Reduce Stress and Improve Well-Being in a Workplace with Mindfulness

Work is very important for our health and well-being. We spend approximately 25% of our adult lives at work. How we spend that time is immensely important for our psychological and physical health. Indeed, the work environment has even become an important part of our social lives, with friendships and leisure time activities often attached to the people we work with. But, more than half of employees in the U.S. and nearly 2/3 worldwide are unhappy at work. This is partially due to work-related stress which is epidemic in the western workplace. Almost two thirds of workers reporting high levels of stress at work. This stress can result in impaired health and can result in burnout; producing fatigue, cynicism, and professional inefficacy.

 

To help overcome unhappiness, stress, and burnoutmindfulness practices have been implemented in the workplace. Indeed, mindfulness practices have been shown to markedly reduce the physiological and psychological responses to stress. As a result, it has become very trendy for business to incorporate meditation into the workday to help improve employee well-being, health, and productivity. These programs attempt to increase the employees’ mindfulness at work and thereby reduce stress and burnout.

 

In today’s Research News article “A mindfulness training program based on brief practices (M-PBI) to reduce stress in the workplace: a randomised controlled pilot study.” (See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6060839/ ), Arredondo and colleagues recruited stressed employees and randomly assigned them to either be in a wait-list control group or to receive an 8-week mindfulness training program. The training occurred once a week for 1.5 hours and included daily practices. The participants were measured before and after training and 20 weeks later for mindfulness, perceived stress, self-compassion, decentering, burnout, and heart rate variability.

 

They found that in comparison to baseline and the wait-list control group the mindfulness trained group had significant decreases in perceived stress and the components of burnout of emotional exhaustion, depersonalization, and reduced personal accomplishment, and significant increases in mindfulness, self-compassion, and decentering. These differences were enduring as they were still significant at the 20-week follow-up. They also found an increase in heart rate variability indicative of reduced stress.

 

These results are very encouraging and suggest that mindfulness training can be very beneficial in reducing workplace stress levels and burnout. It also appears to improve the overall psychological well-being of the employees improving mindfulness, self-compassion, and decentering. The ability of mindfulness training to reduce stress and burnout, and to increase self-compassion and decentering have been previously observed with different participant population. The study would have been stronger had an active control group been included. But, nevertheless the findings are suggestive that mindfulness training can be quite beneficial for stressed employees.

Abstract

Work stress is a major contributor to absenteeism and reduced work productivity. A randomised and controlled study in employee-volunteers (with Perceived Stress Scale [PSS-14]>22) was performed to assess a mindfulness program based on brief integrated mindfulness practices (M-PBI) with the aim of reducing stress in the workplace. The PSS-14 of the employees before and after 8-weeks M-PBI program, as well as after a 20-week follow-up, was assessed (primary endpoint). The employees also carried the following questionnaires (secondary endpoints): Five Facet Mindfulness Questionnaire (FFMQ), Self-Compassion Scale (SCS), Experiences Questionnaire-Decentering (EQ-D), and Maslach Burnout Inventory-General Survey (MBI-GS). Heart Rate Variability (HRV) was measured during each session in a subgroup of employees (n = 10) of the interventional group randomly selected. A total of 40 employees (77.5% female median [SD] age of 36.6 [5.6] years) took part in this study: 21 and 19 in the intervention and control group, respectively. No differences in baseline characteristics were encountered between the groups. Results show a significant decrease in stress and increase in mindfulness over time in the intervention group (PSS-14 and FFMQ; p < 0.05 both). Additionally, an improvement in decentering (EQ-D), self-compassion (SCS) and burnout (MBI-GS) were also observed compared to the control group (p < 0.05 in all). HRV measurement also showed an improvement. In conclusion, a brief practices, 8-weeks M-BIP program is an effective tool to quickly reduce stress and improve well-being in a workplace.

Low Back Pain is the leading cause of disability worldwide and affects between 6% to 15% of the population. It is estimated, however, that 80% of the population will experience back pain sometime during their lives. The pain interferes with daily living and with work, interfering with productivity and creating absences. There are varied treatments for low back pain including chiropractic care, acupuncture, biofeedback, physical therapy, cognitive behavioral therapy, massage, surgery, opiate pain killing drugs, steroid injections, and muscle relaxant drugs. These therapies are sometimes effective particularly for acute back pain. But, for chronic conditions the treatments are less effective and often require continuing treatment for years and opiate pain killers are dangerous and can lead to abuse, addiction, and fatal overdoses. Obviously, there is a need for safe and effective treatments for low back pain that are low cost and don’t have troublesome side effects.

 

Pain involves both physical and psychological issues. Physically, exercise can be helpful in strengthening the back to prevent or relieve pain. Psychologically, the stress, fear, and anxiety produced by pain tends to elicit responses that actually amplify the pain. So, reducing the emotional reactions to pain may be helpful in pain management. Mindfulness practices have been shown to improve emotion regulation producing more adaptive and less maladaptive responses to emotions. Indeed, mindfulness practices are effective in treating pain and have been shown to be safe and effective in the management of low back pain. There is a need to explore the utility of mindfulness training when it is used as a supplement to opioid treatment for chronic low back pain.

