Improve Fibromyalgia with Tai Chi

Fibromyalgia is a mysterious disorder whose causes are unknown. It is very common affecting over 5 million people in the U.S., about 2% of the population with about 7 times more women affected than men. It is characterized by widespread pain, abnormal pain processing, sleep disturbance, and fatigue that lead to psychological distress. Fibromyalgia may also have morning stiffness, tingling or numbness in hands and feet, headaches, including migraines, irritable bowel syndrome, sleep disturbances, thinking and memory problems, and painful menstrual periods. The symptoms are so severe and debilitating that about half the patients are unable to perform routine daily functions and about a third have to stop work. Although it is not itself fatal, suicide rates are higher in fibromyalgia sufferers.

 

There are no completely effective treatments for fibromyalgia. Drugs and lifestyle changes are recommended but produce only limited symptomatic relief but also can produce unwanted side effects. Alternatively, mindfulness practices have been shown to be effective in reducing pain from fibromyalgia. Tai Chi is an ancient mindfulness practice involving slow prescribed movements. It is gentle and completely safe, can be used with the elderly and sickly, is inexpensive to administer, can be performed in groups or alone, at home or in a facility or even public park, and can be quickly learned. In addition, it can also be practiced in social groups without professional supervision. This can make it fun, improving the likelihood of long-term engagement in the practice. In addition, Tai Chi  has been shown to reduce pain in patients with spinal cord injury. Hence, Tai Chi may be an excellent treatment for the symptoms of fibromyalgia.

 

In today’s Research News article “A randomized controlled trial of 8-form Tai chi improves symptoms and functional mobility in fibromyalgia patients.” See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC5571653/, Jones and colleagues recruited fibromyalgia sufferers and randomly assigned them to either practice Tai Chi or receive education on fibromyalgia from physicians, dieticians, and counselors for 90 minutes, twice a week, for 12 weeks. Homework and home practice was also prescribed. The participants were measured before and after the 12-week treatment period for fibromyalgia impact, including fatigue, morning tiredness, stiffness, depression, anxiety, work ability, and physical function

 

They found that the Tai Chi group in comparison to baseline and the education group had significant decreases in fibromyalgia impact, pain, sleep quality, self-efficacy, functional mobility, and balance. There were also significant reductions in pain, and improvements in sleep quality, self-efficacy, functional mobility, and balance. Importantly, none of the participants dropped out from the Tai Chi group. Hence, Tai Chi was found to be well tolerated and acceptable, and to produce clinically significant improvements in the symptoms of fibromyalgia.

 

Tai Chi can significantly improve the suffering from fibromyalgia while being acceptable for practice for patients in pain. It has no known negative side effects, is inexpensive and convenient to practice, and can be practiced alone or in groups. It appears to not only improve the psychological symptoms that can be produced by other mindfulness practices, but can also improve physical mobility including balance. Hence, Tai Chi would appear to be a nearly ideal treatment for fibromyalgia, either alone or in combination with other treatments.

 

So, improve fibromyalgia with Tai Chi.

Abstract

Previous researchers have found that 10-form Tai chi yields symptomatic benefit in patients with fibromyalgia (FM). The purpose of this study was to further investigate earlier findings and add a focus on functional mobility. We conducted a parallel-group randomized controlled trial FM-modified 8-form Yang-style Tai chi program compared to an education control. Participants met in small groups twice weekly for 90 min over 12 weeks. The primary endpoint was symptom reduction and improvement in self-report physical function, as measured by the Fibromyalgia Impact Questionnaire (FIQ), from baseline to 12 weeks. Secondary endpoints included pain severity and interference (Brief Pain Inventory (BPI), sleep (Pittsburg sleep Inventory), self-efficacy, and functional mobility. Of the 101 randomly assigned subjects (mean age 54 years, 93 % female), those in the Tai chi condition compared with the education condition demonstrated clinically and statistically significant improvements in FIQ scores (16.5 vs. 3.1, p=0.0002), BPI pain severity (1.2 vs. 0.4, p=0.0008), BPI pain interference (2.1 vs. 0.6, p=0.0000), sleep (2.0 vs. −0.03, p=0.0003), and self-efficacy for pain control (9.2 vs. −1.5, p=0.0001). Functional mobility variables including timed get up and go (−.9 vs. −.3, p=0.0001), static balance (7.5 vs. −0.3, p= 0.0001), and dynamic balance (1.6 vs. 0.3, p=0.0001) were significantly improved with Tai chi compared with education control. No adverse events were noted. Twelve weeks of Tai chi, practice twice weekly, provided worthwhile improvement in common FM symptoms including pain and physical function including mobility. Tai chi appears to be a safe and an acceptable exercise modality that may be useful as adjunctive therapy in the management of FM patients.

