Improve the Quality of Life with Aging with Tai Chi

The aging process involves a systematic progressive decline in every system in the body, the brain included. This markedly reduces the quality of life in aging individuals. There is some hope for age related decline, however, as there is evidence that it can be slowed. There are some indications that physical and mental exercise can reduce the rate of decline. For example, contemplative practices such as meditation, yoga, and Tai Chi or qigong have all been shown to be beneficial in slowing or delaying physical and mental decline with aging. The research findings have been accumulating. So, it makes sense to step back and summarize what has been learned about the effectiveness of Tai Chi for reducing the decline in quality of life during aging.

 

In today’s Research News article “Effectiveness of Tai chi exercise on overall quality of life and its physical and psychological components among older adults: a systematic review and meta-analysis.” (See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC7485323/ ) Wang and colleagues review, summarize, and perform a meta-analysis of the published research studies of the effectiveness of Tai Chi practice in reducing the age related decline in quality of life. They identified 10 published randomized controlled trials with a total of 1170 participants who were aged 50 years and older.

 

They report that the published research found that Tai Chi practice significantly improved the quality of life in older and elderly individuals. This improvement was significant particularly for physical component of quality of life. But was not significant for the psychological component. This is not surprising as Tai Chi practice has been shown to improve strength and balance and reduce the likelihood of falls in older individuals. These benefits should improve their physical well being and reduce the likelihood of injury.

 

It is a bit surprising that the psychological component of quality of life was not improved. Tai Chi is often practiced in social groups while the elderly are often isolated and have high levels of loneliness. So, it would be expected that the social interactions occurring with Tai Chi practice would improve their psychological well-being. This needs to be further researched.

 

Tai Chi is an ancient mindfulness practices involving slow prescribed movements. It is gentle and completely safe, can be used with the healthy and sickly, is inexpensive to administer, can be performed in groups or alone, at home or in a facility or even public park, and can be quickly learned. In addition, it can be practiced in social groups without professional supervision. This can make it fun, improving the likelihood of long-term engagement in the practice. This makes Tai Chi an almost ideal practice for the improvement of quality of life in aging individuals.

Abstract

With the aging of the world’s population, the quality of life of older adults is becoming more important. There are many studies on the use of Tai chi exercise, a popular form of mind-body exercise practiced by older adults. However, the effectiveness of Tai chi exercise on the quality of life of older adults is unclear. For this systematic review and meta-analysis, six databases (PubMed, CENTRAL, CINAHL, EMBASE, Scopus, CNKI) were searched in English and Chinese languages to screen for relevant randomized controlled trials (RCT), and their risk of bias was assessed by two independent reviewers. The results of quality of life, physical component, and psychological component among older adults were meta-analyzed using RevMan5.3 software. The search retrieved 2577 records. After screening, a total of 10 RCTs were included in this evaluation, with a total of 1170 participants. The meta-analysis showed that compared with the control group, Tai chi exercise had a significant impact on the overall quality of life (SMD=1.23; 95%CI: 0.56–1.98; P<0.0001), and on the physical component of quality of life (MD=5.90; 95%CI: 1.05–10.75; P=0.02), but no significant impact on the psychological component of quality of life. This study had high heterogeneity. The results of this study suggest the potential use of Tai chi exercise as an activity for increased quality of life in older adults. Future research may enhance experimental rigor and explore the rationale behind Tai chi exercise.

Low Back Pain is the leading cause of disability worldwide and affects between 6% to 15% of the population. It is estimated, however, that 80% of the population will experience back pain sometime during their lives. There are varied treatments for low back pain including chiropractic care, acupuncture, biofeedback, physical therapy, cognitive behavioral therapy, massage, surgery, opiate pain killing drugs, steroid injections, and muscle relaxant drugs. These therapies are sometimes effective particularly for acute back pain. But, for chronic conditions the treatments are less effective and often require continuing treatment for years and opiate pain killers are dangerous and can lead to abuse, addiction, and fatal overdoses.

 

Obviously, there is a need for safe and effective treatments for low back pain that are low cost and don’t have troublesome side effects. Mindfulness practices are effective in treating pain and have been shown to be safe and effective in the management of low back painYoga practice has been shown to have a myriad of health benefits. These include relief of chronic painYoga practice has also been shown to be effective for the relief of chronic low-back pain.  Other mind-body  practices such as Tai Chi  improves spinal health and reduces pain. The evidence is accumulating, so, it would seem reasonable to summarize what has been learned regarding the ability of mind-body practices to treat chronic low back pain.

