Yoga practice has been shown to have a myriad of benefits for psychological and physical health. It is both an exercise and a mind-body practice that stresses both mental attention to present moment movements, breath control, and flexibility, range of motion, and balance. Yoga, however, can be a challenging physical discipline and injuries are quite common. Indeed, it has been estimated that 20% of yoga practitioners have been injured. It is important to study these injuries to determine their sources in order to reduce their severity and rate of occurrence.

 

In today’s Research News article “Adverse effects of yoga: a national cross-sectional survey.” (See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6664709/), Cramer and colleagues recruited a large sample of German adult yoga practitioners and had them complete a questionnaire on their personal characteristics and the nature of their yoga practice. They were also asked “Have you ever experienced an acute injury or other acute complaint during yoga practice? (Note: here, adverse effects should be listed that occurred suddenly in a specific yoga practice situation).” They were also asked about chronic adverse events. In addition, they were asked to identify the nature of the adverse event and how and when it occurred, how long they had been practicing, had they recovered, and did it occur during a supervised practice.

 

There were a wide variety of yoga practices represented among the respondents. Adverse events were in fairly infrequent with only 0.6 events per 1000 hours of practice. The injuries were for the most part minor involving strains and sprains, but 2% were considered serious. Power yoga was found to be the practice type producing the most adverse events and Hatha yoga the least.

 

21% of the practitioners had experienced an acute adverse musculoskeletal event. Almost 30% of the events occurred during hand, shoulder, or head stands, 24% during forward or backward bends, and 12% during sitting positions. 55% of the events occurred during teacher supervised practice, 22% at home repeating what they had learned in class, and 23% at home self-taught. Only 4% of the adverse events had they not recovered from.

 

Chronic problems occurred in 10% of the practitioners, 91% of which involved chronic back, neck or shoulder pain, tendon shortening or sciatica. They were less likely to occur with Hatha Yoga practice than other types of practice. 52% of the chronic problems were associated with teacher supervised practice, 28% at home repeating what they had learned in class, and 20% at home self-taught. The practitioners had not recovered from only 15 % of the chronic adverse events.

 

The results are interesting and informative and although there were substantial numbers of adverse events reported they were generally minor and significantly less than the numbers that occur per 1000 hours of practice with other exercises such as running, soccer, tennis, skiing, and weight lifting. Exercise is needed to maintain health but it is not without risks. Yoga practice appears to be less risky than most other exercises. So, it should be considered to be a generally safe exercise, particularly with supervised practice avoiding hand, shoulder, and head stands.

 

So, yoga injuries are common but most can be avoided.

Abstract

Background

While yoga is increasingly used for health purposes, its safety has been questioned. The aim of this cross-sectional survey was to analyze yoga-associated adverse effects and their correlates.

Methods

A cross-sectional anonymous national online survey among German yoga practitioners (n = 1702; 88.9% female; 47.2 ± 10.8 years) was conducted from January to June 2016. Participants were queried regarding their yoga practice, i.e. yoga styles used, length and intensity of yoga practice, practice patterns, and whether they had experienced acute or chronic adverse effects of their yoga practice. Independent predictors of acute or chronic adverse effects were identified using multiple logistic regression analyses.

Results

Ashtanga yoga (15.7%), traditional Hatha yoga (14.2%), and Sivananda yoga (22.4%) were the most commonly used yoga styles. 364 (21.4%) yoga users reported 702 acute adverse effects, occurring after a mean of 7.6 ± 8.0 years of yoga practice. The most commonly reported yoga practices that were associated with acute adverse effects were hand-, shoulder- and head stands (29.4%). Using Viniyoga was associated with a decreased risk of acute adverse effects; practicing only by self-study without supervision was associated with higher risk. One hundred seventy-three participants (10.2%) reported 239 chronic adverse effects. The risk of chronic adverse effects was higher in participants with chronic illnesses and those practicing only by self-study without supervision. Most reported adverse effects concerned the musculoskeletal system. 76.9% of acute cases, and 51.6% of chronic cases reached full recovery. On average 0.60 injuries (95% confidence interval = 0.51–0.71) per 1000 h of practice were reported, with Power yoga users reporting the highest rate (1.50 injuries per 1000 h; 95% confidence interval = 0.98–3.15).

Conclusions

One in five adult yoga users reported at least one acute adverse effect in their yoga practice, and one in ten reported at least one chronic adverse effect, mainly musculoskeletal effects. Adverse effects were associated with hand-, shoulder- and head stands; and with yoga self-study without supervision. More than three quarters of of cases reached full recovery. Based on the overall injury rate per 1000 practice hours, yoga appears to be as safe or safer when compared to other exercise types.

Because of great advances in treatment, many patients today are surviving cancer. But cancer survivors frequently suffer from anxiety, depression, mood disturbance, post-traumatic stress disorder (PTSD), sleep disturbance, fatigue, sexual dysfunction, loss of personal control, impaired quality of life, and psychiatric symptoms which have been found to persist even ten years after remission. Also, cancer survivors can have to deal with a heightened fear of reoccurrence. This is particularly true with metastatic cancer. So, safe and effective treatments for the symptoms in cancer and the physical and psychological effects of the treatments are needed.