Practising the ancient martial art of Tai Chi is so beneficial to elderly people’s health that it should be “the preferred mode of training”, according to scientists.” – The Telegraph

 

We celebrate the increasing longevity of the population. But, aging is a mixed blessing. The aging process involves a systematic progressive decline of the body and the brain. Every system in the body deteriorates including motor function with a decline in strength, flexibility, and balance. It is inevitable. In addition, many elderly experience withdrawal and isolation from social interactions. There is some hope as there is evidence that these declines can be slowed. For example, a healthy diet and a regular program of exercise can slow the physical decline of the body with aging. Also, contemplative practices such as meditation, yoga, and tai chi or qigong have all been shown to be beneficial in slowing or delaying physical and mental decline.

 

Tai Chi has been practiced for thousands of years with benefits for health and longevity. Tai Chi training is designed to enhance function and regulate the activities of the body through regulated breathing, mindful concentration, and gentle movements. Only recently though have the effects of Tai Chi practice been scrutinized with empirical research. But, it has been found to be effective for an array of physical and psychological issues. It appears to strengthen the immune systemreduce inflammation and increase the number of cancer killing cells in the bloodstream. Because Tai Chi is not strenuous, involving slow gentle movements, and is safe, having no appreciable side effects, it is appropriate for all ages including the elderly and for individuals with illnesses that limit their activities or range of motion. Tai Chi has been shown to help the elderly improve attentionbalance, reducing fallsarthritiscognitive functionmemory, and reduce age related deterioration of the brain.

 

In today’s Research News article “Effects of tai chi qigong on psychosocial well-being among hidden elderly, using elderly neighborhood volunteer approach: a pilot randomized controlled trial.” See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC5221552/, Chan and colleagues recruited “hidden elderly” participants, who were over 60 years of age and did not participate in any social activities. They were randomly assigned to either a control condition which received usual care or to receive Tai Chi practice for 60 minutes, twice a week for 3 months and were encouraged to practice at home for 30 minutes daily. They were measured before and after the 3-month practice period and 3 months later for their social network, social support, loneliness, mental health, self-esteem, and health quality of life.

 

They found compared to baseline and the control group that the group that practiced Tai Chi had significant improvements in loneliness, social support, and their physical quality of life that persisted for the three month follow-up period. There were no adverse events and the participants indicated that they were pleased with the practice. Hence, practicing Tai Chi was safe, acceptable, and effective; significantly improving the social, psychological, and physical conditions of the “hidden elderly.”

 

It is not clear that Tai Chi practice per se produced the benefits. Since the practice was twice a week in a group condition, these individuals who did not participate in social activities prior to the study, were thrust into a social context as a prerequisite for participation. Hence, the improvement in their social condition and their loneliness may be simply due to the required social participation. In future research, there needs to be a control condition with comparable social participation to ascertain if Tai Chi practice or simply social participation was responsible for the improvements.

 

Regardless of whether Tai Chi was responsible or not, getting the “hidden elderly” out of their isolated situation and engaging in social activities is important for their psychological and physical well-being. Tai Chi is safe and acceptable for the elderly, is a light exercise, can be practiced virtually anywhere at little of no cost, and promotes social engagement. So, it would seem to be an almost ideal vehicle to promote the well-being of the elderly.

 

So, improve well-being in the elderly with Tai Chi.

Abstract

Purpose

To test the feasibility and preliminary effectiveness of a tai chi qigong program with the assistance of elderly neighborhood volunteers in strengthening social networks and enhancing the psychosocial well-being of hidden elderly.

Patients and methods

“Hidden elderly” is a term used to describe older adults who are socially isolated and refuse social participation. This pilot randomized controlled trial recruited 48 older adults aged 60 or above who did not engage in any social activity. They were randomized into tai chi qigong (n=24) and standard care control (n=24) groups. The former group underwent a three-month program of two 60-minute sessions each week, with the socially active volunteers paired up with them during practice. Standard care included regular home visits by social workers. Primary outcomes were assessed by means of the Lubben social network and De Jong Gieveld loneliness scales, and by a revised social support questionnaire. Secondary outcomes were covered by a mental health inventory and the Rosenberg self-esteem scale, and quality of life by using the 12-Item Short Form Health Survey. Data was collected at baseline, and at three and six months thereafter.

Results

The generalized estimating equations model revealed general improvement in outcomes among participants on the tai chi qigong program. In particular, participants reported a significantly greater improvement on the loneliness scale (B=−1.32, 95% confidence interval [CI] −2.54 to −0.11, P=0.033) and the satisfaction component of the social support questionnaire (B=3.43, 95% CI 0.10–6.76, P=0.044) than the control group.