 

In today’s Research News article “Effectiveness of walking versus mind-body therapies in chronic low back pain: A systematic review and meta-analysis of recent randomized controlled trials.” (See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC7458239/ ) Nduwimana and colleagues reviewed, summarized, and performed a meta-analysis of the published randomized controlled trials comparing the effectiveness of m1nd-body practices and walking for the relief of chronic low back pain. They identified 31 published trials containing a total of 3193 participants.

 

They report that the published research found that both walking and m1nd-body practices were effective in significantly reducing low back pain. Of the m1nd-body practices, yoga was particularly effective not only reducing pain but also producing significant reductions in activity limitations resulting from low back pain. On the short-term (up to 3 months) yoga produced significant pain reductions while on the intermediate-term (3 to 6 months), walking produced significant pain reductions.

 

These results are important as low back pain is ubiquitous and produces not only discomfort but also limitations on the ability of patients to conduct their lives. So, its relief greatly reduces the suffering and improves the quality of life. The comparison of mind-body practices to walking is important as both are exercises and their relative effectiveness assessed. The results suggest that yoga practice has better short-term effectiveness while walking appears to be better for the intermediate term. This further suggests that future research should explore the effectiveness of the combination of mind-body practices and walking in the treatment of low back pain.

Abstract

Purpose:

Walking and mind-body therapies (MBTs) are commonly recommended to relieve pain and improve function in patients with chronic low back pain (CLBP). The purpose of this study was to compare the effectiveness of walking and MBTs in CLBP.

Methods:

We included randomized controlled trials (RCTs) comparing walking or MBTs to any other intervention or control in adults with CLBP. Studies were identified through PubMed, Cochrane Library, PsycINFO, Scopus, and ScienceDirect databases. The research was limited to studies published in English and French between January 2008 and December 2018. Two reviewers independently selected the studies, extracted data, and assessed studies quality using the Physiotherapy Evidence Database (PEDro) scale. Statistical analyses were performed under a random-effects model. We analyzed pain and activity limitation, with the calculation of standardized mean differences and 95% confidence intervals for the different treatment effects.

Results:

Thirty one randomized controlled trials involving 3193 participants were analyzed. Walking was as effective as control interventions in the short-term and slightly superior in the intermediate term with respect to pain (Standardized mean differences (SMD)  = –0.34; 95% CI, –0.65 to –0.03; P = .03) and activity limitation (SMD = –0.30; 95% CI, –0.50 to –0.10; P = .003). In contrast, yoga was more effective than control interventions in the short term in terms of pain (SMD = –1.47; 95% CI, –2.26 to –0.68; P = .0003) and activity limitation (SMD = –1.17; 95% CI, –1.80 to –0.55; P = .0002). Yoga was no longer superior to the control interventions for pain at the 6-month follow-up.

Conclusion:

MBTs, especially yoga, seem to be more effective in the short term, and walking seems to be more effective in the intermediate term, for the relief of pain and activity limitation in patients with CLBP. A combination of walking and MBTs fits the biopsychosocial model and might be valuable therapy for CLBP throughout follow-up due to combined effects.

Schizophrenia is the most common form of psychosis. Its effects about 1% of the population worldwide. It appears to be highly heritable and involves changes in the brain. It is characterized by both positive and negative symptoms. Positive symptoms include hallucinations; seeing and, in some cases, feeling, smelling or tasting things that aren’t there, or delusions; unshakable beliefs that, when examined rationally, are obviously untrue. Negative symptoms include a reduced ability to function normally, neglect of personal hygiene, lack of emotion, blank facial expressions, speaking in a monotone, loss of interest in everyday activities, social withdrawal, an inability to experience pleasure, and a lack of insight into their symptoms. The symptoms of schizophrenia usually do not appear until late adolescence or early adulthood.

 

Schizophrenia is very difficult to treat with psychotherapy and is usually treated with antipsychotic drugs. These drugs, however, are not always effective, sometimes lose effectiveness, and can have some difficult side effects. Mindfulness training has been shown to be beneficial for a variety of mental health problems, including psychosis. Mindfulness has also been shown to associated with lower symptom severity of schizophrenia. Also, there is accumulating research that mindfulness and yoga practices may be beneficial for patients with major mental illnesses. Tai Chi and Qigong  practices have also been shown to improve the symptoms of schizophrenia. The evidence is accumulating. So, it makes sense to step back and summarize what has been learned about mind-body practices as treatments for schizophrenia.

 

In today’s Research News article “Effects of Mind-Body Exercises on Schizophrenia: A Systematic Review With Meta-Analysis.” (See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC7457019/ ) Wei and colleagues review, summarize and perform a meta-analysis of the published controlled research studies of the effectiveness of mind-body practices to improve the symptoms of schizophrenia. They included Tai Chi, Qigong, yoga, and other mindful movement practices. They identified 13 studies (11 randomized controlled trials) employing a total of 1159 patients with schizophrenia.