 

In today’s Research News article “Cost of Opioid-Treated Chronic Low Back Pain: Findings from a Pilot Randomized Controlled Trial of Mindfulness Meditation-Based Intervention.” (See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC5836724/ ), Zgierska and colleagues recruited adults with chronic low back pain and were being treated with opioids. They were randomly assigned to either continue with only treatment as usual or receive additional mindfulness training delivered in 8-weekly 2-hour sessions with home practice. They were measured before and after training for pain severity, pain responses to heat, healthcare utilization, productivity loss, medication use, and costs associated with disability and treatment.

 

They found that in comparison to baseline and the treatment as usual control, the patients that received mindfulness training had significantly reduced pain severity and pain response to heat. In addition, the mindfulness group tended to have fewer lost days of work while the control group tended to use more opioid medication. In looking at the economic costs of opioid treatment for low back pain, they found that adding the mindfulness training did not increase overall costs. Hence, mindfulness training appears to additionally relieve chronic low back pain beyond the effects of opioid medication, yet does not cost any more.

 

So, improve opioid-treated chronic low back pain with mindfulness.

Abstract

Objective

Opioid-treated chronic low back pain (CLBP) is debilitating, costly and often refractory to existing treatments. This secondary analysis aims to pilot-test the hypothesis that mindfulness meditation (MM) can reduce economic burden related to opioid-treated CLBP.

Design

26-week unblinded pilot randomized controlled trial, comparing MM, adjunctive to usual-care, to usual care alone.

Setting

Outpatient

Participants

Thirty-five adults with opioid-treated CLBP (≥ 30 morphine-equivalent mg/day) for 3+ months enrolled; none withdrew.

Intervention

8 weekly therapist-led MM sessions and at-home practice.

Outcome Measures

Costs related to self-reported healthcare utilization, medication use (direct costs), lost productivity (indirect costs), and total costs (direct+indirect costs) were calculated for 6-month pre- and post-enrollment periods and compared within and between the groups.

Results

Participants (21 MM; 14 control) were 20% men, age 51.8 ± 9.7 years, with severe disability, opioid dose of 148.3 ± 129.2 morphine-equivalent mg/day, and individual annual income of $18,291 ± $19,345. At baseline, total costs were estimated at $15,497 ± 13,677 (direct: $10,635 ± 9,897; indirect: $4,862 ± 7,298) per participant. Although MM group participants, compared to controls, reduced their pain severity ratings and pain sensitivity to heat-stimuli (p<0.05), no statistically significant within-group changes or between-group differences in direct and indirect costs were noted.

Conclusions

Adults with opioid-treated CLBP experience a high burden of disability despite the high costs of treatment. Although this pilot study did not show a statistically significant impact of MM on costs related to opioid-treated CLBP, MM can improve clinical outcomes and should be assessed in a larger trial with long-term follow-up.

Mindfulness training has been shown to be effective in improving physical and psychological health. One reason for these benefits is that mindfulness training improves the individual’s physical and psychological reactions to stress. Stress is an integral part of life, that is actually essential to the health of the body. In moderation, it is healthful, strengthening, and provides interest and fun to life. If stress, is high or is prolonged, however, it can be problematic. It can significantly damage our physical and mental health and even reduce our longevity, leading to premature deaths. So, it is important that we develop methods to either reduce or control high or prolonged stress or reduce our responses to it.

 

Mindfulness practices have been found routinely to reduce the psychological and physiological responses to stress. The research, however, at times, involves weak research designs and suffers from lack of control for social support and experimenter and participant expectancy effects. In addition, it is not known how mindfulness training influences levels of perceived stress. In today’s Research News article “Investigating the Specific Effects of an Online Mindfulness-Based Self-Help Intervention on Stress and Underlying Mechanisms.” (See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6061241/ ),  Gu and colleagues examined the effects of mindfulness training on stress levels in a well controlled experimental design.

 

They recruited university students and staff and randomly assigned them to one of three conditions; online Mindfulness-Based Self-Help training, listening to classical music online, or to a wait list. The Mindfulness training occurred over 2 weeks with 4 times per week 10-minute online recordings and home practice. The online listening to classical music conditions paralleled the mindfulness condition in being presented over 2 weeks with 4 times per week 10-minute recorded instructions and home practice. The participants were measured before during and after the training for mindfulness, self-compassion, worry, perceived stress, how engaged was the participant in practice, and participant expectancies.

 

They found that in comparison to before training and the music and wait list conditions, the mindfulness group had significantly lower levels of perceived stress and worry and significantly higher levels of mindfulness and self-compassion. They also performed a mediation analysis to investigate whether the effects of stress may have been mediated by the effects on mindfulness, worry, and or self-compassion. They found that higher mindfulness scores produced by the mindfulness intervention were associated with lower perceived stress. Similarly, lower worry scores produced by the mindfulness intervention were associated with lower perceived stress and higher self-compassion or scores produced by the mindfulness intervention were associated with lower perceived stress. Importantly, there were no significant differences between the conditions in engagement or expectancy effects.

 

These results demonstrate that mindfulness training lowers perceived stress levels and this could not be accounted for by expectancy or engagement effects. They further demonstrated that a mindfulness intervention lowers perceived stress by increasing mindfulness and self-compassion and lowers worry. Previous research has demonstrated that mindfulness training decreases perceived stress and worry, and increases mindfulness and self-compassion. The contribution of the current study is to demonstrate that the effects were not due to experimental contaminants and that the effects on perceived stress are due to effects on all three of these variables.