Conclusion

The pilot study confirmed that tai chi qigong with elderly neighborhood volunteers is a safe and feasible social intervention for hidden elderly. Its potential benefits in improving social and psychological health suggest the need for a full-scale randomized controlled trial to reveal its empirical effects.

Parkinson’s Disease (PD) is an incurable progressive degenerative disease of the central nervous system. The condition is caused by the death of nerve cells in the brain that produce the neurotransmitter dopamine. There are around seven million people worldwide and one million people in the U.S. living with PD and about 60,000 people are diagnosed with PD every year. PD is associated with aging as the vast majority of patients are diagnosed after age 50. In fact, it has been speculated that everyone would eventually develop PD if they lived long enough.

 

Its physical symptoms include resting tremor, slow movements, muscle rigidity, problems with posture and balance, loss of automatic movements, and slurring of speech. PD itself is not fatal but is often associated with related complications which can reduce life expectancy, such as falls, choking, and cardiovascular problems. Parkinson’s Disease (PD) also has psychological effects, especially cognitive decline, anxiety, and depression. All of these symptoms result in a marked reduction in the quality of life.

 

There are no cures for Parkinson’s Disease or even treatments to slow its progression. There are only treatments that can produce symptomatic relief. So, there is a need to discover new and different treatments. Mindfulness training has been found to improve the psychological symptoms and the quality of life with PD patients. Tai Chi and Qigong are ancient mindfulness practices involving slow prescribed movements. They are gentle and completely safe, can be used with the elderly and sickly, is inexpensive to administer, can be performed in groups or alone, at home or in a facility or even public park, and can be quickly learned. The parameters of Tai Chi and Qigong practice that maximize its benefits for the relief of the symptoms of Parkinson’s Disease have not been explored. In today’s Research News article “The Effects of Group-Based versus Individual-Based Tai Chi Training on Nonmotor Symptoms in Patients with Mild to Moderate Parkinson’s Disease: A Randomized Controlled Pilot Trial.” See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC5546134/, Yang and colleagues investigate the relative effectiveness of Tai Chi practiced alone or in group contexts. They recruited patients with Parkinson’s Disease and randomly assigned them to practice Tai Chi for 45 minutes, 3 times per week, for 13 weeks either alone or in a group of 6 to 7 participants. Home practice was also prescribed. They were measured before and after training for cognitive ability, depression, and the non-motor symptoms of Parkinson’s Disease including cardiovascular, sleep/fatigue, mood/cognition, perceptual problems, attention/memory, gastrointestinal, urinary, and sexual function.

 

They found that after training both groups showed significant improvement in the overall non-motor symptoms of Parkinson’s Disease and particularly in sleep. Only the group Tai Chi practice participants, however, showed a significant improvement in cognitive (thinking) performance. In addition, the group Tai Chi practice participants had significantly greater compliance with home practice. Hence, Tai Chi regardless of whether practiced in groups of individually improved the non-motor symptoms of Parkinson’s Disease, but practicing it in groups appears to produce an additional improvement in the ability of the patients to process information (Cognition) and improve their likelihood of practicing at home.

 

The present results, as prior research, make it clear that Tai Chi practice is helpful in relieving the symptoms of Parkinson’s Disease. But, the results also make it clear that practicing in a group may be superior to practicing alone. Group Tai Chi practice may be more fun and engaging, potentiating its effectiveness. Its effectiveness for cognition is particularly important as cognitive decline is a common symptom of the progression of Parkinson’s Disease and complicates the patient’s ability to carry on normal life functions.

 

So, improve Parkinson’s Disease with group Tai Chi practice.

Abstract

Objective

To compare the effects of group-based and individual-based Tai Chi training on nonmotor symptoms in patients with mild to moderate Parkinson’s disease.

Design

Randomized controlled pilot study.

Methods

36 community-dwelling patients with Parkinson’s disease (PD) were randomly assigned to either group-based training group (n = 19) or individual-based group (n = 17). Both groups received same content of Tai Chi training 3 times a week for 13 weeks. Participants were also asked to perform home exercises daily. The Non-Motor Symptoms Scale was used to assess global nonmotor symptoms change. Sleep quality, depression, and cognition were evaluated by Parkinson’s Disease Sleep Scale, Hamilton Depression Scale, and Beijing version-Montreal Cognitive Assessment, respectively. Home exercise compliance was recorded.