 

They report that the published studies found that mind-body practices produced significant improvements of both the positive and negative symptoms of schizophrenia and also depression. In addition, the greater the number of weekly mind-body sessions the greater the improvement in the positive symptoms and the greater the duration of the mind-body sessions the greater the improvements in the negative symptoms.

 

These results are impressive as schizophrenia is difficult to treat. But the results show that mind-body practices are safe and effective treatments that improve not only the positive and negative symptoms of schizophrenia but also the frequently co-occurring depression. There also appears to be a dose response effect such that the greater the frequency and duration of mind-body practices the greater the benefits. This suggests that mind-body practices should be recommended as a part of the treatment for schizophrenia.

Abstract

Background

Mind–body exercises (MBEs) have been widely accepted as a complementary therapy for the patients with low exercise tolerance. Currently, the number of experimental studies investigating the effect of MBEs for improving symptoms in people with schizophrenia is increasing. However, results are inconsistent.

Methods

We systematically reviewed and meta-analyzed the effects of mind–body exercises on schizophrenia. Seven electronic databases (Pubmed, Web of Science, PsycINFO, Embase, Cochrane Central Register of Controlled Trials [CENTRAL], CNKI and Wangfang) were screened through October 2019 and risks of bias of included studies were assessed in Review Manager 5.3.

Results

Meta-analysis on 13 studies with 1,159 patients showed moderately significant effects in favor of mind–body exercise intervention to improve positive symptoms (SMD = 0.31; 95% CI 0.01 to 0.60; p = 0.04), negative symptoms (SMD = 0.37; 95% CI 0.14 to 0.60; p = 0.002), and depression (SMD = 0.88; 95% CI 0.63 to 1.13; p<0.00001). Meta-regression analysis revealed that the improvement in positive symptoms was positively associated with the frequency of intervention (p = 0.04), while a marginally significant correlation was observed between the improved negative symptoms and duration of each session (p = 0.06).

Conclusions

This meta-analysis supports the therapeutic effects of MBEs to aid in the treatment of schizophrenia. Further studies need to incorporate rigorous design and large sample size to identify the optimal type and dose of mind–body exercise to inform clinical practices on MBEs’ recommendations for the management of schizophrenia symptoms.

Humans are social animals. This is a great asset for the species as the effort of the individual is amplified by cooperation. In primitive times, this cooperation was essential for survival. But in modern times it is also essential, not for survival but rather for making a living and for the happiness of the individual. This ability to cooperate is so essential to human flourishing that it is built deep into our DNA and is reflected in the structure of the human nervous system. Empathy and compassion are essential for appropriate social engagement and cooperation. In order for these abilities to emerge and strengthen, individuals must be able to see that other people are very much like themselves.

 

Mindfulness has been found to increase prosocial emotions such as compassion, and empathy and prosocial behaviors such as altruism.  It is not clear, however, exactly how meditation training improves empathy. Is it due to increased mindfulness or perhaps to a better ability to recognize the emotions being experienced by others. In today’s Research News article “Higher trait mindfulness is associated with empathy but not with emotion recognition abilities.” (See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC7481693/ ) Vilaverde and colleagues explored the relationships between mindfulness, empathy, and emotion recognition. They recruited healthy adults with no meditation experience and had them complete measures of mindfulness, empathy, attentional ability, and emotion recognition. Emotion recognition both from pictures of faces expression and voice recordings were measured.

 

They found that the higher the levels of mindfulness the higher the overall levels of empathy and the cognitive empathy subscale. Interestingly, mindfulness was not associated with emotion recognition. These results are a bit confusing as cognitive empathy involves the ability to recognize the emotions of others, yet there was no significant relationship between this ability and the ability to recognize emotions from facial expressions of vocal cues. Since, the measures were self-reports it is possible that the mindful participants believed that they were able to recognize others’ emotions, while they were, in fact, not able to. It is also possible that the ability to recognize emotions in pictures of faces and in voice recordings is not the same as recognizing them in an individual physically present, where multiple cues are available. Regardless, the results replicate previous findings that being mindful is associated with greater empathy.

 

So, mindfulness is associated with higher levels of empathy.