 

Mindfulness training has been shown to help with general cancer recovery . Yoga is both an exercise and a mindfulness practice that has also been shown to be helpful with the residual symptoms in cancer survivors, the psychological and physical ability to deal with cancer treatment and improves sleep. So, it’s reasonable to further explore the potential benefits of yoga practice to improve sleep and relieve fatigue in patients who have survived cancer.

 

In today’s Research News article “Influence of Yoga on Cancer-Related Fatigue and on Mediational Relationships Between Changes in Sleep and Cancer-Related Fatigue: A Nationwide, Multicenter Randomized Controlled Trial of Yoga in Cancer Survivors.” (See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6552348/), Lin and colleagues recruited patients who had survived a variety of different cancers, and who had completed primary care and who had sleep disturbance, from centers all over the U.S. They were randomly assigned them to receive standard care plus yoga training or standard care alone. Yoga training occurred for 75 minutes, twice a week, for 4 weeks and consisted of breathing exercises, physical alignment postures, and mindfulness exercises. They were measured before and after treatment for cancer related fatigue, including general, physical, emotional, mental, and vigor domains, and sleep, including subjective sleep quality, sleep latency, sleep duration, habitual sleep efficiency, sleep disturbance, sleep medication use, and daytime dysfunction.

 

They found that in comparison to baseline and the treatment-as-usual group, the participants who received yoga training had significant improvements in all domains of fatigue with moderate effect sizes. The yoga group also had significant improvements in sleep, including increases in overall sleep quality and subjective sleep quality and decreases in daytime dysfunction. Finally, they found, employing path analysis, that yoga training improved fatigue directly and indirectly by improving sleep that in turn improved sleep.

 

These are potentially important findings. Fatigue and sleep disturbance are difficult residual problems. There is a need to have long-term follow-up of these patients to determine whether yoga practice produces sustained relief of sleep disturbance and fatigue. Nevertheless, the findings show that at least on the short-term, yoga practice can address these symptoms and as a result improve the well-being and quality of life of cancer survivors. In addition, practicing yoga has been shown to produce widespread benefits for the physical and psychological health of practitioners. So, yoga practice would seem to be an excellent additional treatment for patients who have survived cancer.

 

So, improve sleep and relieve fatigue in cancer survivors with yoga.

Abstract

Background: Cancer-related fatigue (CRF) often co-occurs with sleep disturbance and is one of the most pervasive toxicities resulting from cancer and its treatment. We and other investigators have previously reported that yoga therapy can improve sleep quality in cancer patients and survivors. No nationwide multicenter phase III randomized controlled trial (RCT) has investigated whether yoga therapy improves CRF or whether improvements in sleep mediate the effect of yoga on CRF. We examined the effect of a standardized, 4-week, yoga therapy program (Yoga for Cancer Survivors [YOCAS]) on CRF and whether YOCAS-induced changes in sleep mediated changes in CRF among survivors. Study Design and Methods: Four hundred ten cancer survivors were recruited to a nationwide multicenter phase III RCT comparing the effect of YOCAS to standard survivorship care on CRF and examining the mediating effects of changes in sleep, stemming from yoga, on changes in CRF. CRF was assessed by the Multidimensional Fatigue Symptom Inventory. Sleep was assessed via the Pittsburgh Sleep Quality Index. Between- and within-group intervention effects on CRF were assessed by analysis of covariance and 2-tailed t test, respectively. Path analysis was used to evaluate mediation. Results: YOCAS participants demonstrated significantly greater improvements in CRF compared with participants in standard survivorship care at post-intervention (P < .01). Improvements in overall sleep quality and reductions in daytime dysfunction (eg, excessive napping) resulting from yoga significantly mediated the effect of yoga on CRF (22% and 37%, respectively, both P < .01). Conclusions: YOCAS is effective for treating CRF among cancer survivors; 22% to 37% of the improvements in CRF from yoga therapy result from improvements in sleep quality and daytime dysfunction. Oncologists should consider prescribing yoga to cancer survivors for treating CRF and sleep disturbance.

People with larger bodies often have trouble with joint pain; yoga can help by improving the body’s alignment to reduce strain on joints by allowing the frame to bear more of the body’s weight.” – Ann Pizer

 

Obesity has become an epidemic in the industrialized world. In the U.S. the incidence of obesity, defined as a Body Mass Index (BMI) of 30 or above has more than doubled over the last 35 years to currently around 35% of the population, while two thirds of the population are considered overweight or obese (BMI > 25). Obesity has been found to shorten life expectancy by eight years and extreme obesity by 14 years. This occurs because obesity is associated with cardiovascular problems such as coronary heart disease and hypertension, stroke, metabolic syndrome, diabetes, cancer, arthritis, and others.

 

Obviously, there is a need for effective treatments to obese individuals. But, despite copious research and a myriad of dietary and exercise programs, there still is no safe and effective treatment. Mindfulness is known to be associated with lower risk for obesityalter eating behavior and improve health in obesity. This suggests that mindfulness training may be an effective treatment for overeating and obesity alone or in combination with other therapies. Yoga may be particularly beneficial for the obese as it is both a mindfulness practice and an exercise. Yoga practice has been shown to have a myriad of physical and psychological benefits. These include significant loss in weight and body mass index (BMI), resting metabolism, and body fat in obese women with Type 2 diabetes and improve health in the obese.