Results

There was no significant difference between two groups at baseline. After 13 weeks, there were no statistical significance between two groups. However, the within-group effect was different. Participants in group-based and individual-based groups showed a significant improvement on global nonmotor symptoms (P < 0.001, P = 0.004) and sleep (P < 0.001, P < 0.001). But only group-based training patients presented a significant improvement in cognitive impairment compared with baseline (P = 0.002, P − 0.116). For depression, no group gained a significant improvement(P = 0.123, P = 0.170). Group-based participants had a higher home-exercise compliance rate (HeCR) than individual-based participants did (P = 0.019), and HeCR showed a moderate correlation with MoCA-BJ and NMSS scores changes in this study.

Conclusion

Group-based Tai Chi training is considered to be a more effective and a more labor-saving method in the clinical settings, and patients tend to have a higher compliance rate in their home exercise program

Receiving a diagnosis of cancer has a huge impact on most people. Feelings of depression, anxiety, and fear are very common and are normal responses to this life-changing and potentially life-ending experience. These feeling can result from changes in body image, changes to family and work roles, feelings of grief at these losses, and physical symptoms such as pain, nausea, or fatigue. People might also fear death, suffering, pain, or all the unknown things that lie ahead. So, coping with the emotions and stress of a cancer diagnosis is a challenge and there are no simple treatments for these psychological sequelae of cancer diagnosis.

 

But cancer diagnosis is not necessarily a death sentence. Over half of the people diagnosed with cancer are still alive 10 years later and this number is rapidly increasing. It is estimated that 15 million adults and children with a history of cancer are alive in the United States today. But, surviving cancer carries with it a number of problems. “Physical, emotional, and financial hardships often persist for years after diagnosis and treatment. Cancer survivors are also at greater risk for developing second cancers and other health conditions.” National Cancer Survivors Day.

 

Mindfulness training has been shown to help with cancer recovery and help to alleviate many of the residual physical and psychological symptoms, including stress,  sleep disturbance, and anxiety and depression.. Tai Chi or Qigong practice has been shown to improve quality of life, reduce fatigue, and lower blood pressure and cortisol levels.  In today’s Research News article “Qigong in Cancer Care: Theory, Evidence-Base, and Practice.” See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC5597070/, Klein reviews and summarized the published research literature on the application of Qigong practice for the treatment of patients with cancer. She defined Qigong practice as “Qigong refers to all skills of mind-body exercises that integrate breathing adjustment, body adjustment and mind adjustment into one.”

 

She reports that Qigong practice appears to strengthen the immune system and decrease the inflammatory responses; processes that tend to exacerbate the progression of cancer. In addition, Qigong practice appears to affect the DNA itself, preserving the telomeres, which are associated with cellular health. Qigong practice by cancer patients appears to improve the patients’ quality of life, reduce the fatigue that often accompanies cancer treatment, strengthen immune function, reduce cortisol levels, a stress marker, and reduce the cognitive impairments that frequently accompany cancer treatment. These benefits are present regardless of the variant of Qigong that is practiced. Finally, she reports that the research literature reports that Qigong practice is safe, with no known negative side effects.

 

The research suggests that Qigong practice is a safe and effective treatment for cancer patients. It appears to produce physiological changes that promote the individual’s ability to fight the cancer, mitigate the negative effects of cancer treatments, and overall improve the individual’s ability to carry on with their lives. Also, Qigong is a gentle practice, completely safe, can be used by anyone, including the elderly and sickly, is inexpensive to administer, is convenient as it can be performed in groups or alone, at home or in a facility or even public park, and can be quickly learned. In addition, it can also be practiced in social groups without professional supervision. This can make it fun, improving the likelihood of long-term engagement in the practice.

 

So, improve the physiology and quality of life with cancer with Qigong.

Abstract

Background: The purpose of this discussion is to explore the theory, evidence base, and practice of Qigong for individuals with cancer. Questions addressed are: What is qigong? How does it work? What evidence exists supporting its practice in integrative oncology? What barriers to wide-spread programming access exist? Methods: Sources for this discussion include a review of scholarly texts, the Internet, PubMed, field observations, and expert opinion. Results: Qigong is a gentle, mind/body exercise integral within Chinese medicine. Theoretical foundations include Chinese medicine energy theory, psychoneuroimmunology, the relaxation response, the meditation effect, and epigenetics. Research supports positive effects on quality of life (QOL), fatigue, immune function and cortisol levels, and cognition for individuals with cancer. There is indirect, scientific evidence suggesting that qigong practice may positively influence cancer prevention and survival. No one Qigong exercise regimen has been established as superior. Effective protocols do have common elements: slow mindful exercise, easy to learn, breath regulation, meditation, emphasis on relaxation, and energy cultivation including mental intent and self-massage. Conclusions: Regular practice of Qigong exercise therapy has the potential to improve cancer-related QOL and is indirectly linked to cancer prevention and survival. Wide-spread access to quality Qigong in cancer care programming may be challenged by the availability of existing programming and work force capacity.