Abstract

Mindfulness involves an intentional and non-judgemental attention or awareness of present-moment experiences. It can be cultivated by meditation practice or present as an inherent disposition or trait. Higher trait mindfulness has been associated with improved emotional skills, but evidence comes primarily from studies on emotion regulation. It remains unclear whether improvements extend to other aspects of emotional processing, namely the ability to recognize emotions in others. In the current study, 107 participants (Mage = 25.48 years) completed a measure of trait mindfulness, the Five Facet Mindfulness Questionnaire, and two emotion recognition tasks. These tasks required participants to categorize emotions in facial expressions and in speech prosody (modulations of the tone of voice). They also completed an empathy questionnaire and attention tasks. We found that higher trait mindfulness was associated positively with cognitive empathy, but not with the ability to recognize emotions. In fact, Bayesian analyses provided substantial evidence for the null hypothesis, both for emotion recognition in faces and in speech. Moreover, no associations were observed between mindfulness and attention performance. These findings suggest that the positive effects of trait mindfulness on emotional processing do not extend to emotion recognition abilities.

Christmas is a Christian holiday celebrating the birth of Jesus. In fact, though, there is no historical records of the exact date of Jesus’ birth. December 25 was selected simply because it coincided with the very popular pagan celebrations of the Winter Solstice, allowing the Christians to coopt the celebration that they were unable to stop. Regardless of this history though Christmas has become a major celebration worldwide.

 

The Christian religions teach that this celebrates the birth of the savior, who came to earth to redeem us and save us from suffering. This event did not occur in a past. It occurred in the eternal present moment. We can also give birth to a savior. We can allow a personal mindfulness, a present moment awareness, to be born. If we do it can similarly be a savior. It can save us from much suffering. The suffering that we impose on ourselves.

 

The present moment contains only what is. Regrets and problems of the past don’t exist. Worries about the future don’t exist. If we accept it as it is and don’t constantly try to change it, it can bring us a relief of our worries and our woes. It can let us see the wonders of life and the happiness that is always present right here and right now. This surely would be a miraculous birth.

 

In our modern world Christmas has been commercialized and presents have become the focus. But as nice as it is to give and receive presents the joy is short-lived and can be followed almost immediately with disappointment and depression. In fact, this is the time of year when depression is rampant and the highest suicide rates of the year occurs.

 

The celebration would be much better if presence was the focus. The great sage Thich Nhat Hahn has said that the greatest gift that you can give another person is your presence. It is also true that the greatest gift that you can get is the presence of others. We’re not talking of simply being physically present, but being totally and deeply engaged with others; seeing, hearing, and interacting with them with full attention and caring. This produces a joy that lasts and warms from the inside. Indeed, the good feeling produced can last a lifetime.

 

In giving them our presence, we deeply observe and listen to them. We see all their non-verbal signals and attune our attention on what they are telling us. Without doing anything else this produces a very positive experience. How many times have we heard people tell us that they don’t feel noticed, that they don’t feel heard, and that they are not appreciated. Our presence is an antidote. We can satisfy these deep and often ignored deep needs. We can do this simply just by being there for them.

 

We may do this with the idea that we’re giving to others. But it quickly becomes obvious that this deeply affects us. The warmth radiates affecting all. Seeing the happiness in those we care about is deeply satisfying and a source of great happiness. So, in being present for others we bring them happiness and this, in turn, brings us happiness. This can produce an upward spiral of good feelings.

 

Celebrating a Christmas mindfully can amplify the good feelings and joy of the occasion. It can save us from our own self-produced suffering. It can help alleviate the suffering of others. It can bond us to others and them to us. It can make this holiday truly special.

Athletic performance requires the harmony of mind and body. Excellence is in part physical and in part psychological. That is why an entire profession of Sports Psychology has developed. “In sport psychology, competitive athletes are taught psychological strategies to better cope with a number of demanding challenges related to psychological functioning.” They use a number of techniques to enhance performance including mindfulness training. It has been shown to improve attention and concentration and emotion regulation and reduces anxiety and worry and rumination, and the physiological and psychological responses to stress. As a result, mindfulness training has been employed by athletes and even by entire teams to enhance their performance.

 

In today’s Research News article “Mindfulness Training Enhances Endurance Performance and Executive Functions in Athletes: An Event-Related Potential Study.” (See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC7474752/ ) Nien and colleagues explored the effects of mindfulness training on college athletes. They recruited healthy college athletes who had no experience with mindfulness training and randomly assigned them to either receive mindfulness training or to a wait-list control condition. Mindfulness training consisted of 2 30-minute training sessions per week for 5 weeks and included mindful breathing, mindful meditation, body scanning, mindful yoga, and mindful walking. They were measured before and after the 5-week training period for athletic endurance, cognitive function (Stroop task), and mindfulness. The Stroop Task measures attention, response speed, and behavioral inhibition.