Abstract

Previous research examining the effects of mindfulness-based interventions (MBIs) and their mechanisms of change has been hampered by failure to control for non-specific factors, such as social support and interaction with group members, facilitator contact and expectation of benefit, meaning that it remained possible that benefits of MBIs could have been attributable, perhaps entirely, to non-specific elements. This experimental study examined the effects of a 2-week online mindfulness-based self-help (MBSH) intervention compared to a well-matched classical music control condition and a waitlist control condition on perceived stress. This study also tested mindfulness, self-compassion and worry as mechanisms of the effects of MBSH versus both control conditions on stress. University students and staff (N = 214) were randomised to MBSH, classical music, or waitlist conditions and completed self-report measures pre-, mid- and post-intervention. Post-intervention, MBSH was found to significantly reduce stress compared to both control conditions. Bootstrapping-based mediation analyses used standardised residualised change scores for all variables, with mediators computed as change from baseline to mid-intervention, and the outcome computed as change from baseline to post-intervention. Changes in mindfulness, self-compassion and worry were found to significantly mediate the effects of MBSH versus both control conditions on changes in stress. Findings suggest that cultivating mindfulness specifically confers benefits to stress and that these benefits may occur through improving theorised mechanisms.

When an intrusive thought pops up, you let it exist in your mind without providing it any weight. You experience the thought, but don’t judge it, change it or try to make it go away. You wait until it passes instead of thinking it should or shouldn’t be there.” – OCD

 

Obsessive-Compulsive Disorder (OCD) sufferer have repetitive anxiety producing intrusive thoughts (obsessions) that result in repetitive behaviors to reduce the anxiety (compulsions). In a typical example of OCD, the individual is concerned about germs and is unable to control the anxiety that these thoughts produce. Their solution is to engage in ritualized behaviors, such as repetitive cleaning or hand washing that for a short time relieves the anxiety. The obsessions and compulsions can become so frequent that they become a dominant theme in their lives. Hence OCD drastically reduces the quality of life and happiness of the sufferer and those around them. About 2% of the population, 3.3 million people, are affected at some time in their life. Mindfulness training has been shown to be effective in treating OCD.

 

Obsessional thoughts are not restricted to those diagnosed with OCD. In fact, many normally functioning individuals have occasional obsessional intrusive thoughts. It is not known if mindfulness affects these thoughts in otherwise high functioning adults. In today’s Research News article “Which Facets of Mindfulness Protect Individuals from the Negative Experiences of Obsessive Intrusive Thoughts?” (See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6061236/ ), Emerson and colleagues examined the relationships of dispositional mindfulness with obsessional intrusive thoughts in normal adults.

 

They recruited staff, students or alumni of a university and had them complete a number of psychometric scales on-line. These scales measured obsessive compulsive symptoms, obsessional intrusive thoughts, and mindfulness. The relationships among these variables were then explored with univariate and multivariate regression analysis.

 

They found that the higher the individual’s level of dispositional mindfulness the lower the levels of obsessional intrusive thoughts, the less of an emotional reaction they had to them, the less difficulty they had controlling them, the lower the levels of dysfunctional appraisals they engaged in, the fewer the control strategies they applied, and the fewer compulsions they had. In other words, mindfulness predicted significantly lower obsessional intrusive thoughts and lessened reactions to them. They further found that the facets of mindfulness of nonjudgment, nonreactivity and acting with awareness were associated with fewer obsessional intrusive thoughts and less difficulty controlling them. In contrast, the observing facet of mindfulness was associated with a greater number of obsessional intrusive thoughts and more difficulty controlling them.

 

The results suggest that dispositional mindfulness overall tends to counteract obsessional intrusive thoughts in normal adults. This is especially true for acting toward them with awareness, treating them without judgement, and without reacting to them. But, mindfully observing these thoughts by itself tends to heighten them. These results also suggest that mindfulness training might be effective in lessening the frequency of obsessional intrusive thoughts and their effects on the individual.

 

So, lessen the frequency and impact of obsessional intrusive thoughts with mindfulness.

Obsessive intrusive thoughts (OITs) are experienced by the majority of the general population, and in their more extreme forms are characteristic of obsessive–compulsive disorder (OCD). These cognitions are said to exist on a continuum that includes differences in their frequency and associated distress. The key factors that contribute to an increased frequency and distress are how the individual appraises and responds to the OIT. Facets of mindfulness, such as nonjudgment and nonreactivity, offer an alternative approach to OITs than the negative appraisals and commonly utilised control strategies that often contribute to distress. Clarifying the role of facets of mindfulness in relation to these cognitions offers a means to elucidate individual characteristics that may offer protection from distress associated with OITs. A sample of nonclinical individuals (n = 583) completed an online survey that assessed their experiences of OITs, including frequency, emotional reaction and appraisals, and trait mindfulness. The findings from a series of multiple regression analyses confirmed that specific facets of mindfulness relating to acting with awareness and acceptance (nonjudgment and nonreactivity) consistently predicted less frequent and distressing experiences of OITs. In contrast, the observe facet emerged as a consistent predictor of negative experiences of OITs. These findings suggest that acting with awareness and acceptance may confer protective characteristics in relation to OITs, but that the observe facet may reflect a hypervigilance to OITs. Mindfulness-based prevention and intervention for OCD should be tailored to take account of the potential differential effects of increasing specific facets of mindfulness.