 

In today’s Research News article “Quality of Life in Yoga Experienced and Yoga Naïve Asian Indian Adults with Obesity.” (See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6515061/), Telles and colleagues recruited overweight and obese (BMI>25) adults (aged 20-59 years) and assessed them for participation in yoga practice and their quality of life, including general self-esteem, enjoyment in physical activities, satisfactory social contacts, satisfaction concerning work, sexual pleasure, and focus on eating behavior.

 

They found that in comparison to non-participants in yoga practice, the yoga participants had significantly higher overall quality of life including higher levels of general self-esteem, enjoyment in physical activities, satisfactory social contacts, and satisfaction concerning work. Hence, participation in yoga practice was found to be associated with significantly higher quality of life in overweight and obese individuals.

 

These findings are correlational and causation cannot be determined. It is possible that yoga practice causes improved quality of life, or that people with high quality of life tend to engage in yoga practice, or that some other factor, e.g. affluence, large social network, results in higher levels of both. Nevertheless, it is clear that practicing yoga is associated with better, more enjoyable lives, that overweight and obese yoga practitioners have a higher quality of life.

Abstract

Background

Obesity adversely affects quality of life which then acts as a barrier to weight loss and weight loss maintenance. Hence, those interventions which positively influence the quality of life along with weight reduction are considered useful for sustained weight loss in persons with obesity. An earlier study showed better quality of life in obese adults who had experience of yoga compared to yoga naïve obese adults. However, the main limitation of the study was the small sample size (n=20 in each group).

Objective

The present study aimed to determine whether with larger sample sizes the quality of life would differ in yoga experienced compared to yoga naïve adults with obesity.

Methods

There were 596 Asian Indian obese adults (age range 20 to 59 years; group mean age ± SD; 43.9 ± 9.9 years): of whom (i) 298 were yoga experienced (154 females; group mean age ± SD; 44.0 ± 9.8 years) with a minimum of 1 month of experience in yoga practice and (ii) 298 were yoga naïve (154 females; group mean age ± SD; 43.8 ± 10.0 years). All the participants were assessed for quality of life using the Moorehead–Ardelt quality of life questionnaire II. Data were drawn from a larger nationwide trial which assessed the effects of yoga compared to nutritional advice on obesity over a one-year follow-up period (CTRI/2018/05/014077).

Results

There were higher participant-reported outcomes for four out of six aspects of quality of life in the yoga experienced compared to the yoga naïve (p < 0.008, based on t values of the least squares linear regression analyses, Bonferroni adjusted, and adjusted for age, gender, and BMI as covariates). These were enjoyment in physical activities, ability to work, self-esteem, and social satisfaction.

Conclusion

Obese adults with yoga experience appear to have better quality of life in specific aspects, compared to yoga naïve persons with a comparable degree of obesity.

Around 2 ¼ million people are incarcerated in the United States. Even though prisons are euphemistically labelled correctional facilities very little correction actually occurs. This is supported by the rates of recidivism. About three quarters of prisoners who are released commit crimes and are sent back to prison within 5-years. The lack of actual treatment for the prisoners leaves them ill equipped to engage positively in society either inside or outside of prison. Hence, there is a need for effective treatment programs that help the prisoners while in prison and prepares them for life outside the prison.

 

Contemplative practices are well suited to the prison environment. Mindfulness training teaches skills that may be very important for prisoners. In particular, it puts the practitioner in touch with their own bodies and feelings. It improves present moment awareness and helps to overcome rumination about the past and negative thinking about the future. It also relieves stress and improves overall health and well-being. Finally, mindfulness training has been shown to be effective in treating depressionanxiety, and anger. It has also been shown to help overcome trauma in male prisoners. Yoga practice, because of its mindfulness plus physical exercise characteristics, would seem to be ideal for the needs of an incarcerated population. Indeed, it has been shown to be beneficial for prisoners.

 

In today’s Research News article “Imprisoning Yoga: Yoga Practice May Increase the Character Maturity of Male Prison Inmates.” (See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6584840/), Kerekes and colleagues recruited male adult prisoners and randomly assigned them to 10 weeks, once a week, for 90 minutes of either yoga training or physical activity. They were measured before and after training for temperament (novelty seeking, harm avoidance, reward dependence, and persistence) and character self-directedness, cooperativeness, and self-transcendence).

 

They found that both groups showed significant improvements in novelty seeking, harm avoidance, and self-directedness. The prisoners that practiced yoga had a significantly greater improvement in self-directedness than the physical exercise group. Hence it appears that engaging in exercise in general reduces novelty seeking and harm avoidance and increases self-directedness, but yoga practice produces greater improvements in self-directedness.

 

Engaging in exercise appears to be beneficial for male prisoners. Novelty seeking tends to drive impulsiveness that is a problem for prisoners. Thus, reducing novelty seeking should improve their behavior. Exercise also appears to increase the prisoner’s ability to control their behavior by increasing self-directedness. Yoga is a disciplined practice. So, it is no surprise that it would produce greater self-discipline in the practitioner. This should assist the prisoner in having greater control of their behavior, which should, in turn, improve their ability to function effectively in prison and in society when they are released.