 

To measure the nervous systems processing of information, the electroencephalogram (EEG) was recorded and evoked potentials to the stimuli onsets were measured during the Stroop Task. They focused on the N2 response in the evoked potential which is a negative going electrical response in the EEG occurring between a 2.0 to 3.5 tenths of a second following the target stimulus presentation. The N2 component is thought to reflect attentional monitoring of conflict and inhibitory control.

 

They found that in comparison to baseline and the wait-list control group, the athletes who received mindfulness training had significant increases in mindfulness, athletic endurance, and attention and behavioral inhibition accuracies on the Stroop cognitive task. They also found that the mindfulness trained athletes had significantly lower N2 responses in the evoked potential.

 

These are interesting findings but conclusions must be tempered with the knowledge that the comparison condition was passive. This opens the possibility of alternative explanations such as participant expectancy, attention, or bias effects. Nevertheless, the results suggest that mindfulness training improves the athlete’s executive function and endurance. The results of both the Stroop Task and the N2 component of the evoked potential are compatible as each measures the ability to inhibit responses in the face of informational conflict.

 

The mechanisms by which mindfulness training has these effects was not explored in the present study. But previous research has demonstrated that mindfulness training improves executive function including behavioral inhibition. Additionally, previous research has demonstrated that mindfulness training improves the physiological and psychological responses to stress. These improved responses to stress may well explain the increased athletic endurance observed in the mindfulness training athletes as endurance measures the individual’s ability to maintain function under stress.

Abstract

Mindfulness interventions have been linked to improved sport performance and executive functions; however, few studies have explored the effects of mindfulness on sport performance and executive functions simultaneously. This study sought to examine whether a mindfulness training program would affect both the endurance performance and executive functions of athletes. In addition, event-related potentials (ERPs) associated with the Stroop task were assessed to investigate the potential electrophysiological activation associated with the mindfulness training. Applying a quasiexperimental design, forty-six university athletes were recruited and assigned into a five-week mindfulness training program or a waiting list control group. For each participant, the mindfulness level, endurance performance assessed by a graded exercise test, executive functions assessed via Stroop task, and N2 component of ERPs were measured prior to and following the 5-week intervention. After adjusting for the preintervention scores as a covariate, it was found that the postintervention mindfulness level, exhaustion time, and Stroop task accuracy scores, regardless of task condition, of the mindfulness group were higher than those of the control group. The mindfulness group also exhibited a smaller N2 amplitude than the control group. These results suggest that the five-week mindfulness program can enhance the mindfulness level, endurance performance, and multiple cognitive functions, including executive functions, of university athletes. Mindfulness training may also reduce conflict monitoring in neural processes.

Stress is epidemic in the western workplace with almost two thirds of workers reporting high levels of stress at work. In high stress occupations, like healthcare, burnout is all too prevalent. Burnout is the fatigue, cynicism, emotional exhaustion, sleep disruption, and professional inefficacy that comes with work-related stress. It is estimated that over 45% of healthcare workers experience burnout. It not only affects the healthcare providers personally, but also the patients, as it produces a loss of empathy and compassion. Burnout, in fact, it is a threat to the entire healthcare system. Currently, over a third of healthcare workers report that they are looking for a new job. Hence, burnout contributes to the shortage of doctors and nurses.

 

Preventing the negative psychological consequences of stress in healthcare professionals has to be a priority. Contemplative practices have been shown to reduce the psychological and physiological responses to stress and improve well-being. Indeed, mindfulness has been shown to be helpful in treating and preventing burnoutincreasing resilience, and improving sleep. Mindfulness-Based Stress Reduction (MBSR) is a mindfulness training program that was developed to help deal with stress. MBSR  consists of discussion, meditation, yoga and body scan practices. The evidence has been accumulating regarding MBSR’s effectiveness for the treatment of healthcare professionals, so it makes sense to summarize what has been learned.

 

In today’s Research News article “The Effectiveness of Mindfulness-Based Stress Reduction on the Psychological Functioning of Healthcare Professionals: a Systematic Review.” (See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC7511255/ ) Kriakous and colleagues review and summarize the published research studies of the benefits of Mindfulness-Based Stress Reduction (MBSR) for the psychological health of healthcare workers. They identified 30 published research studies.

 

They report that the published research studies found that Mindfulness-Based Stress Reduction (MBSR) significantly reduced healthcare workers’ levels of perceived stress, anxiety, and depression and increased mindfulness and self-compassion. Mindfulness practices have been previously reported to decrease perceived stress, anxiety, and depression and increase self-compassion in a wide variety of healthy and ill individuals. The present research summarizes these benefits for stressed healthcare professionals. These benefits of mindfulness training are important for reducing burnout, improving the psychological health of healthcare workers, and maintaining the workforce, and thereby improving the levels of care delivered to patients.