The program consisted of once weekly 2.5-hour sessions and assigned home practice of meditation, yoga, body scan and discussion. The participants were measured on-line before and after training for mindfulness, perceived stress, anxiety, and depression.

 

They found that following MBSR training there were significant decreases, with large effect sizes, in perceived stress and significant increases in mindfulness, including the describing, observing, acting with awareness, non-judging, and non-reacting facets. The improvements in perceived stress were significantly greater in the 38% of participants who indicated that their perceived stress was very high. Juul and colleagues compared these results to those obtained in a comparable randomized controlled trial with assigned, non-paying, participants. They found that the reductions in perceived stress were significantly larger in the current study with self-selected, paying, participants than the reduction in the comparison study.

 

These results suggest that Mindfulness-Based Stress Reduction (MBSR) is effective in relieving stress in otherwise healthy individuals. This has been previously observed. The present study, however, also suggests that the stress reduction is greatest in highly stressed individuals and in people who self-select and pay for the program. Paying for the program is thought to produce high levels of motivation and high expectations for positive benefits. These motivation and expectancy effects maybe important in producing large improvements in perceive stress.

Abstract

We aimed to evaluate the effectiveness of Mindfulness-Based Stress Reduction (MBSR) when implemented in a community setting as a self-referred and self-paid course. Pre-post changes and Cohen’s d effect sizes were calculated for questionnaire measures of mindfulness, perceived stress, and symptoms of anxiety and depression. We compared these effect sizes with those from intervention groups in randomized controlled trials (RCTs), with populations similar to our study sample. These RCTs reported significant effects of MBSR compared to control condition. MBSR was delivered in three different Danish cities by ten different MBSR teachers with various professional backgrounds and MBSR teaching experience. One hundred and thirty-two participants were included in the study: 79% were women, mean age 45 ± 10.4 years, 75% of the participants had more than 15 years of education, 38% had a Perceived Stress Scale (PSS) score≥18, and 27% had a history of mental disorder. Post MBSR, the proportion of participants with a PSS≥18 decreased by 16% points (95%CI −26 to −6), p = 0.0032. Within-group effect sizes for (i) the total study population (ii) the subgroup with PSS≥18 at baseline (iii) intervention group in reference RCTs were as follows: PSS: d = 0.50:1.47:1.00, Symptom Check List 5: d = 0.48:0.81:0.77, Five Facet Mindfulness Questionnaire: d = 0.67:1.09:1.00. Our results showed that MBSR was effective. The effects were largest among the participants reporting highest perceived stress level at baseline. Our participants were mainly women who were middle-aged, with high educational levels, and more perceived stress and a greater history of mental disorder than the general population, and who were able to seek out and pay for an MBSR course. Reaching vulnerable groups with a clear need for stress management will, however, require other implementation strategies.

Training helps people to become more aware of their thoughts, feelings, and body sensations so that instead of being overwhelmed by them, they are better able to manage them. Practising mindfulness can give more insight into emotions, boost attention and concentration, and improve relationships.” – Mental Health Foundation

 

Mindfulness training has been shown through extensive research to be effective in improving physical and psychological health and particularly with the physical and psychological reactions to stress. The vast majority of the mindfulness training techniques, however, require a certified trained therapist. This results in costs that many clients can’t afford. In addition, the participants must be available to attend multiple sessions at particular scheduled times that may or may not be compatible with their busy schedules and at locations that may not be convenient. As an alternative, online mindfulness training programs have been developed. These have tremendous advantages in decreasing costs, making training schedules much more flexible, and eliminating the need to go repeatedly to specific locations.

 

One difficulty with understanding the effects of mindfulness training is that they often contain multiple components such as training on the ideas of mindfulness, practicing mindfulness in everyday activities, meditation, chanting, body scanning, yoga, etc. It cannot be determined then what component or combination of components are responsible for the effects. It would be helpful to compare one form of training with the same training minus single components to begin to isolate what components are necessary and sufficient for the benefits.

 

In today’s Research News article “A Randomised Controlled Trial of a Brief Online Mindfulness-Based Intervention in a Non-clinical Population: Replication and Extension.” (See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6061247/ ), Cavanagh and colleagues compared a 2-week online mindfulness training containing meditation with the same training without meditation. They recruited university students and staff to participate in a “Learning Mindfulness online” course and randomly assigned them to receive either mindfulness training, mindfulness training without meditation, or a wait-list control condition.

 

The mindfulness training consisted of a 5-minute mindfulness video and a 2000-word teaching on mindfulness that recommended performing one activity per week mindfully. The training also had a daily guided walking exercise. When meditation was included it consisted of instructions on meditation and a daily 10-minute guided meditation. The participants were measured before and after training for mindfulness, perceived stress, anxiety, depression, perseverative thinking, and a daily questionnaire on the use of training components.

 

They found that in comparison to baseline and the wait-list control, both mindfulness training groups had significantly higher levels of mindfulness and significantly lower levels of perceived stress, anxiety, depression, and perseverative thinking. They also found that perseverative thinking mediated the effects of mindfulness on perceived stress, anxiety, and depression. That is mindfulness was associated with decreased perseverative thinking (worry, rumination) which was, in turn, associated with lower perceived stress, anxiety, and depression.