 

So, improve prisoners’ self-directedness with yoga.

Abstract

Background: A specific personality profile, characterized by low character maturity (low scores on the self-directedness and cooperativeness character dimensions) and high scores on the novelty seeking temperament dimension of the temperament and character inventory (TCI), has been associated with aggressive antisocial behavior in male prison inmates. It has also been shown that yoga practiced in Swedish correctional facilities has positive effects on the inmates’ well-being and on risk factors associated with criminal recidivism (e.g., antisocial behavior). In this study, we aimed to investigate whether the positive effect of yoga practice on inmates’ behaviors could be extended to include eventual changes in their personality profile.

Methods: Male prison inmates (N = 111) in Sweden participated in a randomized controlled 10-week long yoga intervention trial. Participants were randomly assigned to either a yoga group (one class a week; n = 57) or a control group (free of choice weekly physical activity; n = 54). All the inmates completed the TCI questionnaire before and after the intervention period as part of an assessment battery.

Results: After the 10-week-long intervention period male inmates scored significantly lower on the novelty seeking and the harm avoidance and significantly higher on the self-directedness dimensions of the TCI. There was a significant medium strong interaction effect between time and group belonging for the self-directedness dimension of character favoring the yoga group.

Conclusion: A 10-week-long yoga practice intervention among male inmates in Swedish correctional facilities increased the inmates’ character maturity, improving such abilities as their capability to take responsibility, feel more purposeful, and being more self-acceptant—features that previously were found to be associated with decreased aggressive antisocial behavior.

Yoga is like music. The rhythm of the body, the melody of the mind, and the harmony of the soul creates the symphony of life.” B.K.S. Iyengar

 

Yoga practice has been shown to have a large number of beneficial effects on the psychological, emotional, and physical health of the individual and is helpful in the treatment of mental and physical illness. The acceptance of yoga practice has spread from the home and yoga studios to its application with children in schools. Studies of these school programs have found that yoga practice produces a wide variety of positive emotional, psychosocial, and physical benefits.

Teachers also note improvements in their students following yoga practice. These include improved classroom behavior, social–emotional, and cognitive skills. In addition, school records, academic tests, and physiological measures have shown that yoga practice produces improvements in student grades and academic performance.

 

To better understand the effects of yoga practice on adolescents it is important to take into consideration that yoga is a not only a mindfulness practice, but it is also a physical exercise. It is important to compare yoga practice in schools to other forms of physical exercise to determine if yoga produces its benefits because of improved mindfulness or because of the physical exercise provided.

 

In today’s Research News article “.” (See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6521753/), Vhavle and colleagues recruited adolescents and randomly assigned them to participate in school in 1-hour daily practice for 2 months of either yoga or physical exercise. They were measured before and after training for executive function, cognition, attention, visual scanning, and memory with a numerical and an alphabetical trail making test.

 

All participants had fast times in completing the trail making tests indicating high levels of ability. The researchers found that both groups took significantly longer after training than before for the numerical trail making test. Only the yoga group took a significantly shorter time to complete the alphabetical trail making test after training than before and this was significantly different from the exercise group.

 

The numerical trail making test emphasizes visual scanning and cognitive function. The scores were very fast and so all of the students were very adept at this test. Hence, there may have been a ceiling effects making it impossible to detect further improvements. On the other hand, the alphabetical trail making test measures executive function, attention, and working memory and yoga practice produced significant improvement s in performance. These results then, suggests that school-based yoga practice may enhance adolescents’ thinking skills better than physical exercise. This suggests that the mindfulness component and not the exercise component of yoga practice is the most important aspect of the practice for the improvement of thinking ability in the youths.

 

So, improve thinking in adolescents with yoga practice.

 

Studies also show that exercise facilitates children’s executive function (i.e., processes required to select, organize, and properly initiate goal-directed actions) by increasing activation in the prefrontal cortex and serotonergic system. By integrating physical movement with breathing exercises and mindful awareness, yoga serves as a promising form of physical and cognitive training to enhance learning-related outcomes.” – Yoga4Classrooms

Abstract

Purpose:

Executive function, attention, and memory are an important indicator of cognitive health in children. In this study, we analyze the effect of yoga and physical exercise on executive functioning, attention, and memory.

Methods:

In this prospective two-armed randomized controlled trial, around 802 students from ten schools across four districts were randomized to receive daily 1 h yoga training (n = 411) or physical exercise (n = 391) for 2 months. Executive function, attention, and memory were studied using Trail Making Test (TMT). Yoga (n = 377) and physical exercise (n = 371) students contributed data to the analyses. The data were analyzed using intention-to-treat approach using Student’s t-test.

Results:

There was a significant increase in numerical TMT (TMTN) values within yoga (t = −2.17; P < 0.03) and physical activity (PA) (t = −3.37; P < 0.001) groups following interventional period. However, there was no significant change in TMTN between yoga and PA groups (t = 0.44; P = 0.66). There was a significant increase in alphabetical TMT (TMTA) values within yoga (t = 6.21; P < 0.00) and PA groups (t = 1.19; P < 0.234) following interventional period. However, there was no significant change in TMTA between yoga and PA groups (t = 3.46; P = 0.001).