 

So, improve the psychological health of health care professionals with mindfulness.

Abstract

Objectives

Burnout and occupational stress are frequently experienced by healthcare professionals (HCPs). Mindfulness-based stress reduction (MBSR) has been found to improve the psychological health outcomes of HCPs. To date, systematic reviews and meta-analyses have primarily focused upon empirical investigations into the reduction of stress amongst HCPs using MBSR and are limited to empirical studies published before December 2019. This systematic review aimed to update the current evidence base and broaden our understanding of the effectiveness of MBSR on improving the psychological functioning of HCPs.

Methods

Three electronic databases (Medline, Psych Info and Web of Science) were searched without time frame restrictions. Quantitative studies included randomised controlled trials, clinical controlled trials, pre-post designs and studies with up to a 12-month follow-up period. All studies included in the review employed a MBSR programme, standardised measures of psychological functioning and qualified HCPs as participants.

Results

Using PRISMA guidelines thirty studies were included in the review. The reviewed literature suggested that MBSR was effective in reducing HCPs experiences of anxiety, depression and stress. MBSR was also found to be effective in increasing HCP levels of mindfulness and self-compassion. However, MBSR did not appear as effective in reducing burnout or improving resilience amongst HCPs. Abbreviated MBSR programmes were found to be as effective as the traditional 8-week MBSR programmes.

Conclusions

MBSR is an effective intervention which can help improve the psychological functioning of HCPs. Recommendations include improving the overall quality of the studies by employing more robust controlled designs with randomisation, increased sample sizes with heterogeneous samples, and making active comparisons between interventions used.

Arthritis is a chronic disease that most commonly affects the joints. Depending on the type of arthritis, symptoms may include pain, stiffness, swelling, redness, and decreased range of motion. It affects an estimated 52.5 million adults in the United States. It is associated with aging as arthritis occurs in only 7% of adults ages 18–44, while 30% adults ages 45–64 are affected, and 50% of adults ages 65 or older. Due to complications associated with rheumatoid arthritis (RA), the lifespan for people with RA may be shortened by 10 years. This is due to a higher incidence of cardiovascular disease in rheumatoid arthritis (RA) patients, with the risk more than double that of non-RA individuals.

 

Obviously, there is a need to explore alternative treatments for rheumatoid arthritis. One possibility is contemplative practice. A variety of which have been shown to have major mental and physical benefits including a reduction in the inflammatory response and have been shown to improve arthritis. Indeed, yoga practice has been shown to be effective in treating arthritis. So, it makes sense to investigate the effects of yoga practice on the inflammatory response in patients with rheumatoid arthritis.

 

In today’s Research News article “Effect of an 8-Week Yoga-Based Lifestyle Intervention on Psycho-Neuro-Immune Axis, Disease Activity, and Perceived Quality of Life in Rheumatoid Arthritis Patients: A Randomized Controlled Trial.” (See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC7492675/ ) Gautam and colleagues recruited adult patients with rheumatoid arthritis and continued with their usual care. They were randomly assigned to either no-treatment or ashtanga yoga practice modified for patients with rheumatoid arthritis. They practiced 5 times per week for 2 hours for 8 weeks. The practice included postures, breathing exercises, meditation, relaxation, and personal lifestyle management. They were measured before and after training for disease activity and quality of life. They also had blood drawn and assayed for biochemical markers of inflammation.

 

They found that in comparison to baseline and the no-treatment group, the yoga group had a significant decrease in rheumatoid arthritis disease activity and a significant increase in their psychological and social quality of life, with greater effects occurring in women than in men. In addition, they found that yoga practice decreased the gene expressions of and levels of pro-inflammatory biochemical markers IL-6, TNF-α, and CTLA4, and a significant increase in TGF-β, an anti-inflammatory marker, suggesting reduced inflammation in the yoga group.

 

This study did not have an active control condition or a long-term follow-up and as such the results must be interpreted with caution. Future studies should include long-term follow-up and an active control condition such as aerobic exercise to determine if the results were due to exercise in general or specifically to yoga practice and whether the benefits were lasting. Nevertheless, the results replicate the findings of other research that yoga practice improves arthritis symptoms and reduces inflammatory responses. The results suggest that yoga practice reduces the inflammatory responses that promote the disease and thereby reduce the disease symptoms which in turn improves the patient’s quality of life. This is good news for these patients signaling that practicing yoga may help relieve their suffering and retard disease progression.

