfied with things as they are and want them to be different. We’re not satisfied with where we live and want to have a nicer home.  We’re not satisfied with our appearance and want to lose weight. We’re not satisfied with what people think of us and want to be universally liked. We’re not satisfied with how we’re treated by our spouses and want them to be more understanding. We’re not satisfied with our children and want them to be obedient, respectful, straight “A” students and star athletes. We’re not satisfied with our health and want to have fewer aches and pains. We’re not satisfied with our jobs and want to make more money, have more time off and be appreciated by our bosses and coworkers. Even on the very short-term, things are not satisfactory. We want the car ahead of us to be moving faster, we want time to pass quickly so that we can be done with work for the day, we want to stop ruminating about past indiscretions, we want to finish a meal quickly so we can get back to the TV, etc. In other words, we’re not free from our desires. In fact, we’re slaves to them. We’re not happy with the way things are. In fact, we seem to want everything to be different. So, we can’t be truly free as long as we’re slaves to our desires.

 

True freedom can only be produced when we are liberated from our incessant needs and wants. That is not to say that we shouldn’t have desires, but rather that we will not be controlled by them. True freedom comes from equanimity. It comes when we’re able to desire something, seek it out, but be OK whether we get it or not. It comes when we not only accept the way things are but enjoy each second for what it is, a precious moment in a limited lifetime. It comes when what other people do and say is seen as a reflection of them and not of us and comes when we look at them with compassion and understanding. In other words, we can want ourselves, things, people, and circumstances to be different but we accept them as they are and appreciate and enjoy life and each experience as a gift.

 

This sounds wonderful, but is it achievable? It sure doesn’t seem so as ourselves and the people we know haven’t achieved it. Is it possible to actually get to this state of complete freedom? It is, but it takes effort and discipline. There have been many instances throughout history and there are many exemplars present right now of people who have achieved complete equanimity. Jesus is a wonderful example. He worked hard and suffered to make his world a better place but in the end accepted what was. The Buddha, Christian mystics, Sufi masters, Zen masters, Gandhi, and a host of everyday people have all achieved true liberation. So, it is possible.

 

We do not, however, have to be aiming only at complete liberation. It is far better to work to simply improve on our current state and thereby become more and more liberated. We can do this by engaging in mindfulness practices such as meditation, yoga, tai chi, contemplative prayer, etc. we can learn to focus more and more on the present moment. We can learn to appreciate what is. We can learn to enjoy every moment. Just by improving a little we can become happier and happier, more accepting, and more liberated from our desires. We can achieve greater equanimity and with it greater freedom. But, we get there slowly and incrementally, building toward our complete liberation. Now wouldn’t that be a reason to celebrate Independence Day.

In our lives we are confronted with a variety of situations and environments. In order to successfully navigate these differing situations, we must be able to adapt and self-regulate. The Autonomic Nervous System (ANS) is designed to adapt physiologically to the varying demands on us. It is composed of 2 divisions; the sympathetic division underlies activation, including increases in respiration, heart rate, and blood pressure, while the parasympathetic division underlies relaxation, including decreases in respiration, heart rate, and blood pressure.

 

Mindfulness training has been shown through extensive research to be effective in improving physical and psychological health and particularly with the physical and psychological reactions to stress. These include alterations of the Autonomic Nervous System (ANS) producing physiological relaxation including reductions in breathing rates. But the effects of meditation on breathing has not been well studied.

 

In today’s Research News article “Breath Rate Variability: A Novel Measure to Study the Meditation Effects.” (See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6329220/), Soni and colleagues recruited experienced meditators and nonmeditators who expressed interest in meditation. They were asked to close their eyes and sit quietly for 15 minutes. During which time they measured for heart rate and respiration.

 

They found that the meditators had significantly greater mean and median breathing time, standard deviation of the breath to breathe interval, standard deviation of the average breath to breathe interval, root mean square standard deviation of the average breath to breathe interval and significantly lower breath rate. These measures suggest that meditation practice produces short-term changes in breathing when at rest. In addition, heart rate and breath rate variability measures suggested that there was a significant increase in long-term parasympathetic input in the meditators.

 

These are complex but interesting results that suggest that meditation practice alters respiration over both the short and long term. On the short term, while at rest with eyes closed meditators have better, more relaxed, control of respiration. On the long term, meditator appear to have increased parasympathetic control of respiration. This suggest an overall relaxation of respiration. Meditation would appear to alter the overall balance in the autonomic nervous system toward increased parasympathetic (relaxation) control and decreases sympathetic (activation) control. This may explain some of the benefits of meditation for stress reduction.