 

The primary findings that mindfulness training decreases perseverative thinking, perceived stress, anxiety, and depression and that rumination (perseverative thinking is an important mediator http://contemplative-studies.org/wp/index.php/category/research-news/anxiety/of the effects, are not new as have been documented repeatedly elsewhere. What is new is that a relatively brief, online, training is sufficient to produce these benefits. The fact that it could be taught exclusively online is important and suggests that mindfulness training can be implemented broadly, at low cost, and great convenience.

 

It was surprising that the inclusion of meditation in the mindfulness training did not add any extra benefits. This may suggest that training on the application of mindfulness to day to day living is the most important component of mindfulness training for producing improvements in the psychological state of otherwise healthy individuals. This suggests that it is using mindfulness in ongoing day to day activities is very important for the training to be effective.

 

So, improve psychological health with mindfulness.

Abstract

Building on previous research, this study compared the effects of two brief, online mindfulness-based interventions (MBIs; with and without formal meditation practice) and a no intervention control group in a non-clinical sample. One hundred and fifty-five university staff and students were randomly allocated to a 2-week, self-guided, online MBI with or without mindfulness meditation practice, or a wait list control. Measures of mindfulness, perceived stress, perseverative thinking and anxiety/depression symptoms within were administered before and after the intervention period. Intention to treat analysis identified significant differences between groups on change over time for all measured outcomes. Participation in the MBIs was associated with significant improvements in all measured domains (all ps < 0.05), with effect sizes in the small to medium range (0.25 to 0.37, 95% CIs 0.11 to 0.56). No significant changes on these measures were found for the control group. Change in perseverative thinking was found to mediate the relationship between condition and improvement on perceived stress and anxiety/depression symptom outcomes. Contrary to our hypotheses, no differences between the intervention conditions were found. Limitations of the study included reliance on self-report data, a relatively high attrition rate and absence of a longer-term follow-up. This study provides evidence in support of the feasibility and effectiveness of brief, self-guided MBIs in a non-clinical population and suggests that reduced perseverative thinking may be a mechanism of change. Our findings provide preliminary evidence for the effectiveness of a mindfulness psychoeducation condition, without an invitation to formal mindfulness meditation practice. Further research is needed to confirm and better understand these results and to test the potential of such interventions.

Migraine headaches are a torment far beyond the suffering of a common headache. It is an intense throbbing pain usually unilateral, focused on only one side of the head and lasts from 4 hours to 3 days. They are actually a collection of neurological symptoms. Migraines often include: visual disturbances, nausea, vomiting, dizziness, extreme sensitivity to sound, light, touch and smell, and tingling or numbness in the extremities or face. Migraines are the 8th most disabling illness in the world. While most sufferers experience attacks once or twice a month, about 4% have chronic daily headaches. Migraines are very disruptive to the sufferer’s personal and work lives as most people are unable to work or function normally when experiencing a migraine.

 

There is no known cure for migraine headaches. Treatments are targeted at managing the symptoms. Prescription and over-the-counter pain relievers are frequently used. There are a number of drug and drug combinations that appear to reduce the frequency of migraine attacks. These vary in effectiveness but unfortunately can have troubling side effects and some are addictive. Behaviorally, relaxation and sleep appear to help lower the frequency of migraines. Mindfulness practices have been shown to reduce stress and improve relaxation. So, they may be useful in preventing migraines. Indeed, it has been shown that mindfulness practice can reduce headache pain.

 

In today’s Research News article “Mindfulness for female outpatients with chronic primary headaches: an internet-based bibliotherapy.” (See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6036307/ ), Tavallaei and colleagues recruited women suffering with migraine headaches and randomly assigned them to receive either an on-line mindfulness training based upon the Mindfulness-Based Stress Reduction (MBSR) program or medical treatment as usual. The mindfulness training program was presented over the internet in 8 weekly sessions and included meditation, body scan, and didactic presentation. The participants were measured before and after treatment for mindfulness, migraine disability, pain, and distress including depression, anxiety, and stress.

 

They found that in comparison to the baseline and the treatment-as-usual control group that the group who received mindfulness training had significantly lower levels of migraine disability, distress, and pain and significantly higher levels of mindfulness. They found that the reductions in pain were due to changes in the emotional reactions to pain and not the sensory experiences of pain. So, the pain was perceived normally but the women did not react to the sensations emotionally and this resulted in a lower impact of the headache pain.

 

The results suggest that mindfulness training increases the quality of life and reduces the psychological distress of women with migraine headaches. Similar findings have been reported in other prior research studies. The importance of the present study resides in the presentation of the program over the internet. Presentation over the internet is important as in-person programs are inconvenient and expensive. Presentation over the internet allows for widespread, convenient, and inexpensive distribution of the therapy to affected populations. This makes mindfulness training more readily available to migraine sufferers.