Conclusion:

The results suggest that yoga improves executive function, attention, and working memory as effectively as physical exercise intervention in adolescent schoolchildren.

More than 35 million people worldwide and 1.2 million people in the United States are living with HIV infection. These include a significant number of children and adolescents. In 1996, the advent of the protease inhibitor and the so-called cocktail changed the prognosis for HIV. Since this development a 20-year-old infected with HIV can now expect to live on average to age 69. Hence, living with HIV is a long-term reality for a very large group of people. People living with HIV infection experience a wide array of physical and psychological symptoms which decrease their perceived quality of life. The symptoms include chronic pain, muscle aches, anxiety, depression, weakness, fear/worries, difficulty with concentration, concerns regarding the need to interact with a complex healthcare system, stigma, and the challenge to come to terms with a new identity as someone living with HIV.

 

Mindfulness training has been shown to improve psychological well-being, lower depression and strengthen the immune system of patients with HIV infection. Yoga practice has also been found to be effective in treating HIV. Most studies, however, focus on adult patients with HIV. There are, however, a large number of children and adolescents who are infected with HIV. Hence it makes sense to examine the ability of yoga training to treat HIV infection in children and adolescents.

 

In today’s Research News article “Effect of Yoga on Immune Parameters, Cognitive Functions, and Quality of Life among HIV-Positive Children/Adolescents: A Pilot Study.” (See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6521755/), Chandra and colleagues recruited children and adolescents (aged 8 to 18 years) who had HIV infection from a HIV/AIDS rehabilitation center. Treatment as usual was continued while they were provided with daily 1-hour yoga practice sessions for 6 months. They were measured before and after training for immune system function, health-related quality of life, fatigue-related quality of life, depression, and cognitive function.

 

They found that in comparison to baseline, yoga practice produced a significant decrease in in HIV viral load and a significant increase in plasma CD4 counts. There was also a significant increase in health-related quality of life, including the health and general activities, feelings, getting along with others, and about school subscales, and fatigue-related quality of life, including general fatigue (b) sleep fatigue, and (c) cognitive fatigue. After yoga practice the children and adolescents had significant improvements in cognitive function and increases in depression.

 

The observed effectiveness of yoga practice for the treatment of HIV infected children and adolescents, parallels that observed in prior studies with adults. These include reducing the presence of the virus in the blood, improvement of immune system function, quality of life, and mental abilities. This was a pilot study and did not have a control condition, so conclusions need to be tempered. The results, though are encouraging and should motivate conducting a large randomized controlled trial. Regardless, the results are very encouraging and suggest that yoga practice is beneficial for the health and well-being of youths infected with HIV.

 

So, improve the symptoms of HIV infection in children with yoga.

Abstract

Context:

HIV/AIDS individuals have problems relating to immune system, quality of life (QOL), and cognitive functions (CFs). Yoga is found to be useful in similar conditions. Hardly, any work is reported on yoga for HIV-positive adults/adolescents. Hence, this study is important.

Aim:

The aim of the study is to determine the effect of yoga on immune parameters, CFs, and QOL of HIV-positive children/adolescents.

Settings and Design:

Single-group, pre–post study with 4-month yoga intervention.

Methods:

The study had 18 children from an HIV/AIDS rehabilitation center for children/adolescents. CD4, CD8, CD4/CD8 ratio, and viral loads were studied. CF tests included six letter cancellation test, symbol digit modalities test, digit-span forward backward test, and Stroop tests. QOL was assessed using PedsQL-QOL and fatigue questionnaire. Depression was assessed using CDI2-SR.

Statistical Analysis Used:

t-test and Wilcoxon signed-rank tests, as applicable.

Results:

The study included 18 children/adolescents. There was improvement in general health of the participants. There was statistically significant increase in CD4 cells counts (p = 0.039) and significant decrease in viral load (p = 0.041). CD4/CD8 ratio moved to normal range. QOL significantly improved. CFs had mixed results with improved psychomotor performance (PP) and reduced executive functions.

Conclusions:

There was improvement in general health and immune parameters. While depression increased, QOL improved. CFs showed mixed results with improved PP and reduced executive functions.

Chronic diseases are long-term illnesses that last more than a year, limit the individual’s ability to conduct their lives, and require medical attention. These include cancer, cardiovascular diseases, digestive diseases, skin diseases, diabetes, pulmonary diseases, neurological diseases, arthritis, chronic pain, obesity, and mental illness. Chronic diseases affect approximately 40% of the total adult population. As such they put a tremendous strain on the medical systems not to mention on individuals and families.

 

People with chronic diseases often also suffer from chronic fatigue, pain, stress, and mood disorders especially depression. They also have difficulty working and have a decreased quality of life. So, it is very important to find effective means of treating patients with chronic illnesses. Yoga practice has been found to be helpful for patients with many of these illnesses. But it is important to further study the effectiveness of yoga practice for patients with chronic illnesses.