 

So, relax short-term and long-term breathing with meditation

Abstract

Context:

Reliable quantitative measure of meditation is still elusive. Although electroencephalogram (EEG) and heart rate variability (HRV) are known as quantitative measures of meditation, effects of meditation on EEG and HRV may well take long time as these measures are involuntarily controlled. Effect of mediation on respiration is well known; however, quantitative measures of respiration during meditation have not been studied.

Aims:

Breath rate variability (BRV) as an alternate measure of meditation even over a short duration is proposed. The main objective of this study is to test the hypothesis that BRV is a simple measure that differentiates between meditators and nonmeditators.

Settings and Design:

This was a nonrandomized, controlled trial. Volunteers meditate in their natural habitat during signal acquisition.

Subjects and Methods:

We used Photo-Plythysmo-Gram (PPG) signal acquisition system from BIO-PAC and recorded video of chest and abdomen movement due to respiration during a short meditation (15 min) session for 12 individuals (all males) meditating in a relaxed sitting posture. Seven of the 12 individuals had substantial experience in meditation, while others are controls without any experience in meditation. Respiratory signal from PPG signal was derived and matched with that of the video respiratory signal. This derived respiratory signal is used for calculating BRV parameters in time, frequency, nonlinear, and time-frequency domain.

Statistical Analysis Used:

First, breath-to-breath interval (BBI) was calculated from the respiration signal, then time domain parameters such as standard deviation of BBI (SDBB), root mean square value of SDBB (RMSSD), and standard deviation of SDBB (SDSD) were calculated. We performed spectral analysis to calculate frequency domain parameters (power spectral density [PSD], power of each band, peak frequency of each band, and normalized frequency) using Burg, Welch, and Lomb–Scargle (LS) method. We calculated nonlinear parameters (sample entropy, approximate entropy, Poincare plot, and Renyi entropy). We calculated time frequency parameters (global PSD, low frequency-high frequency [LF-HF] ratio, and LF-HF power) by Burg LS and wavelet method.

Results:

The results show that the mediated individuals have high value of SDSD (+24%), SDBB (+29%), and RMSSD (+26%). Frequency domain analysis shows substantial increment in LFHF power (+73%) and LFHF ratio (+33%). Nonlinear parameters such as SD1 and SD2 were also more (>20%) for meditated persons.

Conclusions:

As compared to HRV, BRV can provide short-term effect on anatomic nervous system meditation, while HRV shows long-term effects. Improved autonomic function is one of the long-term effects of meditation in which an increase in parasympathetic activity and decrease in sympathetic dominance are observed. In future works, BRV could also be used for measuring stress.

“Distractions in the mind translate to micro movements in the eyes or eyelids, and vice-versa. Stillness of eyes brings stillness of mind, and vice-versa.” – Giovanni

 

We spend a tremendous amount of waking time with our minds wandering and not on the present environment or the task at hand. We daydream, plan for the future, review the past, ruminate on our failures, exalt in our successes. In fact, we spend almost half of our waking hours off task with our mind wandering. Mind wandering is also present even during meditation. Mind wandering interferes with our concentration on the present moment. Focused meditation, on the other hand, is the antithesis of mind wandering. Indeed, the more mindful we are the less the mind wanders.

 

A system of the brain known as the Default Mode Network (DMN) becomes active during wind wandering and relatively quiet during focused on task behavior. Meditation is known to reduce the size, connectivity, and activity of the Default Mode Network (DMN).  Hence, brain activity may help identify mind wandering when it occurs. Eye movements occur even when the eyes are closed and during meditation. They may also be indicators of mind wander in during meditation.

 

In today’s Research News article “Spontaneous eye movements during focused-attention mindfulness meditation.” (See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6345481/), Matiz and colleagues recruited adult experienced meditators. They engaged in a 7-minute focused breath meditation or a 7-minute mind wandering where they were asked to “remember or imagine one or more events of their past or future in which they, or another person, were the protagonist.” During the session brain activity, the electroencephalogram (EEG), was recorded. They derived a measure from the EEG that indicated vertical and horizontal eye movements. They also measured the total amount of meditation experience for each participant.