Abstract

Our aim was to investigate effectiveness of mindfulness by bibliotherapy on disability, distress, perceived pain and mindfulness in women with tension headaches and migraines. Primary headaches have been of great interest to mental health researchers because of the high prevalence, as well as significant disability and distress in the affected people. Despite the promising results of in-person treatment and some limitations that such interventions may cause, patients may be encountered with problems when using health care services. The present study is a quasi-experimental randomized design with pre-test, post-test, and control group. The study population consisted of 1396 women with migraine headache referring to headache clinic of Baqiyatallah Hospital in Tehran. Of these, 30 patients (including tboh experimental and control group) were selected by objective sampling method and were randomly assigned to the two groups. The experimental group, in addition to medical treatment as usual, was treated for a period of 8 sessions by Mindfulness-based Stress Reduction Internet-based Bibliotherapy, but the control group used only the medical treatment. The sample had no attritions. Data were collected by the four scales of (DASS-21), Migraine Disability Assessment Test (MIDAS), McGill’s Short Form Questionnaire (MPQ-SF), and Mindfulness Inventory (MAAS). We used covariance analysis to analyze the findings in the measured scales. MBSR-IBB treatment had no significant effect on pain sensory dimension (P <0.44), despite improvement of mindfulness (P <0.0001). In contrast, the greatest effect was on the level of disability (P <0.0001). We observed also a significant improvement in distress (P <0.0001). In conclusion, in spite of the presence of headaches, the mindfulness improved the quality of life and reduced the level of mental distress. In addition, using the Internet-based bibliotherapy method, these services can be used with easier access, lower cost, and more flexibility.

Stress is epidemic in the western workplace with almost two thirds of workers reporting high levels of stress at work. In high stress occupations, like healthcare, burnout is all too prevalent. Burnout is the fatigue, cynicism, emotional exhaustion, sleep disruption, and professional inefficacy that comes with work-related stress. It is estimated that over 45% of healthcare workers experience burnout. Currently, over a third of healthcare workers report that they are looking for a new job. It not only affects the healthcare providers personally, but also the patients, as it produces a loss of empathy and compassion. Burnout, in fact, it is a threat to the entire healthcare system as it contributes to the shortage of doctors and nurses.

 

Preventing burnout has to be a priority. Unfortunately, it is beyond the ability of the individual to change the environment to reduce stress and prevent burnout, so it is important that methods be found to reduce the individual’s responses to stress; to make the individual more resilient when high levels of stress occur. Contemplative practices have been shown to reduce the psychological and physiological responses to stress. Indeed, mindfulness has been shown to be helpful in treating and preventing burnoutincreasing resilience, and improving sleep. It would be best to provide techniques to combat burnout early in a medical career. Medical residency is an extremely stressful period and many express burnout symptoms. This would seem to be an ideal time to intervene.

 

In today’s Research News article “Mind-Body Skills Training for Resident Wellness: A Pilot Study of a Brief Mindfulness Intervention.” (See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC5954313/ ), Romcevich and colleagues conducted a small pilot study of the effectiveness of a Mind-Body Skills Training (MBST) program to reduce burnout in hospital residents. They recruited 2nd through 4th year residents and had them complete 4 weekly 90-minute training sessions and 12 on-line modules including relaxation, breath following meditation, and loving kindness meditation. They were measured before and after training and 6 months later for burnout, perceived stress, resilience, mindfulness, self-compassion, and burnout subscales of emotional exhaustion, depersonalization, and personal achievement.

 

They found that after training there were significant improvements in personal achievement, perceived stress, resilience, and mindfulness and an unexpected decrease in self-compassion. At the 6-month follow-up there were significant improvements in self-compassion, depersonalization and mindfulness. Hence, the pilot results suggest that Mind-Body Skills Training (MBST) may be effective in improving well-being and decreasing symptoms of burnout in medical residents.

 

It will be necessary to replicate these results in a larger controlled randomized trial. But the pilot results are encouraging. They suggest that this form of mindfulness training may be a safe and effective method to improve well-being and intervene early in the career to prevent future burnout in medical professionals.

Abstract

Background:

Interventions to address burnout include mind-body skills training (MBST), but few studies have evaluated the feasibility of MBST for busy pediatric residents.

Objective:

In this pilot study, we tested the feasibility of a brief MBST intervention, using in-person peer-led training supported by online modules, to decrease stress and burnout in pediatric resident physicians.

Methods:

Of 99 (10%) residents, 10 residents at Nationwide Children’s Hospital in Ohio participated in up to four 90-minute MBST sessions more than 1 month, led by a co-resident with 5 years of informal training in mind-body skills. Participants were offered 8 assigned online modules through OSU Center for Integrative Health and Wellness. Measures including Maslach Burnout Inventory (MBI), Cohen’s Perceived Stress, Smith’s Brief Resilience, Cognitive and Affective Mindfulness Scale-Revised, and Neff’s Self-Compassion Scale (NSS) were administered before (T1) and after (T2) the course. Participants were offered optional monthly “maintenance” sessions for 6 months and completed a third set of measures at this follow-up (T3).

Results:

The residents completed an average of 4.3/8 online modules and attended an average of 2.8/4 in-person sessions. There was significant improvement in positive attitude, perceived stress, and resilience post intervention (T2). Follow-up evaluation (T3) also demonstrated significant improvement in burnout (depersonalization) and mindfulness. More than 75% of participants found the course worthwhile.

Conclusions:

A short mixed-method mindfulness-based skills course may be a practical way to offer resilience and stress management training to busy resident physicians.