 

In today’s Research News article “Mental Wellbeing, Quality of Life, and Perception of Chronic Illness in Yoga-Experienced Compared with Yoga-Naïve Patients.” (See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6542302/), Telles and colleagues recruited patients with chronic noncommunicable diseases and asked if they practiced yoga. They were measured for their perception of their disease, mental well-being, and quality of life, including psychological, environmental, and total facets.

 

They found that the experienced yoga practitioners had significantly greater mental well-being, perceived control over their illnesses, and in their psychological, environmental, and total quality of life. They also found that the longer that the patients had been practicing yoga the higher the scores on perception of their illness, mental well-being, and quality of life.

 

These findings are not due to active manipulation and it is possible that patients who chose to practice yoga are characteristically different from patients who do not. These findings need to be replicated in a randomized controlled trial. With this caveat in mind the results suggest that yoga practice is beneficial for patients with chronic noncommunicable diseases, improving their perception of their illness, mental well-being and quality of life. Chronic diseases are difficult to deal with. So, it is important to find the potential of yoga practice to help relieve at least some of their suffering.

 

So, improve the psychological well-being of the chronically ill with yoga.

Abstract

Background

Perception of chronic illness and a positive outlook improve recovery, and yoga can improve wellbeing. This study aimed to compare perception, mental wellbeing, and quality of life in yoga-experienced compared with yoga-naïve patients with chronic illness and to determine whether the duration of yoga practice in the yoga-experienced group had any correlation with the perception of illness, mental wellbeing, and quality of life.

Material/Methods

A cross-sectional comparative study recruited 419 patients with chronic non-communicable disease. Yoga-experienced patients (n=150) (mean age, 41.9±13.6 years) and yoga-naïve patients (n=269) (mean age, 41.2±12.6 years) were assessed for the perception of their illness, mental wellbeing, and quality of life using the Warwick-Edinburgh mental wellbeing scale (WEMWBS) and the World Health Organization quality of life (WHOQOL-BREF) self-reporting questionnaire.

Results

The yoga-experienced group had significantly increased mental wellbeing, personal control as a dimension of their perception of illness, and psychological and environmental quality of life compared with the yoga-naïve group (all, p<0.05), when comparisons were made using the Mann-Whitney U test. The duration of yoga practised in months was positively-correlated with mental wellbeing and different aspects of quality of life. There was a negative correlation with the perception of illness suggesting that the illness was perceived to be less severe (all, p<0.05) when correlations were made using Spearman’s rank correlation coefficient.

Conclusions

In patients with chronic illness, yoga improved mental wellbeing, aspects of quality of life, and resulted in a positive perception of illness.

Tai chi helps with back pain in several ways. It strengthens the muscles in your abdomen and pelvic area that are crucial to supporting the lower back; it improves your balance and flexibility; and it makes you more aware of your posture when you sit, stand, and walk.” – Benjamin Kligler

 

Low Back Pain is the leading cause of disability worldwide and affects between 6% to 15% of the population. It is estimated, however, that 80% of the population will experience back pain sometime during their lives. The pain interferes with daily living and with work, interfering with productivity and creating absences. There are varied treatments for low back pain including chiropractic care, acupuncture, biofeedback, physical therapy, cognitive behavioral therapy, massage, surgery, opiate pain killing drugs, steroid injections, and muscle relaxant drugs. These therapies are sometimes effective particularly for acute back pain. But, for chronic conditions the treatments are less effective and often require continuing treatment for years and opiate pain killers are dangerous and can lead to abuse, addiction, and fatal overdoses. Obviously, there is a need for safe and effective treatments for low back pain that are low cost and don’t have troublesome side effects.

 

Pain involves both physical and psychological issues. Physically, exercise can be helpful in strengthening the back to prevent or relieve pain. Psychologically, the stress, fear, and anxiety produced by pain tends to elicit responses that actually amplify the pain. So, reducing the emotional reactions to pain may be helpful in pain management. Mindfulness practices have been shown to improve emotion regulation producing more adaptive and less maladaptive responses to emotions. Indeed, mindfulness practices are effective in treating pain and have been shown to be safe and effective in the management of low back pain. Tai Chi, Qigong, and yoga  are both exercises and mindfulness practices that have been found to be effective for back pain. There is accumulating evidence. So, it is useful to take a step back and summarize what has been found.

 

In today’s Research News article “Are Mindful Exercises Safe and Beneficial for Treating Chronic Lower Back Pain? A Systematic Review and Meta-Analysis of Randomized Controlled Trials.” (See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6571780/), Zou and colleagues review, summarize, and perform a meta-analysis of the effects of the mindful movement practices of Tai Chi, Qigong, and yoga on chronic low back pain. The identified 17 randomized controlled trials that included a total of 2022 participants.

 

They report that the published research found that both Tai Chi and yoga practices produced significant reductions in pain intensity and back specific disability in patients with chronic low back pain. There were no reported adverse events with Tai Chi practice but there were a few adverse events reported with yoga practice. So, although both are effective in treating chronic low back pain, more care must be taken with yoga practice to protect against injury.

 

The results of the published research strongly suggests that Tai Chi  practice should be prescribed for chronic low back pain, reducing pain and disability. In addition, Tai Chi is a gentle and safe mindfulness practice. It is appropriate for all ages including the elderly and for individuals with illnesses that limit their activities or range of motion. It is inexpensive to administer, can be performed in groups or alone, at home or in a facility, and can be quickly learned. In addition, it can be practiced in social groups. This can make it fun, improving the likelihood of long-term engagement in the practice. So, Tai Chi practice would appear to be an excellent gentle practice to chronic low back pain.