 

They found that during the 7-minutes of mind wandering there were significantly more eye movements, including both vertical and horizontal movements, than during the7-minutes of  focused meditation. In addition, they found that the more meditation experience that the meditator had, the fewer the eye movements that were recorded under both conditions. Hence, experienced meditators not only move their eyes less during meditation and but also during mind wandering.

 

These are interesting findings that suggest that analysis of the brain’s electrical activity, electroencephalogram (EEG), may be able to detect when mind wandering is occurring during meditation. This could lead to the possibility of providing biofeedback to the meditator when the mind is wandering, lessening the amount of mind wandering and thereby deepening the meditative experience. This is an intriguing possibility for future research.

Abstract

Oculometric measures have been proven to be useful markers of mind-wandering during visual tasks such as reading. However, little is known about ocular activity during mindfulness meditation, a mental practice naturally involving mind-wandering episodes. In order to explore this issue, we extracted closed-eyes ocular movement measurements via a covert technique (EEG recordings) from expert meditators during two repetitions of a 7-minute mindfulness meditation session, focusing on the breath, and two repetitions of a 7-minute instructed mind-wandering task. Power spectral density was estimated on both the vertical and horizontal components of eye movements. The results show a significantly smaller average amplitude of eye movements in the delta band (1–4 Hz) during mindfulness meditation than instructed mind-wandering. Moreover, participants’ meditation expertise correlated significantly with this average amplitude during both tasks, with more experienced meditators generally moving their eyes less than less experienced meditators. These findings suggest the potential use of this measure to detect mind-wandering episodes during mindfulness meditation and to assess meditation performance.

The aging process involves a systematic progressive decline in every system in the body, the brain included. This includes our mental abilities which decline with age including impairments in memory, attention, and problem-solving ability. It is inevitable and cannot be avoided.

 

Using modern neuroimaging techniques, scientists have been able to view the changes that occur in the nervous system with aging. In addition, they have been able to investigate various techniques that might slow the process of neurodegeneration that accompanies normal aging. They’ve found that mindfulness practices reduce the deterioration of the brain that occurs with aging restraining the loss of neural tissue. Indeed, the brains of practitioners of meditation and yoga have been found to degenerate less with aging than non-practitioners. Tai Chi and Qigong have also been shown to be beneficial in slowing or delaying physical and mental decline with aging and to increase brain matter in the elderly.

 

An interconnected system of the brain known as the Default Mode Network (DMN) is involved in internally focused tasks such as recalling memories, daydreaming, sleeping, imagining the future and trying to take the perspective of others. The DMN involves neural structures including the medial prefrontal cortex, anterior and posterior cingulate cortices, precuneus, inferior parietal cortex, and lateral temporal cortex. These areas of the DMN are functionally connected, such that they are simultaneously active during memory retrieval. It is possible that Tai Chi practice improves memory in aging adults by altering the functional connectivity of the DMN.

 

In today’s Research News article “Different Modulation Effects of Tai Chi Chuan and Baduanjin on Resting State Functional Connectivity of the Default Mode Baduanjin is a mind-body training that is very similar to Tai Chi and consists of 8 movements for limbs, body-trunk, and eye movements. Network in Older Adults.” (See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6374601/), Liu and colleagues recruited healthy older participants, aged 50 to 70 years, who had not been regularly exercising and randomly assigned them to a health education program or to engage in a Tai Chi or Baduanjin practice for 60 minutes per session, five days per week, for 12 weeks. Baduanjin is a mind-body training that is very similar to Tai Chi and consists of 8 movements for limbs, body-trunk, and eye movements. Before and after the 12-week intervention period the participants were measured for memory function and underwent Magnetic Resonance Imaging (MRI) scans.

 

They found that in comparison to baseline and the health education control group the older adults who practiced Tai Chi or Baduanjin had significant improvements in memory function. The MRI scans demonstrated increased Tai Chi Practice produced functional connectivity within the Default Mode Network (DMN) between the medial prefrontal cortex and right putamen/caudate and the cingulate cortex and right putamen/caudate. On the other hand, Baduanjin practice produced decreased functional connectivity between the medial prefrontal cortex and orbital prefrontal gyrus/putamen.

 

These results are interesting and suggest that both Tai Chi and Baduanjin practice improve memory in older adults and also alter the connectivity between structures in the Default Mode Network (DMN). Interestingly, the two practices appear to produce different changes in functional connectivity within the DMN. This suggests that the two practice my improve memory in different ways, both altering the DMN, but differently.