Tai Chi Softball Training requires practitioners to hold a racket and control a softball on the surface of the racket while performing Tai Chi. Participants were measured before, at 7 weeks, and after training for lower limb strength and balance, and upper limb shoulder mobility, handgrip strength, and fine motor control.

 

They found that while the control group had deterioration in all measures, the participants in Tai Chi Softball Training had significant improvements in these same measures, including fine motor control, fine motor function, handgrip strength, hand and forearm strength, shoulder mobility, leg strength, and dynamic balance. The practice was found to be safe, as there were no significant adverse effects observed for participation in Tai Chi Softball Training.

 

These are wonderful results demonstrating that Tai Chi Softball Training is very effective in improving physical functional health in the elderly. This is particularly important as the progressive decline in motor ability in this group impacts their quality of life, health, and even their longevity. It would be interesting in future research to compare Tai Chi Softball Training to regular Tai Chi practice and other exercise programs to determine if one is superior to the others. We celebrate the increasing longevity of the population. But, aging is a mixed blessing. The aging process involves a systematic progressive decline of the body and the brain. Every system in the body deteriorates including motor function with a decline in strength, flexibility, and balance. It is inevitable. In addition, many elderly experience withdrawal and isolation from social interactions. There is some hope as there is evidence that these declines can be slowed. For example, a healthy diet and a regular program of exercise can slow the physical decline of the body with aging. Also, contemplative practices such as meditation, yoga, and tai chi or qigong have all been shown to be beneficial in slowing or delaying physical and mental decline.

Tai Chi is an ancient Chinese practice involving mindfulness and gentle movements. It is easy to learn, safe, and gentle. Tai Chi has been practiced for thousands of years with benefits for health and longevityTai Chi training is designed to enhance function and regulate the activities of the body through controlled breathing, mindful concentration, and gentle movements. Only recently though have the effects of this practice been scrutinized with empirical research. This research has found that it is effective for an array of physical and psychological issues. The nervous system is a dynamic entity, constantly changing and adapting to the environment. It will change size, activity, and connectivity in response to experience. These changes in the brain are called neuroplasticity. Over the last decade neuroscience has been studying the effects of contemplative practices on the brain and has identified neuroplastic changes in widespread areas. In other words, mindfulness practice appears to mold and change the brain, producing psychological, physical, and spiritual benefits. Hence, it would appear likely that Tai Chi practice may alter the brain networks underlying mindfulness.

Wei and colleagues recruited healthy long-term practitioners of Tai Chi (average 14 years of practice) and age, gender, and education matched non-practitioners. All participants performed a flanker task, a measure of executive cognitive function, in which the participant had to respond to the direction of an arrow surrounded by distracting arrows. They found that the longer the practitioners had been practicing Tai Chi the faster they responded in the flanker, executive function, task. This suggests that Tai Chi practice enhances cognitive function.

 

The participants then underwent functional Magnetic Resonance Imaging (fMRI) brain scanning. The brain scans revealed that the Tai Chi practitioners had a significant reduction in amplitude of low frequency fluctuations in the brain areas called the Default Mode Network (DMN) which underlies mind wandering and self-referential thinking. There was also a significant reduction in amplitude of low frequency fluctuations in the brain areas called the frontoparietal network (FPN) and the dorsal prefrontal-angular network which are associated with cognitive control and executive function. These changes in the low frequency fluctuations suggest that Tai Chi practice produces changes in these networks increasing their functional connectivity.

 

These results are very interesting and suggest that Tai Chi practice can produce changes in the brain improving the connectivities within large-scale neural systems. At least in terms of the frontoparietal network (FPN) and the dorsal prefrontal-angular network these changes may underlie the ability of Tai Chi practice to improve cognitive control and executive function while the changes in the Default Mode Network (DMN) may underlie the ability of Tai Chi practice to reduce mind wandering and self-referential thinking. Hence, Tai Chi practice alters the nervious system and produces very beneficial effects

 

So, alter brain networks and cognitive ability with Tai Chi.