 

So, improve chronic low back pain with mindful movement practices.

Abstract

Background: Chronic low back pain (CLBP) is a common health issue worldwide. Tai Chi, Qigong, and Yoga, as the most widely practiced mindful exercises, have promising effects for CLBP-specific symptoms. Objective: We therefore conducted a comprehensive review investigating the effects of mindful exercises versus active and/or non-active controls while evaluating the safety and pain-related effects of mindful exercises in adults with CLBP. Methods: We searched five databases (MEDLINE, EMBASE, SCOPUS, Web of Science, and Cochrane Library) from inception to February 2019. Two investigators independently selected 17 eligible randomized controlled trials (RCT) against inclusion and exclusion criteria, followed by data extraction and study quality assessment. Standardized mean difference (SMD) was used to determine the magnitude of mindful exercises versus controls on pain- and disease-specific outcome measures. Results: As compared to control groups, we observed significantly favorable effects of mindful exercises on reducing pain intensity (SMD = −0.37, 95% CI −0.5 to −0.23, p < 0.001, I2 = 45.9 %) and disability (SMD = −0.39, 95% CI −0.49 to −0.28, p < 0.001, I2 = 0 %). When compared with active control alone, mindful exercises showed significantly reduced pain intensity (SMD = −0.40, p < 0.001). Furthermore, of the three mindful exercises, Tai Chi has a significantly superior effect on pain management (SMD= −0.75, 95% CI −1.05 to −0.46, p < 0.001), whereas Yoga-related adverse events were reported in five studies. Conclusion: Findings of our systematic review suggest that mindful exercises (Tai Chi and Qigong) may be beneficial for CLBP symptomatic management. In particular, Tai Chi appears to have a superior effect in reducing pain intensity irrespective of non-control comparison or active control comparison (conventional exercises, core training, and physical therapy programs). Importantly, training in these mindful exercises should be implemented with certified instructors to ensure quality of movement and injury prevention.

In today’s Research News article “Mind-body practices for people living with HIV: a systematic scoping review.” (See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6560810/ ), Ramirez-Garcia and colleagues review and summarize the published research studies of the effectiveness of mind-body practices for the treatment of the symptoms of HIV infection. “Mind-body practices include Tai Chi, Qigong, yoga, meditation, and all types of relaxation” training. They identified 84 published research studies.

 

They report that these studies found that for patients with HIV, Mindfulness-Based Stress Reduction (MBSR) decrease the physical symptoms and the side effects of the drug treatment, and improves the patient’s psychological state. They also report that Cognitive Behavioral Therapy and the combining at least three relaxation techniques decreases the patient’s physical and psychological symptoms, and increase quality of life and health. Yoga practice was also found to lower the patient’s blood pressure. Tai Chi, Qigong, and relaxation techniques were found to improve the patient’s physical and psychological condition.

 

Hence the accumulated research suggests that mind-body therapies in addition to antiretroviral treatment are safe and effective treatments to improve the health, well-being, and quality of life of patients living with HIV. This is important as these patients will be living for many years with the symptoms of HIV and the side effects of its treatment. The addition of mind-body practices can help make living with HIV more tolerable and improve the patients’ lives.

 

So, improve physical and psychological symptoms and quality of life in people living with HIV with Mindbody practices.

Abstract

Background

Mind-body practices are frequently used by people living with HIV to reduce symptoms and improve wellbeing. These include Tai Chi, Qigong, yoga, meditation, and all types of relaxation. Although there is substantial research on the efficacy of mind-body practices in people living with HIV, there is no summary of the available evidence on these practices. The aim of this scoping review is to map available evidence of mind-body practices in people living with HIV.

Methods

The Arksey and O’Malley (Int J Soc Res Methodol 8:19-32, 2005) methodological framework was used. A search of 16 peer-review and grey literature databases, websites, and relevant journals (1983–2015) was conducted. To identify relevant studies, two reviewers independently applied the inclusion criteria to all abstracts or full articles. Inclusion criteria were: participants were people living with HIV; the intervention was any mind-body practice; and the study design was any research study evaluating one or several of these practices. Data extraction and risk of bias assessment were performed by one reviewer and checked by a second, as needed, using the criteria that Cochrane Collaboration recommends for systematic reviews of interventions (Higgins and Green, Cochrane handbook for systematic reviews of intervention. 2011). A tabular and narrative synthesis was carried out for each mind-body practice.

Results

One hundred thirty-six documents drawing on 84 studies met the inclusion criteria. The most widely studied mind-body practice was a combination of least three relaxation techniques (n = 20), followed in declining order by meditation (n = 17), progressive muscle relaxation (n = 10), yoga (n = 9) and hypnosis (n = 8). Slightly over half (47/84) of studies used a RCT design. The interventions were mainly (46/84) conducted in groups and most (51/84) included daily individual home practice. All but two studies were unblinded to participants.