End-stage renal disease (ESRD) is a serious and all too common medical problem that results from a total and permanent failure of the kidneys. As a result, the body retains fluid and harmful wastes build up. Treatment, usually dialysis, is required to replace the work of the failed kidneys. Kidney dialysis uses a machine to filter harmful wastes, salt, and excess fluid from your blood. This restores the blood to a normal, healthy balance. Without dialysis or a kidney transplant the ESRD patient cannot survive It is estimated that ESRD occurs in more than 650,000 patients per year in the United States and is increasing by 5% per year. Those who live with ESRD are 1% of the U.S. Medicare population but account for 7% of the Medicare budget. Worldwide there are an estimated 2 million ESRD patients.

 

End-stage renal disease (ESRD) is frequently accompanied by a number of other serious diseases, such as cardiovascular disease and diabetes. Making matters worse is the fact that ESRD patients often experience psychological distress including depression. It is possible that mindfulness training may be helpful as it has been found be helpful for patients with kidney disease, to help relieve depression and improve the patients ability to effectively deal with stress.

 

In today’s Research News article “The effect of mindfulness program on general health of patients undergoing hemodialysis.” (See summary below or view the full text of the study at: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6009148/ ), Nejad and colleagues recruited patients with End-stage renal disease (ESRD) on dialysis and randomly assigned them to receive 8 1.5 hour weekly sessions of either a mindfulness training program or an End-stage renal disease (ESRD) education program. They were measured before and after training and 1 month later for physical health, anxiety, depression, sleep disturbance, and social problems.

 

They found that in comparison to baseline and to the education control group the patients who received mindfulness training had significant improvements in all measured variables including significantly lower levels of anxiety, depression, sleep disturbance, and social problems and significantly higher levels of physical health. Importantly, these improvements were still present 1 month after the completion of training. This suggests that the benefits are enduring. A strength of this study is that the control condition was an active control which eliminates many sources of bias and contamination.

 

The study thus indicates that mindfulness training produces substantial physical and psychological benefits for patients with End-stage renal disease (ESRD) who are on dialysis. It remains for future research to clarify exactly how mindfulness training may be helping the patients. It is possible that the improved emotion regulation or ability to cope with the physical and psychological effects of stress produced by mindfulness training were responsible for the benefits for these patients. Regardless, training in mindfulness helps relieve at least some of the burden on these patients.

 

So, improve the psychological and physical health of dialysis patients with mindfulness.

Abstract

INTRODUCTION:

Patients undergoing hemodialysis have a low level of health and mindfulness technique in mind-body medicine is used to help patients with chronic illness. Therefore, this clinical trial was conducted to determine the effect of the mindfulness program on the general health of patients undergoing treatment with hemodialysis.

METHODS:

Sixty hemodialysis patients were selected through the convenient method and randomly divided into experimental and control groups. Both groups completed demographic information questionnaire and general health questionnaire. Then, the experimental group received 8 sessions of mindfulness training and the control group received 8 sessions of education in relation to end-stage renal disease and hemodialysis. Educational sessions were performed for both groups in the second 30 min after hemodialysis for 1 h in six individual sessions and two 1.5 h group sessions. Immediately after and 1 month after the intervention, the General Health Questionnaire was completed by both groups.

RESULTS:

Analysis of the findings showed no significant differences between the mean score of general health disorder and its subscales before the intervention in to two groups (P > 0.05). Nevertheless, after intervention in the experimental group, the mean of general health disorder score decreased from 44.17 ± 12.32 to 21.9 ± 6.4 and 1 month after the intervention, the mean score of this score was 23.6 ± 6.2. The mean score of physical symptoms, anxiety and sleep disorder, social functioning deficiency, and depression were also significantly lower in the experimental group immediately after intervention and 1 month after the intervention; however, there were no significant differences between the mean of this score immediately and 1 month after the intervention. In addition, the mean score of general health disorder changes immediately and 1 month after the intervention in the experimental group was significantly more than the control group. In the control group, there was no significant difference between the mean score of general health disorder and its subscales before, immediately, and 1 month after the intervention (P > 0.05).

CONCLUSIONS:

The results of this research showed that mindfulness has been effective in reducing physical and anxiety symptoms, sleep disorder, social dysfunction, and depression symptoms. Therefore, the use of mindfulness as a complementary treatment can improve the general health level in these patients.

Labor Day is a National Holiday in the United States. It was designed to celebrate the accomplishments of the American worker, particularly organized labor. It is important to celebrate this holiday mindfully. Work is a major component of our lives, it dictates our income, contributes to our social lives, and for many people is an essential part of their self-concept and their self-worth. But rather than using the holiday to reflect on this important part of their lives, most people treat Labor Day mindlessly, as a time to vacation and party. Perhaps, though, it’s important to take at least a little time on this holiday to mindfully reflect on work.

 

To understand the importance of work we need only look at the Buddha’s Noble Eightfold Path. Two of the eight components are Right Actions and Right Livelihood. But, Right Livelihood is itself an action and it would seem that Right Livelihood should be contained in Right Actions and not a separate component. But, the Buddha included Right Livelihood as a separate component to underscore its importance for spiritual development. It’s his way of emphasizing that what one does for a living is an extremely important action. The Buddha taught that it was essential for spiritual development to only engage in work that produces greater happiness, wisdom, and well-being, and relieves suffering in ourselves and others and avoid jobs that produce harm.