 

The results demonstrate as has previous research that ancient Chinese mindful movement practices help to restrain age related deterioration in the memory processes. These mindful movement practices are gentle and safe, are appropriate for all ages including the elderly and for individuals with illnesses that limit their activities or range of motion, are inexpensive to administer, can be performed in groups or alone, at home or in a facility, and can be quickly learned. In addition, they can be practiced in social groups. This can make it fun, improving the likelihood of long-term engagement in the practice. So, Tai Chi and Baduanjin practices would appear to be an excellent gentle practice to improve memory in aging individuals.

Abstract

The default mode network (DMN) plays an important role in age-related cognitive decline. This study aims to explore the modulation effect of two mind–body interventions (Tai Chi Chuan and Baduanjin) on DMN in elderly individuals. Participants between 50 and 70 years old were recruited and randomized into a Tai Chi Chuan, Baduanjin or control group. The Wechsler Memory Scale-Chinese Revision and resting-state fMRI scans were administered at baseline and following 12 weeks of exercise. Seed-based resting-state functional connectivity (rsFC) was calculated. We found that (i) compared to the Baduanjin group, Tai Chi Chuan was significantly associated with increased rsFC between the medial prefrontal cortex (mPFC) and right putamen/caudate and (ii) compared to the control group, Tai Chi Chuan increased posterior cingulate cortex rsFC with the right putamen/caudate, while Baduanjin decreased rsFC between the mPFC and orbital prefrontal gyrus/putamen. Baseline mPFC rsFC with orbital prefrontal gyrus was negatively correlated with visual reproduction subscore. These results suggest that both Tai Chi Chuan and Baduanjin can modulate the DMN, but through different pathways. Elucidating the mechanisms underlying different mind–body interventions may shed light on the development of new methods to prevent age-related diseases as well as other disorders associated with disrupted DMN.

Improve Mindfulness by Interacting with Trustworthy People

It is well established that engaging in contemplative practices such as meditation, yoga, and Tai Chi produces increased mindfulness. But the level of mindfulness varies among people who do not engage in contemplative practice; some being high in mindfulness, while others are very low. So, there must be other factors that contribute to the level of mindfulness. To some extent inheritance (the genes) affects an individual’s mindfulness. But what transpires in the environment during an individual’s life may also be important.

 

Being able to trust other people may be important in developing mindfulness. In today’s Research News article “Does interacting with trustworthy people enhance mindfulness? An experience sampling study of mindfulness in everyday situations.” (See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6485709/), Kudesia and Reina examine the relationships of interacting with trustworthy people with mindfulness. They recruited undergraduate students and had them complete measures of mindful attention and mindful metacognition (noticing their thoughts and feelings about experiences).

 

For 8 days the participants engaged in experience sampling where they received “prompts for experience sampling three times a day (9:30 AM, 2:30 PM, 7:30 PM) over their phone.” They rated their mindfulness and indicated if over the time since the last prompt whether they interacted with someone they deemed a leader or a teammate and if so to describe the interaction, rate the trustworthiness of the leader or teammate, and rate their satisfaction with the interaction. “Common interactions with leaders entailed managers, executives, and recruiters in the workplace, coaches of sports teams, and professors, academic advisors, and residence hall advisors in the university. Common interactions with teammates entailed fellow members of workplace teams, volunteer organizations, sports teams, and class project teams.”

 

They found that the students cooperated for the most part, responding to 75% of the prompts. They separately analyzed responses on what they called a particularized pathway where the particular interactions reported to the prompts were associated with the mindfulness at that same time. There were no significant relationships between trustworthiness and mindfulness for this pathway for either leaders or teammates. They also analyzed responses on what they called a generalized pathway where the average trustworthiness over all interactions were associated with the average overall mindfulness. This produced significant positive relationships between trustworthiness and mindful attention and mindful metacognition for both interactions with leaders and with teammates.

 

The results indicate that overall, in general, when mindfulness is high so is the trustworthiness of both leaders and teammates. These are correlative relationships so it cannot be determined causation. Being mindful of experiences may make one feel that the other person is more trustworthy. That is being cognizant of what’s transpiring in the present moment may focus attention on the other person making them seem more trustworthy. Conversely, being with a trustworthy individual may relax the individual so they become more focused on the present moment (mindful). Finding someone else untrustworthy may help to shift thinking to the future consequences of that untrustworthiness or to ruminating about past occurrences that elicited the feelings of untrustworthiness. This would reduce mindfulness.