Conclusion

The amount of available research on mind-body practices varies by practice. Almost half of the studies in this review were at high risk of bias. However, mindfulness, a combination of least three relaxation techniques and cognitive behavioral strategies, and yoga show encouraging results in decreasing physical and psychological symptoms and improving quality of life and health in people living with HIV. More rigorous studies are necessary to confirm the results of Tai Chi, Qigong, and some relaxation techniques.

Receiving a diagnosis of cancer has a huge impact on most people. Feelings of depression, anxiety, and fear are very common and are normal responses to this life-changing and potentially life-ending experience. But, surviving cancer carries with it a number of problems. Painful Chemotherapy-induced peripheral neuropathy is a frequent side effect of cancer treatment. This Neuropathy is characterized by damage to the nervous system resulting from chemotherapy. Between 30-100% of patients can experience this neuropathy.  It can affect patients motor abilities including walking, and balance. But it can also affect driving, relationships, work, writing, exercise, sleep and sexual activity.

 

Mindfulness training has been shown to help with cancer recovery and help to relieve chronic pain. It can also help treat the residual physical and psychological symptoms, including stress,  sleep disturbance, and anxiety and depression. Yoga practice is a form of mindfulness training that has been shown to help relieve chronic pain and be beneficial for cancer patients.  So, it makes sense to examine the ability of yoga practice to help relieve the symptoms of chemotherapy-induced peripheral neuropathy.

 

In today’s Research News article “Impact of Somatic Yoga and Meditation on Fall Risk, Function, and Quality of Life for Chemotherapy-Induced Peripheral Neuropathy Syndrome in Cancer Survivors.” (See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6537287/), Galantino and colleagues recruited cancer survivors who had completed all treatments but who had some degree of peripheral neuropathy. They participated in a 90 minute, twice a week for 8-weeks, Hatha yoga program including postures, breathwork, and meditation. They were also asked to practice at home. They were measured before and after treatment for motor functions, balance, lower extremity flexibility, pain, neurotoxicity, perceived stress, sleep quality, spiritual efficacy, fear of falling, vibration sense, and salivary cortisol. The participants were asked to record their reflections on their yoga practice in a diary.

 

They found that at the completion of the yoga training the patients had significantly improved mobility, flexibility, balance, risk of falling, perceived pain, pain interference with life activities, sensory systems, muscular weakness, foot vibration sensitivity, and perceived stress. They did not find any adverse effects of the yoga practice on the patients. Qualitative analysis of the patient diaries revealed that the patients noted improvements in enhanced sensations in the extremities, that the yoga practice helped them in managing their symptoms, that the improvement in physical function allowed return to work and re-engagement in hobbies, greater ability to relax, and enjoyment of the social aspects of the yoga practice.

 

It should be noted that this was a small pilot study and there wasn’t a control condition so the results need to be interpreted with caution. But the results are very encouraging and suggest that a large randomized controlled trial is justified and needed to verify the efficacy of the yoga program. But prior to the program there was no improvement over time, so participation in the program likely produced the benefits. The benefits obtained in this study are significant and important contributing to the daily functions, mental and physical health of the patients.

 

Chemotherapy-induced peripheral neuropathy is painful, persistent, disruptive, and debilitating. The degree of improvement seen in the patients after yoga practice markedly improved their symptoms and greatly reduced their suffering. Importantly, after yoga practice the symptoms of peripheral neuropathy were greatly reduced allowing the patients to better function and to enjoy their lives.

 

So, improve mobility and quality of life in patients with chemotherapy-induced peripheral neuropathy with yoga.

Abstract

Objective. Chemotherapy-induced peripheral neuropathy (CIPN) syndrome causes significant pain as an adverse effect of treatment, with few nonpharmacological interventions tested. A somatic yoga and meditation (SYM) intervention on functional outcomes and quality of life (QOL) was investigated. Design and methods. Individuals diagnosed with CIPN were enrolled in an open-label, single-arm, mixed-methods feasibility trial. Participants and Setting. In an outpatient rehabilitation center, ten participants with median age 64.4 years (47-81) attended 61% of the sessions with no adverse events. Intervention. SYM twice a week for 8 weeks for 1.5 hours, with home program and journaling. Main outcome measures. Primary functional outcomes included Sit and Reach (SR), Functional Reach (FR), and Timed Up and Go (TUG). Self-reported Patient Neurotoxicity Questionnaire (PNQ) and Functional Assessment of Cancer Therapy—Neurotoxicity (FACT-GOG-NTX) were secondary CIPN outcomes. Biomarkers included salivary cortisol (stress) and bioesthesiometer (vibration). Results: Quantitative findings. Significant improvements were found in flexibility (SR; P = .006); balance (FR; P = .001) and fall risk (TUG; P = .004). PNQ improved significantly (P = .003) with other measures improving non-significantly. Qualitative findings. Five themes emerged: (1) vacillation of CIPN pain perception over time; (2) transferability of skills to daily activities; (3) improvement in physical function; (4) perceived relaxation as an effect of SYM; and (5) group engagement provided a social context for not feeling isolated with CIPN. Conclusion. Preliminary data suggest SYM may improve QOL, flexibility, and balance in cancer survivors with CIPN, with a fully powered randomized controlled trial indicated.