Abstract

Cognitive control impairment is a typical symptom largely reported in populations with neurological disorders. Previous studies have provided evidence about the changes in cognitive control induced by mind-body training. However, the neural correlates underlying the effect of extensive mind-body practice on cognitive control remain largely unknown. Using resting-state functional magnetic resonance imaging, we characterized dynamic fluctuations in large-scale intrinsic connectivity networks associated with mind-body practice, and examined their differences between healthy controls and Tai Chi Chuan (TCC) practitioners. Compared with a control group, the TCC group revealed significantly decreased fractional Amplitude of Low Frequency Fluctuations (fALFF) in the bilateral frontoparietal network, default mode network and dorsal prefrontal-angular gyri network. Furthermore, we detected a significant association between mind-body practice experience and fALFF in the default mode network, as well as an association between cognitive control performance and fALFF of the frontoparietal network. This provides the first evidence of large-scale functional connectivity in brain networks associated with mind-body practice, shedding light on the neural network changes that accompany intensive mind-body training. It also highlights the functionally plastic role of the frontoparietal network in the context of the “immune system” of mental health recently developed in relation to flexible hub theory.

Schizophrenia is the most common form of psychosis. It effects about 1% of the population worldwide. It appears to be highly heritable and involves changes in the brain. It is characterized by positive symptoms such as hallucinations; seeing and, in some cases, feeling, smelling or tasting things that aren’t there, or delusions; unshakable beliefs that, when examined rationally, are obviously untrue. It is also characterized by negative symptoms involving a reduced ability to function normally, neglect of personal hygiene, lack of emotion, blank facial expressions, speaking in a monotone, loss of interest in everyday activities, social withdrawal, an inability to experience pleasure, and a lack of insight into their symptoms. The symptoms of schizophrenia usually do not appear until late adolescence or early adulthood.

 

Schizophrenia is very difficult to treat with psychotherapy and is usually treated with antipsychotic drugs. These drugs, however, are not always effective, sometimes lose effectiveness, and can have some difficult side effects. In addition, even when effective, antipsychotic drugs only treat the positive symptoms of schizophrenia, leaving the negative symptom intact including the loss of functionality and a lack of insight. Hence, there is a need for safe and effective alternative treatments for schizophrenia that can treat the negative symptoms.

 

Mindfulness training has been shown to be beneficial for a variety of mental health problems, including anxietydepressionAntisocial Personality DisorderBorderline personality disorderimpulsivityobsessive compulsive disorderphobiaspost-traumatic stress disorder, sexual dysfunction, and suicidality. It also appears to be helpful with psychosis. Mindfulness has also been shown to associated with lower symptom severity of schizophrenia. This suggests that mindfulness training may be an effective treatment for schizophrenia, including negative symptoms.  Tai Chi practice includes mindfulness training and also gentle physical exercise which may also be beneficial. Hence, it would seem reasonable to examine the ability of Tai Chi practice in treating the symptoms of schizophrenia.

 

In today’s Research News article “Tai Chi for Schizophrenia: A Systematic Review.” See summary below or view the full text of the study at:

https://www.ncbi.nlm.nih.gov/pmc/articles/PMC5434269/ Zheng and colleagues review and perform a meta-analysis of published research studies examining the effectiveness of Tai Chi practice in treating schizophrenia. They found 6 published randomized controlled trials. They found that compared to controls Tai Chi practice produced significant reductions in the negative symptoms of schizophrenia but not the positive symptoms. In addition, they report that there is a significant improvement in social function.

 

These are important findings as antipsychotic drugs only improve the positive symptoms of schizophrenia and not the negative symptoms. Since, Tai Chi practice appears to improve the negative symptoms and not the positive symptoms, it would seem to be acting in a completely different way and by a different mechanism. Tai Chi is most frequently practiced socially and this may account for the improved social function. In addition, Tai Chi practice might be the perfect adjunctive treatment. The combination of Tai Chi practice and antipsychotic drugs should improve all of the symptoms of schizophrenia.

 

So, improve the negative symptoms of schizophrenia with Tai Chi.

Abstract

Background

Tai Chi as a form of moderate aerobic exercise originating in China, could promote balance and healing of the mind-body. Furthermore, Tai Chi has been used as an adjunctive treatment for patients with schizophrenia. However, no meta-analysis or systematic review on adjunctive Tai Chi for patients with schizophrenia has yet been reported.

Aim

A systematic review and meta-analysis was conducted to examine the efficacy of Tai Chi as an adjunctive treatment for schizophrenia using randomized controlled trial (RCT) data.

Method

Two evaluators independently and systematically searched both English- and Chinese-language databases for RCTs of Tai Chi for schizophrenia patients, selected studies, extracted data, conducted quality assessment and data synthesis. Statistical analyses were performed using the Review Manager (version 5.3). The Cochrane Grades of Recommendation, Assessment, Development, and Evaluation (GRADE) was used to assess the strength of the evidence.