 

So, improve mindfulness by interacting with trustworthy people.

Mindfulness is known to increase after meditation interventions. But might features of our everyday situations outside of meditation not also influence our mindfulness from moment-to-moment? Drawing from psychological research on interpersonal trust, we suggest that interacting with trustworthy people could influence the expression of mindfulness. And, extending this research on trust, we further suggest that the influence of trustworthy social interactions on mindfulness could proceed through two pathways: a particularized pathway (where specific interactions that are especially high (or low) in trustworthiness have an immediate influence on mindfulness) or a generalized pathway (where the typical level of trustworthiness a person perceives across all their interactions exerts a more stable influence on their mindfulness). To explore these two pathways, study participants (N = 201) repeatedly reported their current levels of mindfulness and their prior interactions with trustworthy leaders and teammates during their everyday situations using an experience sampling protocol (n¯ = 3,605 reports). Results from mixed-effects models provide little support for the particularized pathway: specific interactions with trustworthy leaders and teammates had little immediate association with mindfulness. The generalized pathway, however, was strongly associated with mindfulness—and remained incrementally predictive beyond relevant individual differences and features of situations. In sum, people who typically interact with more trustworthy partners may become more mindful.

Humans are social animals. This is a great asset for the species as the effort of the individual is amplified by cooperation. In primitive times, this cooperation was essential for survival. But in modern times it is also essential, not for survival but rather for making a living and for the happiness of the individual. This ability to cooperate is so essential to human flourishing that it is built deep into our DNA and is reflected in the structure of the human nervous system. Empathy and compassion are essential for appropriate social engagement and cooperation. In order for these abilities to emerge and strengthen, individuals must be able to see that other people are very much like themselves.

 

Mindfulness has been found to increase prosocial behaviors such as altruism, compassion and empathy. It is not known how mindfulness practice might do this. Mindfulness is actually a complex concept. There are thought to be five distinct facets of mindfulness; describing, observing, acting with awareness, non-judgement, and non-reactivity. It is not known which of these facets of mindfulness are important for the development of empathy.

 

In today’s Research News article “Mediating effect of mindfulness cognition on the development of empathy in a university context.” (See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6472790/), De la Fuente-Anuncibay and colleagues recruited undergraduate students and separated them into students who had and had not previously practiced mindfulness. They were asked to complete measures of empathy and the five facets of mindfulness; describing, observing, acting with awareness, non-judgement, and non-reactivity.

 

They found that the students who practiced mindfulness were higher in their levels of empathy. They further found that mindfulness practice was related to higher levels of empathy directly and also indirectly, such that mindfulness practice was related to the five facets of mindfulness which were in turn related to higher levels of empathy. Further tests of structural models revealed that the mindfulness facets that were mediating the relationship of mindfulness practice with empathy were, observing, describing, and non-reactivity to inner experience.

 

These findings are correlational and so causation cannot be directly determined. But prior research has established that mindfulness practice is a causal factor in improving empathy. So, it is likely that the current results were due to causal connections. This suggests that mindfulness practice increases the individual’s ability to observe and describe present moment experience but not react to it and this improves empathy.

 

Empathy involves the ability to understand and share the feelings of another. The results then suggest that being aware of what is transpiring in the moment but observing it dispassionately may be a key to empathy; seeing others plight without reacting overly emotionally may underlie the development of true empathy. By not reacting and being overwhelmed by one’s own emotions the individual may be better able to share the feelings of the other.

 

So, improve empathy with mindfulness.

Abstract

Numerous interventions propose mindfulness training as a means of improving empathy. Our aim is to analyse the relationship between mindfulness practice and empathy through the mediating process of trait mindfulness. This sample comprised 264 undergraduate students (x¯=24,13years, SD = 11,39). The instruments used were Five Facet Mindfulness Questionnaire and Toronto Empathy Questionnaire. The indirect effect was calculated using 10.000 bootstrap samples for the bootstrap confidence intervals corrected for bias. Empathy improvement is mediated by changes in the cognitions derived from mindfulness (B = .346, p<.01). The direct effect of mindfulness practice on empathy disappears in presence of this mediator (B = .133, p>.05). Mindfulness interventions that aim to improve empathy should focus on three of its components; observing, describing and nonreactivity to inner experience. Given the significance of the results, the research must be extended to larger samples.