Abstract

Various external stressors and environmental challenges lead to the provocation of the immune system in autoimmune diseases like Rheumatoid arthritis (RA). The inappropriate immune response further triggers the cascade of inflammatory changes resulting in precipitation of symptoms and hampers quality of life (QOL). The underlying psycho-somatic component of the disease requires a holistic approach to its treatment dimension rather than the use of pharmacotherapy. The applicability of mind-body interventions has become essential in today’s fast-paced life. Yoga, a mind-body technique, alters the mind’s capacity to facilitate systemic functioning at multiple organ system levels. Hence, we conducted this study to evaluate the impact of 8 weeks of a yoga-based lifestyle intervention (YBLI) on psycho-neuro-immune markers, gene expression patterns, and QOL in RA patients on routine medical therapy. A total of 66 patients were randomized into two groups: yoga group or non-yoga group and were assessed for a panel of inflammatory cytokines (IL-6, IL-17A, TNF-α, and TGF-β), mind-body communicative markers (BDNF, DHEAS, β-endorphin, and sirtuin) and transcript levels of various genes (IL-6, TNF-α, NFKB1, TGF-β, and CTLA4). We assessed disease activity and QOL using the DAS28-ESR and WHOQOL-BREF questionnaire, respectively. Yoga group observed significant improvements in the levels of markers, which influenced the psycho-neuro-immune axis (p < 0.001) with an estimated effect size from small to medium range. In the yoga group, there was a significant reduction in DAS28-ESR (p < 0.001) and improvement seen in the physical health, psychological, social relationships domains (p < 0.001) of QOL, except environmental (p > 0.05). The yoga group showed downregulation of IL-6, TNF-α, and CTLA4 and upregulation of TGF-β. These results suggest that a decrease in disease activity after yoga practice is associated with a significant reduction in inflammatory cytokines, the elevation of mind-body communicative markers, and normalization of various transcript levels, which improved QOL. Thus the adoption of YBLI improves clinical outcome in RA, and decreases systemic inflammation by its beneficial effects on psycho-neuro-immune axis and normalization of dysregulated transcripts. Thus YBLI may be used for RA patients as an adjunctive therapy.

Stress is an integral part of life. But if stress, is high or is prolonged it can be problematic. It can damage our physical and mental health and even reduce our longevity, leading to premature deaths. So, it is important that we develop methods to either reduce or control high or prolonged stress or reduce our responses to it. Mindfulness practices have been found routinely to reduce the psychological and physiological responses to stress.

 

Yoga practice has been shown to have a myriad of benefits for psychological and physical health, social, and spiritual well-being. It is both an exercise and a meditative practice that stresses both mental attention to present moment, movements, breath control, and relaxation. There has, however, not been much attention paid to which components of yoga practice that are important for reducing anxiety and the physiological responses to stress.

 

In today’s Research News article “A Comparison of the Acute Effects of Different Forms of Yoga on Physiological and Psychological Stress: A Pilot Study.” (See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC7503485/ ) Marshall and colleagues recruited physically inactive healthy adult women. They were provided with 2 30-minute yoga sessions separated by 48 hours of power yoga and meditative yoga in counterbalanced order. Power yoga emphasized physical postures while meditative yoga focused on mental focus, breathing, and relaxation. Before and after each yoga session the participants were measured for state anxiety and salivary cortisol levels.

 

They found that compared to baseline after meditative yoga there was a significant reduction in anxiety and salivary cortisol while after power yoga there were no significant changes in either measure. This suggests that meditative yoga improves mood and reduces stress while power yoga does not.

 

This study replicates prior research studies that have demonstrated that yoga practice reduces anxiety and stress levels as indicated by salivary cortisol levels. Yoga practice, however, contains a number of different components. The present study separates the exercise component, postures, from the meditative components, mental focus, breathing, and relaxation. The findings suggest that it is these meditative components and not the exercise components that are responsible for yoga’s ability to improve mood and reduce stress.

 

This study should be seen as a pilot study. It was small and of short duration. Yoga practice is not usually confined to only a single brief session but usually occurs as a number of sessions over a sustained period of time. It is possible that different results might occur after several sessions. It remains for future research to examine this.

 

So, meditative yoga is superior to power yoga in reducing stress and anxiety.