Results

In 6 RCTs conducted in mainland China and Hong Kong, there were 483 participants including 215 subjects in the intervention group and 268 subjects in the control group. The trials lasted 16.0 (6.2) weeks. Compared to control group, we found significant differences regarding improvement of negative symptoms assessed by the Positive and Negative Syndrome Scale (PANSS) negative symptom sub-score (2 trials) and Scale for the Assessment of Negative Symptoms (SANS) (3 trials) over the study period in the intervention group (5 trials with 6 treatment arms, n=451, SMD: -0.87 (95%CI: -1.51, -0.24), p=0.007; I2=90%). Furthermore, there is no significant difference regarding improvement of positive symptoms assessed by the PANSS positive symptom sub-score (2 trials) and Scale for the Assessment of Positive Symptoms (SAPS) (2 trials) over the study period (4 trials with 5 treatment arms, n=391, SMD: -0.09 (95%CI: -0.44, 0.26), p=0.60; I2=65%). All included RCTs did not report side effects. Based on the GRADE, the strength of the evidence for primary outcome was ‘very low’.

Conclusions

The data available on the effectiveness of adjunctive Tai Chi in patients with schizophrenia who are receiving antipsychotic is insufficient to arrive at a definitive conclusion about its efficacy. Furthermore, follow-up time in the available studies was relatively short, and all studies did not use blinded assessment of outcome measures. High-quality randomized trials are needed to inform clinical recommendations.

Qigong and Tai Chi are complex practices embedded in the complexities of life. Research has not begun to address what components of these practices interact with which aspects of health behavior to improve overall well-being. In today’s Research News article “Curriculum, Practice, and Diet Predict Health Among Experienced Taiji and Qigong Practitioners.” See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC4761851/, Komelski and colleagues surveyed on the web Tai Chi practitioners with at least 4 years of practice experience to investigate the components of the practice that are most associated with health.

 

The participants answered questions concerning their general health, They measured the frequency of the Tai Chi practice and its’ complexity by the number of four different components included in the participants practice including; (1) stillness practices, (2) iterative practices, such as a mantra, (3) choreography, sequential Tai Chi movements; and (4) partner and prop training. They also measured self-reported diet quality on a scale from ”“poor” indicated daily intake of fast foods, junk foods, and frequent overeating and “excellent” indicated a diet composed mostly of fresh fruits and vegetables, healthy sources of protein and calcium, whole grains, rare consumption of fast or junk foods, and infrequent overeating.”

 

They found that the higher the frequency and complexity of the Tai Chi practice and the diet quality the higher the health status of the practitioners. But there were also interactions found between the factors. In particular, when practice frequency was high, its complexity did not matter as a predictor of health status, but when practice frequency was low, the lower the complexity, the worse the health status. In addition, when diet quality was low, practice complexity did not matter as a predictor of health status, but when diet quality was high, the greater the complexity, the better the health status.

 

Remarkably, even though there was a wide range of participant ages, from 24 to 83 years, there was no relationship found between age and health status. Obviously, as people get older there are generally more health problems, but not in this sample of Tai Chi practitioners. This may indicate that Tai Chi practice counteracts the negative effects of aging on health. Indeed, research suggests that Tai Chi practices reduced the cognitive decline, decline in balance, increased blood pressure, and brain deterioration with aging. But, it is also possible that since the participants self-rated their overall health they may have been rating it against their expected health for their age.

 

These findings are interesting but must be interpreted cautiously as these were correlative results and causation cannot be concluded. The results do, however, provide some insights into the relationships of the components of Tai Chi practice with the participants health, the more time practice occurred each week and the more complex the practice the better the health of the practitioner. This suggests that Tai Chi practice may have cumulative effects where the more practice of more components of practice the better the health outcomes particularly when the diet is healthy.

 

So, improve health with tai chi or qigong.

Abstract

Objective: To explore the potential influence of curriculum, frequency of practice, and dietary quality on the health of experienced Taiji and qigong practitioners.

Design: Theoretical and cross-sectional study.

Methods: Responses from a volunteer sample of Taiji practitioners from across the United States were collected using an online survey. The instrument was designed to collect data on health-related quality of life, diet, and Taiji practice regimens. All experienced (≥4 years) practitioners (n = 94; mean age, 55.82 years [range, 24–83 years]) were included in the analysis. Relationships among self-reported health, diet, experience, practice frequency, and curricular complexity were analyzed.

Results: Practitioners’ health status did not show the typical negative association with age and was positively associated with complex curricula, practice, and high-quality diets. Significant interaction effects were seen between (1) curricular complexity and additional practice (p < 0.05) and (2) curricular complexity and diet (p < 0.05).