 

We should take a mindful look at our occupations on Labor Day and ask whether they promote greater happiness, wisdom, and well-being, and relieves suffering or produces harm. In some case, the fact that it is Right Livelihood is obvious as with professions such as physician, social worker, peace negotiator, relief worker, therapist, etc. On the other hand, professions such as drug dealer, arms merchant, professional criminal, etc. are clearly not. But for most occupations it is much more difficult to discern whether or not they constitute Right Livelihood. This is a point for deep, mindful, exploration for Labor Day.

 

Working on an oil rig in the Gulf of Mexico produces a product, energy, that is needed for the well-being of virtually everyone. Without affordable energy, every aspect of the economy would collapse. So, working on the oil rig could be seen as promoting well-being and relieving suffering. On the other hand, there is potential for great environmental harm, including oil spills that directly pollute sensitive environments, or contributing to carbon dioxide emissions that can indirectly create great harm by contributing to global warming. So should someone on the eightfold path accept or reject a job working on an oil rig? The answer cannot be given by anyone other than the individual themselves. It is imperative that this be looked at deeply and objectively to determine for themselves if they are doing more harm than good. The primary spiritual impact of Right Livelihood is on the individual engaging in the occupation. So, the decision has to be theirs. That is not to say that experts or friends can’t or shouldn’t be consulted, but that ultimately the individual must decide for themselves and be willing to accept the potential consequences. Needless to say, this should be a focus for deep mindful reflection on Labor Day.

 

The labor movement itself has important consequences for ourselves and others and should also be explored mindfully on Labor Day. It grew out of great labor abuses that existed where unscrupulous employers took advantage of workers, demanding much and paying little. This is an example how great harm can be produced when the wealthy and powerful, as a result of greed, do not practice Right Livelihood. The Buddha taught that there was nothing wrong with being successful and accumulating wealth provided that this was done ethically and honestly, and it promoted the overall well-being of the community. But, for a time, this was not the case. This underscores how the notion of Right Livelihood doesn’t only apply to workers, but also to employers, financiers, Wall Street executives, politicians, etc.

 

The labor movement arose to counteract the rampant abuses of workers. By organizing the workers obtained strength in numbers. This allowed them to stand up to employers and demand better pay and working conditions. It truly exemplifies our interdependence. We are not alone, but rather, are intricately connected to everyone else. This is true for work in general. It is a productive point for mindful contemplation of how our work and in fact, our entire lives are connected to the work of others. If we’re a truck driver we’re totally dependent upon the people who make the trucks, produce the fuel, build the roads, insures, maintains, and repairs the vehicles, makes and enforces the laws governing the roadways, etc. But, we are also dependent upon the work or those who produce our food, make our clothes, build our houses, educate our children, defend and protect us, etc. Our work is interdependent with the work of everyone else. This is an important point for reflection on Labor Day.

 

I recently received an award for my work career. It was a wonderful boost for my ego and made me feel very good about myself. But, with a little mindful reflection, I realized that this was not my award solely. It could never have been achieved without the involvement of a vast array of people, colleagues, students, friends, family superiors, workers, direct reports, police, government, etc. and all of the people who they are dependent upon, and so forth. It couldn’t have been achieved without virtually everyone. It was really an award for a cooperative effort. This kind of thinking made me humble. It made me know that it was not about “I.” Rather, it’s about “we.” Mindful reflection about our work can help us to see the interconnectedness we have with every other living thing.

 

A major issue for Labor Day reflection is what happens in the course of our daily work. We can learn much about ourselves by mindfully examining what transpires at work. What happens can bring us great joy or great suffering, but most of the time, it just provides momentary satisfaction or dissatisfaction. It is the smaller moments that compose the majority of our work lives but they are crucial to our happiness or unhappiness at work. Applying mindfulness and reflection to how we react and our thoughts regarding the events at work, we can gain great insight into the workings of our minds and how they can produce unsatisfactoriness and unhappiness.

 

The fact that your boss failed to mention that your performance was very good that day may make you feel unappreciated at work. But, it is likely that your boss was preoccupied with her own problems. But, looking carefully at your thought process you can begin to see how your response was based on the needs of your own ego. Many people’s feelings of self-worth, or self-hatred for that matter, are built around their work. Not being recognized by a superior may threaten a fragile self-image and produce discomfort and resentment. Work is actually a wonderful opportunity to learn about yourself.

 

You may observe a coworker engaged in petty theft and not report it. Looking deeply at this event you may be able to see that you have a strong need to be liked and you feel that reporting the unethical behavior may cause others to dislike you or see you as a threat. In this case your need for social acceptance causes you to compromise your integrity. The fact that social approval was more important to you than ethics can be a revelation regarding your inner psychological landscape. Once again, work can teach you a lot.

 

There are actually many many events that happen at work every day, small and large, that reveal the workings of your mind and emotions. Applying mindfulness, noticing and being aware of your reactions and actions at work can change your ideas about yourself and change your actions at work and these can lead to greater understanding and acceptance. This, in turn, can lead to greater satisfaction and happiness. Mindfulness is a key. If you are not in the present moment, if you are not paying attention but rather reacting without thinking or noticing, if your mind is wandering and off task, then this splendid opportunity will be lost. So, vow to be mindful at work and become better and happier with the way you make your living.

 

So, on this Labor Day, vow to be mindful and take advantage of the opportunities provided at work to learn about yourself. Grow as a person and grow spiritually by making every work day a mindful work day.