Abstract

Yoga is a frequently recommended stress management strategy; however, the acute stress response to varying types of yoga are not fully clear. Thus, the purpose of this study was to compare the acute effects of meditative and power yoga on indices of physiological and psychological stress. In a crossover counterbalanced design, physically active females (n = 13; age = 20.8 yrs ± 0.8, height = 164.5 cm ± 6.1, body mass = 65.0 kg ± 13.8) who did not regularly participate in yoga or mindful training enrolled in this study. Participants completed two visits each, with a standardized instructional-video 30-min yoga session with either A) meditative (Hatha style) yoga or B) power (Vinyasa style) yoga. Prior to and immediately after each yoga bout, psychological stress was assessed using the State–Trait Anxiety Inventory (STAI) questionnaire, and salivary cortisol samples were obtained to measure indices of physiological stress. State anxiety scores were significantly lower following meditative yoga (p = 0.047) but were not different following power yoga (p = 0.625). Salivary cortisol levels were significantly lower following meditative yoga (p = 0.020) but not following power yoga (p = 0.242). Results indicate that acute engagement in meditative yoga decreases markers of psychological and physiological stress, while power yoga does not impart a significant stress-relieving benefit. Findings indicate that differing types of yoga may have various stress-relieving capabilities and should be considered by individuals seeking anxiolytic benefits.

It is a common human phenomenon that being in a social situation can be stressful and anxiety producing. This includes the anxiety that occurs with artistic performance. Most people can deal with the anxiety and can become quite comfortable. But many do not cope well and the anxiety is overwhelming, interfering with the ability to perform. Anxiety disorders have generally been treated with drugs. But there are considerable side effects and these drugs are often abused. There are a number of psychological therapies for anxiety. But, about 45% of the patients treated do not respond to the therapy. So, there is a need to develop alternative treatments. Recently, it has been found that mindfulness training can be effective for anxiety disorders.

 

A therapeutic technique that contains mindfulness training and Cognitive Behavioral Therapy (CBT) is Acceptance and Commitment Therapy (ACT). It focuses on the individual’s thoughts, feelings, and behavior and how they interact to impact their psychological and physical well-being. It then works to change thinking to alter the interaction and produce greater life satisfaction. ACT employs mindfulness practices to increase awareness and develop an attitude of acceptance and compassion in the presence of painful thoughts and feelings. ACT teaches individuals to “just notice”, accept and embrace private experiences and focus on behavioral responses that produce more desirable outcomes. It is not known whether ACT may be effective in reducing artistic performance anxiety.

 

In today’s Research News article “Examining a Group Acceptance and Commitment Therapy Intervention for Music Performance Anxiety in Student Vocalists.” (See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC7272702/ ) Clarke and colleagues recruited student vocalists who demonstrated performance anxiety and provided them with a six session group program of Acceptance and Commitment Therapy (ACT). Before and after ACT and 3 months later they were measured for music performance anxiety psychological flexibility, anxiety, depression, perceived stress, and well-being.

 

They found that after therapy there were significant reductions in music performance anxiety and psychological inflexibility and significant increases in psychological flexibility and well-being that were maintained 3 months later. Hence, Acceptance and Commitment Therapy (ACT) produced a significant improvement in the psychological well-being of student vocalists who suffered from music performance anxiety. Although not measured, these results would suggest that their vocal performances would improve.

 

This was a small pilot study without a control, comparison, condition. So, great caution must be exercised in interpreting the results. The study though makes a convincing case that a larger randomized control trial should be conducted. Mindfulness training has been shown in prior research to reduce anxiety in clinical and non-clinical populations. So, the reductions in music performance anxiety observed in the present study were not surprising and probably due to the mindfulness training delivered in ACT.

 

So, reduce performance anxiety in student vocalists with mindfulness.

Abstract

Music performance anxiety (MPA) is a distressing and persistent anxious apprehension related to musical performance. The experience of MPA forces many musicians to give up performing or develop maladaptive coping mechanisms (e.g., avoidance or substance use), which can impact their career and wellbeing. High levels of MPA in students and vocalists are reported in the literature. Vocalists present a unique challenge for clinicians in that vocal and breathing mechanisms, required for performance, are negatively impacted when anxious. Acceptance and commitment therapy (ACT) has demonstrated efficacy for the treatment of a range of psychological problems including social anxiety disorder (of which MPA may be indicated as a subtype). This study sought to investigate whether group-based ACT may be a feasible and effective intervention for MPA in Australian student vocalists and aimed to design an intervention that could be adopted by music education providers. Potential participants (N = 31) completed an online survey including demographic questions and outcome measures. Six vocal students (four females; two males; aged M = 20.33 years) with elevated MPA scores participated in the ACT for MPA group program and 3-month follow-up. Group sessions were 2 h each week for six consecutive weeks. Participants were followed up 3 months post-intervention via online survey. There was a significant increase in psychological flexibility and significant decreases in MPA and psychological inflexibility. Gains were maintained at 3-month follow-up. The current study offers preliminary evidence for the feasibility and effectiveness of a group-based ACT protocol for musicians with performance anxiety which may be incorporated into tertiary performance training curricula.