Conclusions: Intervention designers, Taiji teachers, and practitioners should consider the potential influence of curricula, out-of-class practice, and healthy diets for optimizing health-related gains and minimizing age-related losses in interventions and community-based programs.

Human life is one of constant change. We revel in our increases in physical and mental capacities during development, but regret their decreases during aging. Over time there is a systematic progressive decline in every system in the body, the brain included. This includes our mental abilities which decline with age including impairments in memory, attention, and problem solving ability. It is inevitable and cannot be avoided. Using modern neuroimaging techniques, scientists have been able to view the changes that occur in the nervous system with aging. In addition, they have been able to investigate various techniques that might slow the process of neurodegeneration that accompanies normal aging. They’ve found that mindfulness practices reduce the deterioration of the brain that occurs with aging restraining the loss of neural tissue.

 

Mindfulness practices have been shown to improve cognitive processes and mood while gentle mindful movement exercises such as Tai Chi, Qigong and Baduanjin mind-body training have been shown to slow age related cognitive decline. But, most studies of mindful movement effects on cognitive decline and mood, are performed in the elderly. Hence, the effectiveness of these practices with younger people to enhance cognitive function and mood have not been extensively studied. In today’s Research News article “Baduanjin Mind-Body Intervention Improves the Executive Control Function.” See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC5233682/, Chen and colleagues studied the effects of Baduanjin mind-body practice on mood and cognitive function of college students.

 

They recruited college students and randomly assigned them to receive either 8 weeks, 5 days per week, for 90 minutes per day of Baduanjin mind-body training or progressive muscle relaxation training. The Baduanjin mind-body training consisted of 8 movements for limbs, body-trunk, and eye movements. The participants were measured before and after training for mood and cognitive executive function with a flanker task. The flanker task required the participants to respond to the direction of an arrow when distracting arrows were present alongside the target arrow. This test for attention and filtering of irrelevant information. During the flanker task their brain activity was measured with near-infrared spectroscopy which measures blood flows in the brain.

 

They found that following Baduanjin mind-body training, but not relaxation training, there was a significant decrease in the levels of depression and increase in flanker task performance with incongruous distracting stimuli. The flanker task improvement demonstrated that the Baduanjin mind-body training participants were better able to filter out distracting material. In addition, during the flanker task, with distracting incongruent stimuli present, the Baduanjin mind-body training group had an increase in blood flow in the left prefrontal cortex while the relaxation group did not. This suggests that the Baduanjin mind-body training produced a change in the brain that may underlie the improvements in mood and executive function.

 

It has been demonstrated that increases in the activity and size of the prefrontal cortex occurs with improved executive function ability. This suggests that Baduanjin mind-body training changes the brain to improve cognitive function. It is quite remarkable that such a gentle practice can produce neural changes resulting in such beneficial effects. It is also important that these effects were produced in young participants. So, the benefits of participation in mindful movement programs are not restricted to the elderly and do not just protect against cognitive decline but can improve cognitive function even for those at the peak of their abilities.

 

Baduanjin mind-body training is a gentle practice, completely safe, can be used by anyone, including the elderly and sickly, is inexpensive to administer, can be performed in groups or alone, at home or in a facility or even public park, and can be quickly learned. In addition, it can also be practiced in social groups without professional supervision. This can make it fun, improving the likelihood of long-term engagement in the practice.

 

So, improve executive function with mindful movement.

Abstract

This study aims at comparing the effects of the Baduanjin mind-body (BMB) intervention with a conventional relaxation training program on enhancing the executive function. The study also attempts to explore the neural substrates underlying the cognitive effect of BMB intervention using near-infrared spectroscopy (NIRS) technique. Forty-two healthy college students were randomly allocated into either the Baduanjin intervention group or relaxation training (control) group. Training lasted for 8 weeks (90 min/day, 5 days/week). Each participant was administered the shortened Profile of Mood States to evaluate their mood status and the flanker task to evaluate executive function before and after training. While performing the flanker task, the NIRS data were collected from each participant. After training, individuals who have participated in BMB exercise showed a significant reduction in depressive mood compared with the same measure before the intervention. However, participants in the control group showed no such reduction. The before vs. after measurement difference in the flanker task incongruent trails was significant only for the Baduanjin intervention group. Interestingly, an increase in oxygenated hemoglobin in the left prefrontal cortex was observed during the Incongruent Trails test only after the BMB exercise intervention. These findings implicate that Baduanjin is an effective and easy-to-administering mind-body exercise for improving executive function and perhaps brain self-regulation in a young and healthy population.
